The course of the epidermal regeneration after stripping of the corneal layer was comparatively investigated by means of the distribution and activity of various enzymes in clinically normal psoriatic skin and normal skin. The following results were obtained: (1) At first clinically healthy psoriatic skin and normal skin behave principally uniform. (2)The observed augmentation of the activity of various enzymes in psoriatic skin free from specific lesions is so much increased that it essentially corresponds to the condition of the psoriatic lesion although to a different intensity. (3) Differences in the reproduction of the corneal layer were observed. (4) The possible consequences of these findings for the course of the psoriatic reaction are discussed.
Most of the patients (86.6 %) suffered from chronic plaque pso− riasis with a mean age of 48.5 years. Psorcutan Beta ointment showed a very fast onset of action and a convincing global effica− cy on every parameter observed in this study. As soon as one week after the initiation of the study 44.1 % of the patients al− ready observed improvement of skin lesions, another 35 % at the time point of 14 days. Complete remission or at least advance im− provements were observed in 95 % after the 4 weeks of the initia− tion period. Tolerability was excellent for all formulations of Psorcutan and Psorcutan Beta. Only one single patient suffered from an adverse event (transient exacerbation of exanthematic psoriasis). The rapid onset of action of Psorcutan Beta, the short treatment duration, the excellent therapeutic effects and the tolerability of both Psorcutan and Psorcutan Beta in the long observation pe− riod, confirmed the best suitibility of these medications in the treatment of psoriasis vulgaris. In this post marketing surveillance study it was proven that the maximum efficacy was gained by Psorcutan Beta with a marked improvement of skin lesions. Stabilization of the skin state or further amelioration was obtained by a consequent daily appli− cation of Psorcutan. ZusammenfassungIn der Therapie der leichten bis mittelschweren Acne vulgaris ist der Einsatz topischer Aknetherapeutika Mittel der ersten Wahl. Bevorzugt werden solche Substanzen, die mehr als einen Faktor in der komplexen Pathogenese der Akne beeinflussen. In einer Anwendungsbeobachtung wurden Wirksamkeit und Verträg− lichkeit von Azelainsäure in 15 %iger, nicht−alkoholischer Hydro− gelformulierung erfasst. Das Medikament wurde zweimal täg− lich über 9 ± 210 (Mittel: 70,8) Tage aufgetragen. 1243 Patienten aus 296 Zentren mit Acne vulgaris wurden dokumentiert, wobei 1239 Patienten aus 295 Zentren in die Auswertung eingingen. Mehr als die Hälfte der Patienten hatte eine Acne comedonica oder Acne papulopustulosa. Die Therapie wurde in 64,9 % der Fäl− le als Azelainsäure−Monotherapie und in 35,1 % als Kombina− tionstherapie mit anderen topischen oder systemischen Akne− therapeutika durchgeführt. In der Gesamtbeurteilung der Wirk− samkeit besserten sich nach 8 ± 12 Wochen bei der überwiegen− den Mehrzahl der Patienten die Symptome moderat (43,0 %) bis sehr gut (36,1 %). Der therapeutische Durchbruch setzte in 53,6 % innerhalb der ersten 5 ± 10 Wochen nach Behandlungsbeginn ein. Zusammenfassung: Obwohl sich durch die topischen Kortikoide der 4. Generation das Risiko von Nebenwirkungen bei sehr guter Wirksamkeit minimiert hat, herrscht bei der Anwendung im Gesicht immer noch Verunsicherung und Ablehnung. In einer Anwendungsbeobachtung (AWB) bei 575 Patienten mit ekzematösen Dermatosen der Gesichtshaut (vor allem Neurodermitis oder Kontaktekzem) konnte – bei exzellenten Therapieerfolgen – gezeigt werden, dass keine Nebenwirkungen auftreten. Beim bestimmungsgemäßen Gebrauch (keine Anwendung über die – durchschnittlich nach 14 Tagen erreichte – klinische Abheilung hinaus) ist nicht mit dem Auftreten einer perioralen Dermatitis zu rechnen.
Distribution and activity of alkaline phosphatase, glucose-6-phosphatase and 5-nucleotidase have been investigated in clinically unaffected psoriatic skin, gained in the immediate neighborhood to foci. According to previous results an increased enzyme activity could be shown histochemically in histologically already alterated capillaries. Quantitative determination of enzymes however proved no difference to normal skin. These statements aid the supposition, the primary event («Primärereignis») of psoriatic reaction is not in cutaneous-vascular alterations.
