Пирогова АНАТОМІЧНЕ ОБҐРУНТУВАННЯ ХІРУРГІЧНОГО ДОСТУПУ ПРИ ПІЛОНІДАЛЬНІЙ ХВОРОБІ У ДІТЕЙ Резюме. Мета: визначити основні анатомічні орієнтири при виконанні доступу в ході радикального оперативного втручання з приводу пілонідальної хвороби. Матеріали і методи: у дослідженні виконано обстеження 10 трупів дітей, які прижиттєво не мали патології перианальної ділянки та малого тазу. Вік дітей-від 12 до 17 років, серед яких 5 дівчаток та 5 хлопчиків. Тканини, що досліджувались, фіксовані в 10 % розчині нейтрального формаліну протягом 24-48 годин, зневоднені. Гістологічні поперечні зрізи забарвлені гематоксиліном та еозином та за ван Гізоном. Результати дослідження: при обстеженні гістологічної структури та морфометрії зовнішнього сфінктеру заднього проходу встановлено, що площа поперечного перетину пучків м'язових волокон у зовнішньому сфінктері може значно варіювати залежно від кількості м'язових волокон, що його складають, від статури дитини. Також визначені анатомічні співвідношення розмірів зовнішнього сфінктеру заднього проходу. Відповідно до морфометрії, зовнішній сфінктер заднього проходу має еліпсоподібну форму, витягнуту у передньо-задньому напрямку із переважанням каудального компонента.

На основі проведених анатомо-просторових досліджень крижово-куприкової ділянки у 50 хворих на ПХ визначені прогностичні чинники її перебігу у дітей.Результати. Були визначені основні концептуальні припущення та рекомендації при видаленні патологічного вогнища. В усіх випадках розроблена модель розрахунку параметрів та локалізації оперативного доступу дозволила дотримуватись мініінвазивного напрямку хірургічного лікування. Залежно від локалізації, розмірів та варіантів розташування норицевих ходів пілонідальної кісти отримана на основі співвідношення еліпсоподібних площин просторова модель дозволяє визначити необхідні параметри операційної рани, що дозволяє максимально латералізувати операційну рану при мінімальній площі видалених м'яких тканин.Висновки. Формування контурів, локалізації та параметрів інтраопераційного доступу при ПХ у дітей, згідно з розробленою моделлю просторового обґрунтування форми та параметрів оперативного доступу, свідчить про доцільність її еліпсоподібної форми, параметри якої визначаються розташуванням зовнішніх отворів норицевих ходів по відношенню до міжсідничної складки. Контур операційної рани у формі еліпса з чітко визначеними геометричними параметрами розрахованого об'єкту дозволяє виконувати мініінвазивні пластичні оперативні втручання при видаленні пілонідальної кісти без додаткового формування періанального шкірно-підшкірно-жирового клаптя, що дозволяє знизити кількість післяопераційних ускладнень і рецидивів та покращити якість і наслідки лікування.Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом всіх зазначених у роботі установ. На проведення досліджень було отримано інформовану згоду батьків, дітей.Автори заявляють про відсутність конфлікту інтересів.
