Data from Web of Science, SCOPUS, Pub Med, Medline, E-library, and other sources was used in writing this article. The main focus was directed towards literature written in English. The selection of literature was based on such concepts as: etiopathogenesis, historical principles of treatment, methods of surgical and non-surgical intervention. Data from metanalysis publications and randomized clinical trials pertaining to the treatment of the pilonidal sinus at various stages of its formation was used, as well.
Мета роботи: визначити можливі анатомічні чинники патогенезу пілонідальної хвороби в дітей. Матеріали і методи. Проведено аналіз результатів клініко-морфологічних обстежень 37 дітей з діагнозом “пілонідальна хвороба”, пролікованих з 2010 по 2017 рік на базі Вінницької обласної дитячої клінічної лікарні. Середній вік пацієнтів складав (16,4± 0,4) року; хлопчиків було 26 та 11 дівчаток. Для порівняння морфологічної будови пілонідальної кісти із дорослими пацієнтами використовували 17 зразків видалених раніше тканин, які були на обліку в архіві обласного патанатомічного бюро. Результати досліджень та їх обговорення. У 75 % препаратів у дорослих спостерігали відсутність епітеліальної вистилки, що є результатом тривалого рецидивного перебігу пілонідальної хвороби. Останнє сприяє швидкій хронізації запального процесу та виникненню гіалінозу судинних стінок у стінці кісти. Також майже в 94 % випадків у дорослих стінки пілонідальної кісти складались із грубоволокнистої сполучної тканини з вираженою лімфогістіоцитарною інфільтрацією, а у просвіті їх визначались деривати шкіри. Схожі морфологічні ознаки були виявлені лише у 30 % дітей, а у порожнині кісти були виявлені волосяні стрижні з ознаками часткового лізису, які були пов’язані із неатрофованими волосяними цибулинами, що чітко не свідчило на користь їх екзогенного трансдермального проникнення. У 41,66 % дітей при гістологічному обстеженні навколо куприкового ходу визначалась незріла мезенхімоподібна тканина. За рахунок щільної фіксації шкіри міжсідничної складки із оточуючими тканинами при рухах виникає її зміщення й розтягнення пор, в яких накопичуються секрети шкіри та її деривати, які ще більше її розтягують й при виникненні запалення сприяють її самовільному розкриттю. Тому пілонідальна хвороба – це поліетіологічне захворювання, однією із причин розвитку якої є підвищена локальна пітливість тіла, що зумовлює ряд додаткових провокаційних факторів утворення гострого абсцесу міжсідничної ділянки, підтримання хронічного запалення та рецидивного перебігу захворювання.
РЕЗЮМЕ. Пілонідальна хвороба – поширена патологія, історія вивчення якої є досить довгою. Проте досі немає одностайного рішення щодо показань до лікування, підходів та методів хірургічної корекції, плану подальшої реабілітації при цьому захворюванні. Мета – розробка методики післяопераційного ведення дітей, яким виконане радикальне оперативне втручання з приводу пілонідальної хвороби. Матеріал і методи. У нашій роботі наведено варіант ведення та реабілітації пацієнта, якому виконана операція, запропонована G. E. Karydakis у 1992 році (методика Bascom II). Її вважають оптимальною через ряд переваг перед іншими методами хірургічного лікування. Результати. Вважаємо, що ведення ділянки операційної рани доцільно поділити на декілька періодів, кожен з яких має свої особливості та правила, яких варто дотримуватись: передопераційний період, ранній післяопераційний період, віддалений післяопераційний період, пізній післяопераційний період. Висновки. 1. Раціональний підхід до організації періопераційного та післяопераційного періодів у дітей із пілонідальною хворобою є важливим елементом реабілітації, що може значно зменшити частоту післяопераційних ускладнень та ризик рецидивів захворювання, а також зменшити дискомфорт пацієнта. 2. Необхідним є чітке дотримання алгоритму реабілітації дітей, оперованих з приводу пілонідальної хвороби, для стандартизації надання допомоги.
результати морфологічних досліджень зразків інтраопераційно видалених тканин 36 дітей та 16 дорослих із пілонідальною хворобою. У 21 (58,33%) підлітків, на відміну від дорослих, в гістологічних зразках було виявлено мезенхімоподібну тканину, яка належить до тканин лише ембріонального періоду розвитку людини. В 12 (75,0%) досліджених препаратах епітеліальних куприкових кіст у дорослих на тлі ознак неспецифічного запалення спостерігалась відсутність епітеліальної вистилки. Проведений аналіз показав, що наявність певних морфологічних чинників пілонідальної хвороби у дітей, таких як наявність мезенхімальної тканини, відмінності будови стінки куприкової кісти від дорослих та відсутність чіткого підтвердження трансдермального проникнення волосяних стрижнів у стінку та порожнину епітеліальної куприкової кісти свідчать на користь вродженої етіології захворювання. Пирогова АНАТОМІЧНЕ ОБҐРУНТУВАННЯ ХІРУРГІЧНОГО ДОСТУПУ ПРИ ПІЛОНІДАЛЬНІЙ ХВОРОБІ У ДІТЕЙ Резюме. Мета: визначити основні анатомічні орієнтири при виконанні доступу в ході радикального оперативного втручання з приводу пілонідальної хвороби. Матеріали і методи: у дослідженні виконано обстеження 10 трупів дітей, які прижиттєво не мали патології перианальної ділянки та малого тазу. Вік дітей-від 12 до 17 років, серед яких 5 дівчаток та 5 хлопчиків. Тканини, що досліджувались, фіксовані в 10 % розчині нейтрального формаліну протягом 24-48 годин, зневоднені. Гістологічні поперечні зрізи забарвлені гематоксиліном та еозином та за ван Гізоном. Результати дослідження: при обстеженні гістологічної структури та морфометрії зовнішнього сфінктеру заднього проходу встановлено, що площа поперечного перетину пучків м'язових волокон у зовнішньому сфінктері може значно варіювати залежно від кількості м'язових волокон, що його складають, від статури дитини. Також визначені анатомічні співвідношення розмірів зовнішнього сфінктеру заднього проходу. Відповідно до морфометрії, зовнішній сфінктер заднього проходу має еліпсоподібну форму, витягнуту у передньо-задньому напрямку із переважанням каудального компонента.
Pilonidal disease is a common pathology; the history of its investigating is quite long that can explain a large number of existing theories regarding its genesis and methods of correction. However, one of the best methods of performing radical surgery for pilonidal disease was the surgical intervention suggested by G.E. Karydakis in 1992. However, the lack of a clear anatomical reasoning for performing this surgery can lead to damage to important anatomical structures that are within the area of surgery. Objective: to carry out detailed study at the microscopic level the structure of muscular tissues forming the internal and external sphincters of the rectum to provide anatomical grounds for selecting the techniques of radical surgery and to determine clear anatomical landmarks when performing surgical access. Materials and Methods: the study was performed on 10 corpses of children who had no any perianal and pelvic pathology in their lifetime. The age of children (5 girls and 5 boys) ranged from 12 to 17 years. Tissue samples were fixed in 10% neutral formalin solution for 24-48 hours and then dehydrated. Histological cross sections were stained with hematoxylin and eosin and by van Gieson technique. Results and discussion. The histological examination of the removed blocks of tissues located at 3`, 6`, 9` and 12` in the dorsal position revealed the main morphometric characteristics of the soft tissues of the perianal area with a targeted examination of the external sphincter of the anus. It has been established that the cross-sectional area of the muscle bundles of the latter can vary significantly depending on the age and physique of the child. The spatial structure of the external sphincter of the anus has been described as well. According to morphometry, the latter is ellipse-shaped, elongated along the anterior-posterior direction with the centre displaced in the caudal direction relative to the anus. Conclusion. 1. The anatomical structure of the perianal area contains fibers of the external anal sphincter in the thickness of the subcutaneous fat, which can be damaged when performing radical surgery for pilonidal disease in children. 2. A detailed study of the anatomy of the perianal area enables to clearly identify the anatomical landmarks of the access edges in radical surgery for pilonidal disease in children. 3. The implementation of radical surgery for pilonidal disease in children requires more detailed study, since the empirical recommendations for their performing pose some risks of disruption of the normal body functioning due to damage to important anatomical structures of the perianal area. 4. The distal incision boundaries when removing the pilonidal cyst should be replaced not by 2 cm from the edge of the anal ring, but by 3 cm, as there is a risk of damage to part of the sphincter apparatus of the outlet.
Objective: to determine the topical localization of the structural components of the anal sphincter and to formulate the basic postulates of the formation of safe anatomical access in pilonidal disease surgery in children. Materials and methods: the study was conducted on the corpses of 10 children who had no lifelong pathology of the sacrococcygeal region and pelvis aged 12 to 17 years, including 5 girls and 5 boys. Soft tissue columns 1 cm wide and up to 5 cm long were prepared at a distance of 1 cm from the anus by 12 h, 3 h, 6 h and 9 h according to the dial in the back position. After preparation and fixation of the drugs, their staining was performed and cross-sections of anal sphincters 5–7 μm thick were made. The analysis of the received morphometric data is carried out. The results of the study: it was found that the cross-sectional area of the bundle of muscle fibers of the external sphincter of the anus on average in adolescents ranged from 448±32 μm2 to 412±24 μm2. The diameter of its muscle fibers was 13.02±1.56 μm, and the bulk density of muscle fibers is 96.12±1.34%. Regarding the length of the internal anal sphincter, it was found that it is almost the same in different areas and is 1.3±0.03 at the level of 3 and 12 hours, 1.3±0.07 at the level of 6 hours and 1.2±0.03 at the level of 9 hours. In the study of the linear dimensions of the length of different portions of external anal sphincter in certain places of the biopsy revealed a predominance of parameters that were determined at 6 hours, respectively, 5.7±0.06 cm against 4.3±0.04 cm at 3 hours, and 12 hours, respectively 5.1±0.06 cm against 4.3±0.03 cm at 9 years. The thickness of the external sphincter of the anus at 6 hours, respectively 26.7±0.61 mm against 18.5±0.19 mm at 3 hours, (<0.01) and 12 hours, respectively 23.9±0.33 mm against 18.4±0.19 mm at 9 hours. Diameters of separate muscular fibers and bundles were explored. It is established that the average diameter of a muscle fiber makes 13.7±0.18 microns, and the average diameter of a muscular bundle is equal to 435.9±5.15 microns. Conclusions. 1. Existing anatomical descriptions of anal sphincters need in the modern world more thorough research to prevent their injury during surgery. 2. The external anal sphincter has the spatial form of the three-storeyed oval structure extended in the front-back direction with dominance of the caudal muscular portion. 3. When performing radical surgical interventions for pilonidal disease in children by cleft-lift method, it is necessary to complete the edge of surgical access at a distance of not less than 3 cm to the edge of the anal sphincter. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interest was declared by the authors. Pilonidal disease is a pathology that was first described more than 100 years ago. However, it still remains the subject of discussion, because there is still no clear unambiguous definition of this pathology, nor its well-known etiology. Despite the large number of developed and substantiated theories, there are still two opposing views on the cause of pilonidal disease. Some scientists are supporters of the “congenital” theory, others – the theory of acquired origin. However, this does not clarify the ultimate goal of these studies: to develop optimal treatment tactics. The purpose of the work is to clarify the data on the etiology of pilonidal disease in children. The study is based on the results of treatment analysis of 37 children diagnosed with “pilonidal disease” who were hospitalized in the Department of Emergency Surgery of Vinnytsia Regional Children’s Clinical Hospital, of which boys – 26, girls – 11. The mean age of patients was 16.4±0.4 years. Histological sections of tissue samples were stained with hematoxylin and eosin. Microscopy and creation of a photo archive of histological specimens were performed using a light microscope OLIMPUS BX 41 at a magnification of 100 and 200, in the software environment “Quick PHOTO MICRO 2.3”. The study found that there are significant differences in the histological structure of pilonidal cysts in children and adults. In pediatric patients, mesenchymal tissue was found, which is not typical for this group of patients. It was also found that the cavity of the pilonidal cyst in children is covered with a multilayered squamous non-keratinizing epithelium, and there is almost no granulation tissue. In our opinion, pilonidal disease is a polyetiological disease, the main cause of which is the congenital features of the organism, which are realized due to socio-economic and environmental factors.
