Methicillin-resistant Staphylococcus aureus (MRSA) is a global health concern. One of the factors causing hospital infection is related to the ability of MRSA bacteria to form biofilms. Polysaccharide intercellular adhesin (PIA), encoded by ica gene, have an important role in S. aureus intracellular accumulation and aggregation. The aims of this study was to analyze the relationship between icaA, icaD genes and biofilm production in MRSA carrier and clinical isolate in Dr. Soetomo Hospital Surabaya. This study was an observational study using cross sectional approach. The sample was 47 MRSA isolates is as follow 28 isolates from carrier and 19 were clinical isolates. All of MRSA isolates carried mecA gene. PCR was performed to detect icaA and icaD genes. Biofilm formation was detected using microtiter plate assay (MTP). icaA gene was detected in all isolates whereas icaD gene in 96,4% carrier isolates and all (100%) of clinical isolates. Positive MTP results showed in all (100%) of carrier isolates and 57,9% of clinical isolates. Statistic result was significantly different in biofilm formation between carrier and clinical MRSA isolates. The proportion of positive biofilm formation in isolate with positive icaA/D genes was 82.6%. There was not any association between icaA and icaD gene with biofilm production.
Travel insurance is important to protect travelers to various travel risk and health problem. But because of financial condition and lack of knowledge to protect their safety while traveling, the use of travel insurance is very low. This research aim to assess distribution of travel insurance user among travelers especially who visiting Bali as their destination trip. This is cross-sectional study using medical record of foreign travelers visiting international hospital. Result show most travelers have no travel insurance (70%) compared with people take travel insurance for their safety vacation. Based on age, there are no significance difference between age group 0-18 years, 19-64 years dan more that 65 years, approximately the youngest people the lack of awareness to buy travel insurance. Based on health problem the distribution of travelers with no travel insurance tend to be same on average 60-80%. As conclusion the use of travel insurance remains low, furthermore all travelers must be encouraged to use travel insurance for their safety.
In this study we reported cultivation of bacteria associated with mangrove soil from the Ngurah Rai Mangrove Forest, Bali. Mangrove soil samples were serially diluted using sterile artificial seawater, spread onto Starch Casein M agar and incubated at 28oC for 28 days. Cultivation of mangrove soil samples yielded 165 bacterial colonies with 68 isolates were selected and purified based on different morphology. Of these 68 isolates, 22 isolates displayed antibacterial activities ranging from weak to strong inhibition against at least one of four bacterial indicators namely Staphyloccocus aureus, Streptococus mutans, Escherichia coli and Klebsiella pneumoniae using perpendicular streak method. Overall, 19 out of 22 bacteria isolates displayed weak antibacterial potential and two isolates exhibited moderate antibacterial activity. The isolate SA4 was the only bacterium with strong antibacterial potential with measured clear distance ≥ 10 mm against the four bacterial isolates. Sequence analysis based on 16S rRNA gene fragment assigned the isolate SA4 as Bacillus subtilis strain BIL/BS-168. Overall, this study confirmed the untapped potential of antibacterial activities from bacteria associated with mangrove soil.
Bali is one of the most popular tourist destinations in Indonesia. As one of the islands in a tropical country, Bali has its own risk of disease transmission. The increasing number and pattern of international travelers can affect morbidity and mortality. Research on disease problems among international travelers in travel destination countries is rarely reported. This study examines health problems concerning foreign tourists visiting hospitals in Bali. This cross-sectional study used total sampling on all medical records of international travelers visiting an international hospital in Bali. A total of 999 medical records were analyzed in this study. The mean age of the patients was 35.55 years. Most patients were female (53.9%) and used travel insurance (55.3%). Most health problems are complaints in the gastrointestinal system (22.3%). The primary health problem is the gastrointestinal system. Most patients use insurance to finance their health.
