Candy cane syndrome is a rare complication reported in bariatric patients following Roux-en-Y gastric bypass. It occurs when there is an excessive length of roux limb proximal to gastrojejunostomy, creating the possibility for food particles to lodge and remain in the blind redundant limb. Patients present with non-specific symptoms such as abdominal pain associated with nausea and vomiting. Most remain undiagnosed as the disease process is poorly described. We report three cases of candy cane syndrome treated successfully at our institution.
The incidence of prostate cancer in the Autonomous Community of Madrid is lower than that in the US but higher than in most countries or regions in the EU. The different way of using PSA testing in the Health Areas of the Autonomous Community may explain the differences seen in terms of incidence by Area.
Most patients came to the hospital because of symptoms not related to PC. Transrectal ultrasound-guided biopsy is confirmed as the choice technique for PC diagnosis. When a comparison is made to historical series in our Autonomous Community a pattern of earlier diagnosis can be seen.
Acute appendicitis is the most common surgical emergency in the United States. However, unusual causes of appendicitis need to be considered in patients with significant co-morbidities like our patient with history of prostate cancer. Cases of acute appendicitis due to metastatic prostate cancer are rare, with only a few cases reported in the literature. We report a case of a 72 year old male with a history of metastatic prostate cancer that caused acute appendicitis prompting surgical intervention.
Conocer el tratamiento aplicado en los pacientes con cáncer de próstata diagnosticados en la Comunidad de Madrid en el año 2000.MATERIAL Y MÉTODO: El estudio se realizó sobre los 1.745 pacientes remitidos por los Hospitales participantes en el estudio. En 1.104 (63%) de los casos existían datos sobre el tratamiento aplicado. Se comparó la modalidad de tratamiento con el estadio clínico y la edad de los pacientes.RESULTADOS: El tratamiento más utilizado fue la hormonoterapia (35%) seguido de la prostatectomía radical (34%) y la radioterapia (25%). En los pacientes con enfermedad localizada la prostatectomía fue el tratamiento más utilizado (42,3%) y la hormonoterapia cuando la enfermedad estaba localmente avanzada (45,6%) o diseminada (94%). Existen diferencias importantes en la indicación terapéutica entre las distintas Áreas Sanitarias participantes. La mediana de edad de los pacientes con enfermedad localizada y localmente avanzada fue más baja en los tratados con prostatectomía (65 y 64 años respectivamente) que los tratados mediante radioterapia (70 y 69 años respectivamente).CONCLUSIONES: El tratamiento indicado por los Urólogos de la Comunidad de Madrid en los pacientes con cáncer de próstata cumple, en líneas generales, las recomendaciones de la literatura.
A left orbital arteriovenous fistula (AVF) is diagnosed in a patient presenting with proptosis. Intra-orbital AVFs are rare according to the literature search, and therefore, the best treatment modality remains controversial. We present a case of a patient who presented with non-specific symptoms. He was diagnosed with intra-orbital AVF and underwent a trans-arterial embolization. The procedure was complicated by the central retinal artery occlusion, which is one of the most feared complications associated with this procedure. We discuss the modalities in the diagnosis of intra-orbital AVFs as well as stress the importance of an interdisciplinary approach for its timely and efficient management.
