The association between periodontal disease and the development of atherosclerosis has been studied. The systematic review of literature aims to evaluate the association between periodontal disease and subclinical atherosclerosis. A literature search of the PubMed, Scopus, LILACS, BBO, Cochrane Library and Scielo bibliographic databases was conducted using the following descriptors: "periodontal disease", "periodontitis", "carotid", and "therosclerosis". Articles were excluded if they: presented abstracts written in languages other than Portuguese, English and Spanish; experimental studies; did not contain data testing the degree of association between periodontal disease and subclinical atherosclerosis; did measure the extent of periodontal disease by parameters other than the clinical examination; did not measure the carotid artery intima-media wall thickness by ultrasonography; and covered specific population groups. The studies design, measurement protocols and reported data were compared. A total of 63 papers identified only 10 studies that met the inclusion criteria: 1 longitudinal study, 6 cross-sectional, and 3 case control studies. There was a wide heterogeneity between the studies regarding the methods of measuring periodontal disease and atherosclerosis outcomes. The conclusion is that periodontal infections are strongly associated with the development of subclinical atherosclerosis, however the mechanisms involved on the pathogenic process remain unknown.Keywords: atherosclerosis; periodontics; carotid artery diseases. ResumoA associação entre doença periodontal e o desenvolvimento de aterosclerose tem sido pesquisada. Esta revisão sistemática da literatura se propõe a verificar a associação entre a doença periodontal e a aterosclerose subclínica. A pesquisa utilizou as bases de dados: PubMed, Scopus, LILACS, BBO, Biblioteca Cochrane e Scielo com os termos: "doença periodontal", "periodontite", "carótida" e "aterosclerose". Foram excluídos artigos: que apresentaram resumos escritos em idiomas diferentes do português, inglês e espanhol; com abordagem laboratorial em modelos experimentais; sem descrição ou referência a estimadores de associação entre doença periodontal e aterosclerose subclínica; pesquisas que realizaram a medida da doença periodontal por parâmetros diferentes de exames clínicos; pesquisas que não analisaram a espessura da íntima média da artéria carótida através de exame de ultrassom; e que abordassem grupos específicos. Foram comparados os desenhos de estudo, protocolos de medida e os dados relatados. De um total de 63 referências, apenas 10 artigos contemplavam todos os critérios de inclusão, distribuídos em: 1 estudo longitudinal, 6 transversais e 3 casos controles. The aim of this study was to investigate the relationship between periodontal disease and increased thickness of the carotid artery intima-media complex. A cross-sectional study was conducted with 220 adults (age ≥ 35 years) among participants in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). Carotid artery ultrasound and periodontal clinical examinations were conducted and included visible plaque index, gingival bleeding on probing index, probing pocket depth (PPD), and clinical attachment level (CAL). Individuals with increased carotid artery thickness showed fewer teeth and higher frequency of CAL ≥ 3mm, CAL ≥ 4mm, CAL ≥ 5mm, and CAL ≥ 6mm and PPD ≥ 4mm (p < 0.05). Despite the use of 18 definitions for periodontal disease, only one confirmed the hypothesis of an association between periodontal disease and subclinical atherosclerosis. Individuals with 10% or more sites with CAL ≥ 4mm were more likely to present carotid thickening.
Pesquisa qualitativa, exploratório-descritiva que objetiva identificar os elementos que influenciam a percepção de qualidade de vida (QV) de pessoas que vivem com HIV/Aids. Entrevistas semi-estruturadas foram realizadas com 14 pessoas. Análise resultou em quatro categorias: Qualidade de vida: subjetividade e multidimensionalidade; Viver com HIV/Aids e a qualidade de vida; O cuidado para o viver com qualidade; e Os serviços de saúde como potencializadores da QV. Existem vários fatores que interferem na QV dessas pessoas, como: aspectos físicos decorrentes da doença, o seguimento do tratamento e o uso dos antirretrovirais. Como aspectos sociais, identificou-se a presença de discriminação e preconceito. A aceitação do tratamento com medicamentos antirretrovirais melhora consideravelmente a QV. A identificação dos fatores que interferem na QV poderá contribuir para o atendimento mais humanizado às pessoas que vivem com HIV/Aids, direcionando o cuidado para além dos aspectos clínicos, instrumentalizando estas pessoas na busca por um viver com mais qualidade. This is a qualitative exploratory-descriptive work that aims to identify the elements that influence the perception of quality of life (QOL) of people living with HIV/AIDS". Semi-structured interviews with 14 people was carried out. The analysis resulted in four categories Quality of life; subjectivity and multidimensionality; Living with HIV/AIDS and the quality of life as well as Care for the living quality and Health services as enhancers of QOL. There are several factors that interfere in the QOL of those people such as: physical aspects caused by the disease, the continuation of the treatment and the usage of antiretroviral medications. As social aspects, it was identified the presence of prejudice and discrimination. The acceptance of the treatment by antiretroviral medications considerably improves QOL. The identification of the factors that interfere in QOL will be able to contribute to more humanized service provided to people who live with HIV/Aids, leading to the care beyond the clinical aspects, instrumentalizing those people towards a better life quality.
