Aim. To present the first Russian experience in the management of portal hypertension of various etiology by creating a selective portosystemic shunt between left gastric vein and inferior vena cava (leftgastric vein caval bypass).Material and methods. “Left gastric vein to inferior vena cava” bypass was performed in 6 patients with portal hypertension: 4 men and 2 women (mean age 40.4 years). The follow-up period ranged from 10 to 36 months.Results. “Left gastric vein to inferior vena cava” bypass was effective in the management of portal hypertension and its complications in 5 out of 6 patients. Complete eradication of esophagogastric varices and no variceal bleeding were observed in long-term period. These patients demonstrated no clinical or laboratory signs of hepatic encephalopathy and/or hepatic failure within the follow-up. One patient with thrombophilia and extrahepatic portal vein obstruction (factor V Leiden mutation) had thrombosis of the shunt and recurrent variceal bleeding in 6 months after surgery because of unauthorized abandonment of the anticoagulation.Conclusion. “Left gastric vein to inferior vena cava” bypass is an effective procedure for prevention of variceal bleeding (or recurrence) in patients with portal hypertension. The limitation of this technique is insufficient diameter of left gastric vein in many patients. This procedure has certain pathophysiological advantages over other types of portosystemic anastomoses due to highly selective nature of the shunt. Thus, this approach should be introduced into surgical treatment of these patients.
This study demonstrates a successful clinical outcome in a female patient with severe refractory ascites associated with a complex combined form of extrahepatic portal hypertension. The study emphasizes the need and particular importance of comprehensive diagnostics and assessment of the angioarchitectonics in the portal vein system, thorough study of the porto–hepatic circulation and central hemodynamics, as well as of other major homeokinesis disorders. Understanding the pathogenesis of the disease allows for the development of a therapeutic plan with a strictly personalized approach toward the selection and staging of various surgical interventions.
Aim. To investigate the major risk factors and influence of surgical aggression on development of postoperative complications in surgical treatment of patients with liver cirrhosis and portal hypertension. Material and Methods. Features of early postoperative period in 300 patients with liver cirrhosis and portal hypertension were analyzed. The groups of patients were defined where the influence of various technical aspects of operations on surgical results was studied. Also impact of changes of the main parameters of nutritionally-metabolic status, central and porto-hepatic hemodynamics, tissues' "oxygen regime" on the development of postoperative complications was assessed. Цель. Изучить основные факторы риска и влияние хирургической агрессии на развитие осложнений в послеоперационном периоде при хирургическом лечении больных циррозом печени и портальной гипертензией. Материал и методы. Анализировали особенности раннего послеоперационного периода у 300 пациентов с циррозом печени и портальной гипертензией. Выделены группы больных, в которых изучали влияние различных технических аспектов операций на результаты вмешательств. Также исследовали влияние изменений основных параметров нутритивно-метаболического статуса, центральной и портопеченочной гемодинамики, а также кислородного режима тканей на развитие послеоперационных осложнений. Результаты. Важную роль в развитии послеоперационных осложнений следует отвести технике оперирования, выбору оптимальной конструкции сосудистого анастомоза и композиции операций на желудке. Переносимость хирургического вмешательства обусловлена исходным уровнем компенсации заболевания. Хирургическая агрессия способствует уменьшению дефицита резервного потенциала и емкости компенсации в среднем на 10-15%. Под влиянием хирургической агрессии в зависимости от вида хирургического вмешательства и исходной тяжести хронической печеночной недостаточности могут развиваться нарушения нутритивно-метаболического статуса, центральной гемодинамики, портопеченочного кровообращения и некоторых параметров кислородного режима тканей. Выявлено сопряжение и взаимное влияние выявленных нарушений основных направлений трофического гомеокинеза, несущего большую ответственность в развитии послеоперационных осложнений. Дезорганизация трофического гомеокинеза-важнейший инструмент, реализующий патологическое течение послеоперационного периода. Представлена гипотеза и универсальная схема общих путей патогенеза осложнений в раннем послеоперационном периоде. Заключение. Помимо техники операций послеоперационные осложнения у больных циррозом печени и портальной гипертензией являются результатом несоответствия, с одной стороны, между исходной тяжестью течения заболевания, степенью компенсации печеночной недостаточности и уровнем резервного потенциала организма, а с другой-требованиями, предъявляемыми организму хирургическим вмешательством.
