Objective: analyze and assess the psychometric properties of the subscales in the Spanish version of the Exercise Benefits/Barriers Scale in an elderly population in the Northeast of Mexico. Method: methodological study. The sample consisted of 329 elderly associated with one of the five public centers for senior citizens in the metropolitan area of Northeast Mexico. The psychometric properties included the assessment of the Cronbach's alpha coefficient, the Kaiser Meyer Olkin coefficient, the inter-item correlation, exploratory and confirmatory factor analysis.Results: in the principal components analysis, two components were identified based on the 43 items in the scale. The item-total correlation coefficient of the exercise benefits subscale was good. Nevertheless, the coefficient for the exercise barriers subscale revealed inconsistencies.The reliability and validity were acceptable. The confirmatory factor analysis revealed that the elimination of items improved the goodness of fit of the baseline scale, without affecting its validity or reliability. Conclusion: the Exercise Benefits/Barriers subscale presented satisfactory psychometric properties for the Mexican context. A 15-item short version is presented with factorial structure, validity and reliability similar to the complete scale.
Objective: to know the relationship between the sensory function, gait ability, and cognitive function with dependency in older adults. Method: a descriptive cross-sectional design, 146 older adults took part. Measurements: Snellen chart, Audiometer, Stereognosia tests, Semmes-Weinstein monofilament, basic aromas and flavors, GAITRite system, Montreal Cognitive Assessment Test, the Barthel Index, and the Lawton and Brody Index. Results: sensory function, cognitive function and gait explain 25% dependence on basic activities of daily life and 21% dependence on instrumental activities of daily life. The variables that influence dependence on basic activities were taste (p=.029), gait speed (p=.009), cadence (p=.002) and step length (p=.001) and, in instrumental activities, gait speed (p=.049), cadence (p=.028) and step length (p=.010). Conclusion: gait speed, cadence and stride length are variables that influence both dependence on basic and instrumental activities of daily life.
El significado que los adultos mayores otorgan a su dependencia funcional puede impedir el autocuidado para su salud y conducirlo a una dependencia total, situación que es posible evitar a través del desarrollo de intervenciones de enfermería enfocadas a la rehabilitación, educación y reconstrucción de su significado. Con el objetivo de comprender el significado que los adultos mayores otorgan a su propia dependencia funcional y describir los factores que explican la construcción del significado, se realizó un estudio cualitativo con cinco adultos mayores mexicanos que presentaban dependencia funcional según el índice de Barthel y autopercepción de dependencia. Los datos cualitativos se obtuvieron mediante una entrevista a profundidad semiestructurada, el muestreo fue por criterio lógico y el método de análisis se basó en la teoría fundamentada. Se encontró que el significado de dependencia funcional parte de la necesidad de recibir ayuda, la familia es la principal fuente de cuidado. Ser dependiente significa una transformación que involucra dejar de ser o hacer; incluye sufrimiento propio y de la familia y temor a lo desconocido. Los factores involucrados en la construcción del significado son: fuentes de ayuda, sentimientos y pérdidas físicas, sociales y económicas.
Self-care in older adults with type 2 diabetes. Monterrey, México. SUMMARYObjectives: to determine self-care differences by age and gender in older adults with type 2 diabetes, attending in a family health clinic. Material and Methods: The sample was 109 older adults (63 women and 46 men). The self-care was evaluated with the Summary of Diabetes Self-Care Activities. The data analysis was used descriptive statistics (frequencies, measures of dispersion and 95%CI), Mann-Whitney and difference of proportions. Results: The mean of diagnosis time was 12.24 (± 9.9) years, 31.2% (34) said to have some complication by the pathology, the most common of this was retinopathy on 22.9% (25), 33.0% (36) of older did self-care practices, the care that were performed more frequentlywerecare of foot and drugs; The women had self-care practices, this data was reported slightly higher in men (28.6% vs 39.1%; p> 0.05). Adults of 60-69 years had higher self-care practices to those over 70 years (4.9 % vs 30.4 %; p> 0.05). Conclusions: Self-care practices in this population are poor despite they have a disease, according to activities with higher frequencies we concludes that older give most importance to the consumption of drugs, and foot care. Men and younger older adults are reporting higher performing self-care activities.
A sample of 76 older adults (27 with diabetes and 49 without diabetes) were selected to judge three information factors, metamemory related to-drug organization-stress and physical activity in relation to their perception of self-care. In order to do so, an Integration Information Theory was considered to determine systematic cognitive algebra rules underlying judgment about 24 different health self-care scenarios. Results indicate that older adults without diabetes use a summative cognitive rule to integrate relevant health information but older adults with diabetes do not. Both groups agreed that metamemory was the most relevant factor to their self-care followed by stress and physical activity. However, valuation of these factors does depend on the type of group. Implications of these results to healthcare a behavioral nursing intervention are discussed in this paper.
Purpose. To test the effects of a physical-cognitive exercise intervention on gait parameters under dualtask conditions in community-dwelling older adults.Design. A repeated-measures quasi-experimental design, with control and exercise groups, was used.Setting. Study participants consist of a convenience sample recruited from senior citizens' centers in Monterrey, Mexico.Subjects. A total of 143 sedentary participants ages 65 to 92 years per group participated. Intervention. A combined 45-to 60-minute program of physical and cognitive exercises was conducted in three weekly sessions during 12 weeks for the exercise group.Measures. The spatial gait parameters of speed (cm/s), step width, and stride length (cm); and the temporal parameters of single and double support time, cadence (steps per minute), and swing time (s) were measured using the GaitRite. Counting backwards or naming animals represented cognitive performance.Analysis. Two (groups: exercise group vs. control group) by three (time: baseline, week 6, and week 12) repeated-measures multivariate analysis of variance (MANOVA) was applied.Results. Repeated-measures multivariate analysis of variance revealed a significant group effect (Wilks lambda F 4,279 ¼ 6.78, p , .001); univariate analysis showed significant differences for gait speed (m/s), stride length, cadence, step width, and double support time. Time-by-group interaction showed significance in gait speed and stride length.Conclusion. The exercise group participants showed increased gait speed, cadence, and stride length, and reduced their step width and time spent with both feet on the ground. Walking while simultaneously performing a cognitive task might prepare older adults for competing/interfering demands from their environments. Se propuso analizar las propiedades psicométricas de la Escala de Autoeficacia para el Ejercicio. Como objetivo secundario, se analiza la asociación de las dimensiones de la escala por tipo de actividad física en adultas mayores independientes de la comunidad. Investigación instrumental y empírica con dos muestras que suman 614 mujeres mayores (M= 70.27 años, DE= 6.3). Las participantes presentan independencia funcional e integridad cognitiva y son asistentes de clubes de adultos mayores del área urbana. Se aplicaron la Escala de Autoeficacia para el Ejercicio y el Cuestionario de Actividad Física de Adultos Mayores. Se presentan propiedades psicométricas de la escala, análisis descriptivos e inferenciales con matriz de correlación de Spearman. Se utilizaron SPSS versión 21,0 y LISREL 8,80. La adaptación al castellano en esta población presenta cuatro dimensiones con propiedades psicométricas aceptables: estrés, demandas en competencia, influencias externas y el clima. Se estimó un gasto de 1,25 METs (DE= ,54) de actividad física en el hogar, 5,83 METs (DE= 4,40) de ejercicio y 2,09 METs (DE= 1,90) del tiempo libre. Se presenta modelo de ajuste y propiedades psicométricas de la Escala de Autoeficacia para el Ejercicio. For cultural reasons, for many women in Mexico the adoption of active behavior was restricted to the execution of tasks at home. It was proposed to analyze the psychometric properties of the Self-efficacy to Regulate Exercise Scale. As a secondary objective, the association of scale dimensions by type of physical activity in older adults independent of the community was analyzed. Instrumental and empirical research with two samples totaling 614 older women (M = 70.27 years, SD = 6.3). The participants present functional independence and cognitive integrity and were assistants of seniors clubs in the urban area. The Self-efficacy to Regulate Exercise Scale and the Physical Activity Scale of the Elderly were applied. Psychometric properties of the scale, descriptive and inferential analyzes with Pearson's correlation matrix are presented. SPSS version 21.0 and LISREL 8.80 were used. The adaptation to Spanish in this population has four dimensions with acceptable psychometric properties: stress, competing demands, external influences and climate. It was estimated an expenditure of 1,25 METs (SD= ,54) of physical activity in the home, 5,83 METs (SD= 4,40) for exercise and 2,09 METs (SD= 1,90) during free time. After the elimination of three items an adjustment model with four dimensions are presented. Directing strategies to increase self-efficacy secondary to external influences could increase the physical activity of this growing population group. Como objetivo secundário, analisa-se a associação das dimensões da escala por tipo de atividade física em idosos independentes da comunidade. Pesquisa instrumental e empírica com duas amostras, totalizando 614 mulheres mais velhas (M = 70,27 anos, DP = 6,3). Os participantes apresentam independência funcional e integridade cognitiva e são assistentes de clubes idosos na área urbana. Foram aplicadas a Escala de Autoeficácia para Exercício e o Questionário de Atividade Física para Idosos. São apresentadas propriedades psicométricas da escala, análises descritivas e inferenciais com a matriz de correlação de Spearman. Foram utilizadas as versões 21.0 e LISREL 8.80 do SPSS. A adaptação ao espanhol nessa população possui quatro dimensões com propriedades psicométricas aceitáveis: estresse, demandas concorrentes, influências externas e clima. Estima-se uma despesa de 1,25 METs (DP =, 54) de atividade física em casa, 5,83 METs (DP = 4,40) de exercício e 2,09 METs (DP = 1,90) de tempo livre. O modelo de ajuste e as propriedades psicométricas da Escala de Autoeficácia para o Exercício são apresentados. Direcionar estratégias para aumentar a autoeficácia secundária a influências externas pode aumentar a atividade física desse crescente grupo populacional.
A sample of 65 older adults (with and without diabetes) as well as a sample of 84 healthy young people were required to take affective priming studies to compare recognition latencies of stress related word pairs against recognition latencies of positive, negative and neutral word pairs. Moreover, older adults took a stress questionnaire related to relevant disturbing events in the third age. The goal was to test any automatic emotional processing bias to these events. Results suggested that even when people with diabetes obtained low stress test scores, they showed automatic cognitive bias to process stressful events differently than older adults without diabetes and young people. This suggested that people with diabetes patients' controlled strategies to cope with stress might not be aware of such an automatic cognitive bias. It is argued that this information processing style to stressful events makes patients prone to cognitive emotional vulnerability.
