RESUMO: O objetivo foi identificar as consequências de sofrimento moral em enfermeiros. Trata-se de uma revisão integrativa, com coleta dos dados realizada de maio a setembro de 2013 e atualizada em julho de 2015 na Biblioteca Virtual em Saúde, com os termos: Moral Distress, Moral Suffering, Nurse e Nursing. Oitenta e cinco estudos compuseram a amostra final. Os resultados são apresentados em quatro categorias. A primeira: Sentimentos composta pela presença de sentimentos como insatisfação, frustração, raiva; a segunda: Adoecimento emergiu da vivência de adoecimento físico e emocional; a terceira Qualidade do cuidado pela presença de ações que levavam ao cuidado ineficaz e negação da advocacia do paciente; e a quarta: Estratégias de enfrentamento, demonstrou as formas de enfrentamento utilizadas, como: abandono da profissão e a desnaturalização das práticas. Evidencia-se que as consequências estão presentes no cotidiano dos profissionais, e causam danos muitas vezes irreversíveis, inclinando para o abandono da profissão. Objective: To evaluate the frequency and intensity of moral distress in Brazilian nurses. Method: Cross-sectional study performed with nurses from 27 Brazilian states through application of the Brazilian Moral Distress Scale in Nurses (Portuguese acronym: EDME-Br) and descriptive statistical analysis. Results: Participation of 1,226 Brazilian nurses in the study. The intensity and frequency of overall moral distress were rated as moderate level, with averages of 3.08 (± 1.45) and 2.94 (± 1.37), respectively. Specifically, the highest intensity and frequency was related to the factors Acknowledgement, power and professional identity and Work teams, while the lowest was related to the factor Defense of values and rights. Conclusion: Moral distress occurs in precarious work environments, with little expressiveness of the nurses’ role. One highlights the importance of the problem in terms of its amplitude and multicausality, reaching professionals acting in different work contexts.
Objective:to validate an instrument to identify situations that trigger moral distress in relation to intensity and frequency in primary health care nurses. Method:this is a methodological study carried out with 391 nurses of primary health care, applied to the Brazilian Scale of Moral Distress in Nurses with 57 questions. Validation for primary health care was performed through expert committee evaluation, pre-test, factorial analysis, and Cronbach’s alpha. Results:there were 46 questions validated divided into six constructs: Health Policies, Working Conditions, Nurse Autonomy, Professional ethics, Disrespect to patient autonomy and Work Overload. The instrument had satisfactory internal consistency, with Cronbach’s alpha 0.98 for the instrument, and between 0.96 and 0.88 for the constructs. Conclusion:the instrument is valid and reliable to be used in the identification of the factors that trigger moral distress in primary care nurses, providing subsidies for new research in this field of professional practice.
Objective: To analyze the situations generating Moral Distress in Primary Care nurses from different regions of Brazil. Method: Qualitative research, with 13 nurses of the Brazilian Primary Health Care, through a semi-structured interview and submitted to the Discursive Textual Analysis, with the resources of the software ATLAS.ti 7.0. Results: They are presented in four categories, relating the Moral Distress to the professional; health services; users and conflicts in professional practice. Final Considerations: By assuming ethical and political values as guides to their action, nurses give greater visibility to the moral content of many problems and weaknesses that could be seen only by the bias of working conditions.
Resumo Objetivo Analisar associações entre distresse moral e características sociodemográficas e laborais de enfermeiros brasileiros. Métodos Estudo transversal realizado em todo território brasileiro, no período de novembro de 2015 a maio de 2016, com uma amostra de 1227 enfermeiros, a partir da aplicação da Escala Brasileira de Distresse Moral em Enfermeiros (EDME-Br), por meio de questionário eletrônico. A análise empregou estatística descritiva e analítica. Resultados Associações estatisticamente significativas foram estabelecidas em todos os seis fatores, são eles: 1. Reconhecimento, poder e identidade pessoal; 2. Cuidado seguro e qualificado; 3. Defesa de valores e de direitos; 4. Condições de trabalho; 5. Infrações éticas; 6. Equipes de trabalho. As variáveis que apresentaram maior número de associações foram: carga horária; pós-graduação, natureza da atenção/complexidade do serviço, natureza, tipo e tempo do vínculo. Destacou-se o papel da experiência e da formação ampliada na maior percepção e/ou vivência do distresse moral. Conclusão A amplitude do estudo e a diversidade do contexto revelam especificidades quanto à ocorrência do distresse moral no cenário brasileiro, reforçando a necessidade de conhecer elementos que participam da experiência moral dos enfermeiros.

O estudo é quantitativo, transversal, no qual o questionário Protective Nursing Advocacy Scale foi submetido à análise fatorial exploratória. Foi realizado com 451 enfermeiros que atuam em Unidades de Terapia Intensiva das regiões Sul e Sudeste do Brasil. Foram identificados dois fatores sobre o exercício dos enfermeiros na advocacia do paciente de unidades de terapia intensiva: “Antecedentes, barreiras e implicações negativas do exercício da advocacia do paciente pelo enfermeiro de Unidade de Terapia Intensiva”; e “Ações dos enfermeiros e benefícios da advocacia do paciente na Unidade de Terapia Intensiva”. Foi identificado que enfermeiros de unidades de terapia intensiva conhecem as necessidades do paciente, logo agem em seu nome quando escutam seus desejos e avaliam as melhores decisões para seu cuidado, e tratamento, juntamente com o paciente e família. Patient advocacy by Brazilian nurses in the context of intensive care Abstract: The aim of this study is to identify factors affecting the performance of nurses, from intensive care units in the South and Southeast of Brazil, in patient advocacy through an exploratory analysis of the Protective Nursing Advocacy Scale scale. The study is quantitative, transversal, in which the Protective Nursing Advocacy Scale questionnaire was submitted to exploratory factorial analysis. It was carried out with 451 nurses who work in Intensive Care Units of the South and Southeast regions of Brazil. Two factors were identified on the practice of nurses in intensive care unit advocacy: "Background, barriers and negative implications of the practice of patient advocacy by the Intensive Care Unit nurse"; and "Nurses' actions and benefits of patient advocacy in the Intensive Care Unit". It was identified that intensive care unit nurses know the patient's needs, then act on their behalf when they listen to their wishes and evaluate the best decisions for their care and treatment, along with the patient and the family.
