Paracetamol and NSAIDs, in particular acetylsalicylic acid (aspirin) and ibuprofen, are among the most used and environmentally released pharmaceutical drugs. The differences in international trends in the sale and consumption of mild analgesics reflect differences in marketing, governmental policies, habits, accessibility, disease patterns and the age distribution of each population. Biomonitoring indicates ubiquitous and high human exposure to paracetamol and to salicylic acid, which is the main metabolite of acetylsalicylic acid. Furthermore, evidence suggests that analgesics can have endocrine disruptive properties capable of altering animal and human reproductive function from fetal life to adulthood in both sexes. Medical and public awareness about these health concerns should be increased, particularly among pregnant women.
Background:Numerous chemicals are capable of disrupting androgen production, but the possibility that they might act together to produce effects greater than those of the most effective component in the mixture has not been studied directly in human tissues. Suppression of androgen synthesis in fetal life has been associated with testis maldescent, malformations of the genitalia at birth, and poor semen quality later in life.Objectives:Our aim was to investigate whether chemicals can act together to disrupt androgen production in human fetal testis explants and to evaluate the importance of mixture effects when characterizing the hazard of individual chemicals.Methods:We used an organotypic culture system of human fetal testes explants called FEtal Gonad Assay (FEGA) with tissue obtained at 10 and 12 gestational wk (GW 10–12), to screen 27 chemicals individually for their possible anti-androgenic effect. Based on the results of the screen, we selected 11 compounds and tested them as mixtures.Results:We evaluated mixtures composed of four and eight antiandrogens that contained the pharmaceuticals ketoconazole and theophylline and several previously untested chemicals, such as the pesticides imazalil and propiconazole. Mixtures of antiandrogens can suppress testosterone synthesis in human fetal testicular explants to an extent greater than that seen with individual chemicals. This revealed itself as a shift towards lower doses in the dose–response curves of individual antiandrogens that became more pronounced as the number of components increased from four to eight.Conclusions:Our results with the FEGA provide the foundations of a predictive human mixture risk assessment approach for anti-androgenic exposures in fetal life. https://doi.org/10.1289/EHP1014
WHAT IS KNOWN ALREADY:The few epidemiologic studies performed suggest that bisphenols have potential endocrine disruptive properties, but they did not identify clear and direct patterns of endocrine disruption. STUDY DESIGN, SIZE, DURATION:Adult human testis explants in culture were exposed to BPA and the analogs bisphenol F (BPF), bisphenol S (BPS), bisphenol E (BPE), bisphenol B (BPB), and bisphenol A diglycidyl ether (BADGE) at 10 -9 to 10 -5 M for 24 h or 48h.PARTICIPANTS/MATERIALS, SETTING, METHODS: Human adult testes were obtained from prostate cancer patients who had no hormone therapy, or from multiorgan donors. After ex vivo exposure to the investigated bisphenols, the measured outcomes were related to histopathology (gross morphology and germ cell viability determined by anti-caspase 3 immunohistochemistry), and the levels of testosterone, insulin-like fator 3 and inhibin B were measured using immunoassays. The levels of mRNA encoding key enzymes of bisphenol biotransformation were investigated by quantitative PCR: UGT2B15 UDP (glucuronosyltransferase two family, polypeptide B15), GUSB (glucuronidase beta), SULT1A1 and 3 (sulfotransferase family 1 A member 1 and 3) and STS (steroid sulfatase). MAIN RESULTS AND THE ROLE OF CHANCE: A significant dose-dependent inhibition wasfound between testosterone levels measured in the culture medium and concentrations of BPA The active concentrations of BPA and BPA-A used in the study were higher than those found in human biological fluids. WIDER IMPLICATIONS OF THE FINDINGS:Under our experimental conditions, direct exposure to BPA or BPA-A can result in endocrine disturbance in the adult human testis.

Résumé Notre recherche vise à explorer l’évolution psychothérapique de patientes boulimiques dans une perspective psychanalytique au moyen du Rorschach pratiqué avant et après thérapie. L’étude a porté sur 26 cas de boulimie pris en charge sur un mode multifocal, comprenant notamment une thérapie d’orientation psychanalytique. Le retest a été effectué au moins un an après le début du traitement. Des troubles narcissiques et objectaux peuvent être repérés chez les boulimiques à partir d’indices du Rorschach inspirés de la littérature concernant leur évolution avec la thérapie. Les résultats montrent que ces indices progressent d’une façon variable. L’analyse des protocoles au cas par cas, selon le fonctionnement psychique de chaque sujet, nous est apparue plus féconde. Plusieurs types d’évolution psychothérapique ont été repérés : les changements vont dans le sens d’un accroissement de la subjectivation, dans le sens d’une stratégie plus adaptative ou d’une meilleure différenciation par rapport à l’objet.

L’intérêt pour le changement psychique et la particularité du travail en addictologie ont fait naître l’envie chez les auteurs de mener un travail de recherche. 13 patients souffrant d’un trouble de l’usage de l’alcool suivis en psychothérapie ont été examinés au début du traitement (t1) et à stade avancé (t2) un an plus tard. L’examen comprenait un entretien semi-directif et un Rorschach. 11 patients sur 13 ont progressé cliniquement. L’hypothèse était que l’amélioration clinique s’accompagnerait d’un remaniement psychique visible dans le Rorschach. Ce remaniement est étudié au niveau des indices d’angoisse et de représentation psychique. L’impact éventuel de la désirabilité sociale est également examiné. Une étude nomothétique des résultats montre surtout une diminution de l’angoisse. Mais dans une étude idiographique illustrée par trois cas, on observe une modification des représentations. Cette évolution se fait dans un sens plus expressif ou dans un sens plus adaptatif. Au-delà de cette étude, l’expérience de chercheur clinicien a été profitable à la pratique des auteurs et ils poursuivront leurs recherches sur l’exploration des représentations de la dépendance à l’alcool des patients suivis en CSAPA.
