The concentration of Cu, Fe, Pb, Zn and As in the surface water intended for human consumption and other uses in the Mantaro River basin were analysed using multivariate methods. The water samples were collected from seven water bodies in the Junín region in June 2019, in the low discharge period. In each body of water, a sector with six sampling sites was established. The Cu, Fe, Pb, Zn and As contents were determined by the flame atomic absorption spectrophotometry method. The correlation analysis revealed positive and significant correlations (P < 0.05) for the Zn/Pb, Pb/Fe and Zn/Fe pairs with a good association, higher than 0.80 and for the Pb/Cu, Fe/Cu, As/Pb and As/Zn pairs a weak degree of association (P < 0.05). The analysis of the main components showed three components with their own values > 1. The hierarchical grouping analysis classified the evaluated water bodies into three groups according to the concentration of the Cu, Fe, Pb, Zn and As. The high concentrations of heavy metals and arsenic recorded in the CIMIRM and MERIS irrigation channels reveal that the Mantaro River continues to be a sink for mine wastewater discharges and runoff from mining liabilities at the headwaters of the Mantaro basin. It is, therefore, necessary to implement urgent management policies to control and reduce the levels of contamination by potentially toxic metals and metalloids in the Mantaro River.
El objetivo del estudio fue determinar la coproprevalencia de Equinococcus granulosus en canes del distrito rural de San José de Queros, ubicado en la provincia de Concepción, región Junín, en el centro del Perú. El estudio fue observacional de tipo transversal, y se llevó a cabo en 2015 en los tres anexos del distrito. Se recolectaron muestras de heces de 152 canes del mismo número de hogares seleccionados al azar y se procesaron mediante la técnica de ELISA específica de Echinococcus granulosus. El 50% (Usibamba 61.0%, Chaquicocha 51.0% y San José de Quero 41.9%) de muestras fueron positivas al parásito. Asimismo, se determinó que el promedio de canes por casa fue 2.3, el 40.9% de los dueños alimentaba a los canes con vísceras frescas del ganado sacrificado, la proporción de perros que defecan fuera de casa era de 91.3%, y que el 18.4% refiere haber tenido algún familiar con quiste hidatídico. Se concluye que la infestación canina por E. granulosus es frecuente en el distrito San José de Quero, la cual es facilitada por los hábitos de los dueños y los comportamientos epidemiológicos del can.
La Cystoisospora belli, antes denominada Isospora belli, es el agente etiológico de la cystoisosporiasis, una infección oportunista que afecta a pacientes inmunodeprimidos, caracterizada por diarrea crónica y pérdida ponderal. La incidencia de diarrea crónica por este agente, en pacientes infectados por el VIH, ha disminuido considerablemente. Ello gracias al advenimiento de la terapia antirretroviral de gran actividad (TARGA), con la que se ha logrado mejorar la respuesta inmunológica del paciente y disminuir su carga viral. Se presentan seis casos de cystoisosporiasis recurrente y refractaria en pacientes con infección por VIH, en quienes se diagnosticó cystoisosporiasis pese a que previamente se encontraban recibiendo profilaxis con trimetropin/sulfametoxazol (TMP/SMX). Cinco de ellos evolucionaron de manera tórpida y fallecieron, a pesar de una buena respuesta al TARGA (adecuado incremento de CD4 y disminución de la carga viral hasta rangos indetectables), y de tratamiento con TMP/SMX por vía oral y otros medicamentos de segunda línea.
We report the case of a male newborn, product of a 36 week pregnancy, with diagnosis of congenital pneumonia and with a confirmatory test for influenza A (H1N1) virus, without any other suspicious contact. The mother was admitted to the hospital with respiratory failure and the history of a flu-like episode of 5 days of evolution, during the first days of the pandemic in Peru. Due to the severe evolution of the respiratory process, assisted ventilation was given to her and then a cesarean section was performed due to acute fetal distress and oligoamnios. The mother was later confirmed as a case of epidemic influenza A (H1N1) and pulmonary tuberculosis.
Objetivo: Evaluar la mortalidad en pacientes con infección por VIH/SIDA que reciben tratamiento antirretroviral (TAR) en un hospital público de Perú. Materiales y métodos: Se realizó un estudio observacional de cohorte retrospectivo de los pacientes con diagnóstico de infección por VIH/SIDA que recibieron tratamiento antirretroviral. Resultados: De 428 pacientes estudiados, la media de edad fue 37 años y la media del recuento de CD4 al inicio del TAR en el grupo de fallecidos fue 87,2 cels/uL frente a 153,2 cels/uL en el grupo de vivos, fallecieron 66 pacientes (15,4%) después de iniciar TAR. La mayoría (48,5%) de estos pacientes tenían recuento de CD4 menor de 200 cel/uL, 59,1% desarrollaron infección oportunista y 16,7% cambiaron de esquema antirretroviral en algún momento del TAR. La mortalidad durante los primeros 3 meses de TAR fue 32,6%; el análisis con múltiples variables se encontró asociación con tener CD4 mayor de 100 cel/mL (HR:0,57; IC95%: 0,29-1,11), la presencia de infección oportunista (HR: 1,1; IC95% 1,06-1,16) y tener una alta carga viral (HR 1,17; IC95%:1,07-1,48). La probabilidad de sobrevida de los pacientes con CD4 menor de 100 cel/mL a los 8 años en TAR es 68%. Conclusiones: El mayor número de muertes de los pacientes con infección por VIH/SIDA ocurre durante los tres primeros meses de TAR y está asociado a un diagnóstico tardío (alta carga viral, bajo CD4 y presencia de enfermedades oportunistas). Sugerimos la implementación de terapia supervisada durante los primeros meses de TAR en pacientes con CD4 menor de 100 cel/mL, para identificar en forma temprana las infecciones oportunistas.
The objective of the present study was to calculate the economic loss caused by the treatment of human hydatidosis. For this purpose, a cost analysis of the cases reported in the Junin region of Peru in 2013 was conducted. The total cost (direct and indirect) per patient assisted by the Ministry of Health (Ministerio de Salud-Minsa), the social security system (EsSalud), and private health care was 1,220, 3,051, and 4,090 US dollars (USD), respectively. The mean annual loss considering the number of health care services for uncomplicated cases of human hydatidosis was USD 24,542 in Minsa, USD 369,292 in EsSalud, and USD 130,566 in private health care. Overall, the rate of loss of human productivity was a small proportion. Our findings indicate that hydatidosis imposes a heavy economic burden in the Junin region of Peru.
