Este estudo buscou compreender os sentidos atribuídos, pelos profissionais de saúde, à prática da sedação paliativa no processo de morrer, as relações desta prática com a eutanásia, bem como os valores considerados importantes e que subsidiam a equipe de cuidados paliativos na tomada de decisão. Trata-se de uma pesquisa exploratória e descritiva de abordagem qualitativa, baseada na hermenêutica dialética. Os resultados indicam que os sentidos atribuídos pelos profissionais entrevistados acompanham o desenvolvimento científico, os quais têm buscado esclarecer os fatos clínicos de cada processo de tomada de decisão e os valores implicados, seguindo a evolução da reflexão ética sobre a prática da sedação paliativa e eutanásia. No âmbito da reflexão ética, os princípios não são vistos como absolutos, pois, ao considerarem os valores, os profissionais analisam as circunstâncias e as consequências que permeiam as decisões.
IntroduçãoO mundo vem sofrendo constantes mudanças, mas a violência têm sido uma marca de todos os tempos, a qual muda de natureza, mas não diminui seu impacto nos índices de morbimortalidade. A violência tem acompanhado o homem ao longo de sua história, traduzida nos mais diferentes atos, revestida das mais diversificadas intenções. As guerras que num certo período da história da humanidade eram responsáveis pelos maiores números de mortes, no "mundo moderno" foram ultrapassadas por outras formas de violência que poderíamos intitular de uma guerra oculta, não declarada e apolítica, embora muitas vezes seja resultado de políticas perversas. Nesse sentido, as causas violentas têm sido nos dias atuais as principais responsáveis pela mortalidade no âmbito das "causas externas" as quais, no conjunto da mortalidade geral no Brasil, têm ficado atrás somente da mortalidade por doenças cardiovasculares e oncológicas (1,2) . Similarmente aos tempos remotos, as causas violentas foram o impulsor que levou o Estado a preocupar-se com medidas de intervenção, por intermédio do Setor de Saúde e de Segurança Pública.Frente ao aumento exacerbado da violência, doenças cardiovasculares, respiratórias, metabólicas entre outras, responsáveis pelas ocorrências de urgência/emergência, cresce, também, a necessidade de atendimento imediato das vítimas no local da ocorrência, bem como de transporte adequado para um serviço emergencial de atendimento definitivo. Nesse sentido, surgiram os Serviços de Atendimento Pré-hospitalar (SvAPH), os quais possibilitam a intervenção precoce, reduzindo os índices de mortalidade e minimizando seqüelas. Sobre esse surgimento -daquilo que hoje é denominado de Atendimento Pré-Hospitalar (APH) -, suas influências, desenvolvimento, atualidade e perspectivas, bem como o compromisso da Enfermagem com essa questão, é o que abordaremos no presente texto. Alguns marcos no contexto brasileiroOs SvAPH surgem no Brasil em diversas cidades e com características próprias, cuja sistematização é resultado de influências das duas tradicionais escolas de APH surgidas em meados do século passado: o Serviço de Atendimento Médico de Urgência (SAMU), modelo francês e o Serviço de Emergência Médica (SEM), modelo norte-americano. De modo geral, o modelo francês (SAMU) é composto por médicos anestesiologistas, intensivistas, cardiologistas, psiquiatras, emergencistas entre outros, técnicos auxiliares de regulação médica, enfermeiros (incluindo enfermeiros especializados em anestesia) e técnicos em ambulância. É o responsável pela assistência direta às emergências e pela ordenação e coordenação de todo o sistema através da Central de Regulação Médica. O modelo dos EUA é uma associação de esforços da first responder com o SEM que congrega os Técnico em Emergências Médicas (TEM-Básico habilitado para o Suporte Básico de Vida, TEM-Intermediário e TEM-Avançado ou Paramédico), departamento de emergência, médico supervisor, pessoal da saúde, administração hospitalar, administração do SEM e supervisão de agências governamentais (3) . The aim of this study was to understand how a palliative care team seeks solutions for ethical conflicts related to deep palliative sedation in everyday care and whether the discussions and the decisions that are made within the multidisciplinary team involve the sick person and his family. This is a descriptive exploratory qualitative study, based on dialectical hermeneutics. The research subjects were ten professionals on the health team of a hospital's Oncology Palliative Care department. The results indicate that a humanistic attitude assumes, among many things, the prudent use of palliative sedation as an available resource to minimize suffering during the process of dying. The practice of palliative sedation requires a thorough analysis of the clinical facts, ethical reflection by the multidisciplinary team, as well as respect for the values of the sick person and their family and their participation, which would result in a process of moral deliberation. ResumoDeliberação moral em sedação paliativa para uma equipe de cuidados paliativos oncológicos O objetivo deste estudo foi compreender como uma equipe de cuidados paliativos busca soluções para os conflitos éticos relacionados à sedação paliativa, no cotidiano assistencial, e se as discussões e decisões são feitas e deliberadas em equipe multiprofissional, envolvendo também o sujeito doente e sua família. Trata-se de pesquisa exploratório-descritiva de abordagem qualitativa, com base analítica na hermenêutica dialética. Os participantes da pesquisa foram 10 profissionais que integram a equipe de saúde de um setor hospitalar de cuidados paliativos oncológicos. Os resultados indicam que uma assistência humanizada pressupõe, entre tantos aspectos, o uso prudente da sedação paliativa como recurso disponível para a minimização do sofrimento no processo de morrer. A prática da sedação paliativa requer análise minuciosa dos fatos clínicos, reflexão ética em equipe multiprofissional, assim como participação e respeito aos valores da pessoa doente e seus familiares, o que propiciaria um processo de deliberação moral. Discute o significado da práxis e seus diferentes níveis, buscando compreender a práxis de Enfermagem como uma forma específica, identificando os seus elementos constitutivos. Argumenta em favor de uma práxis criativa e reflexiva de Enfermagem, reconhecendo que esta se configura numa articulação teórico-prática, fundada no conhecimento científico; práxis social específica que se constitui pela relação entre diversos atores sociais. Desta relação depende seu compromisso com a saúde do ser humano e da coletividade, com a preservação da vida; relação historicamente determinada-embora também determinante-pelo processo de produção em saúde, inserida numa relação mais ampla do respectivo modo de produção da vida em sociedade, por sua vez, principal determinante do processo de viver humano. Discute el significado de la praxis y sus distintos niveles, con la finalidad de comprender la praxis de Enfermería como una forma específica, identificando sus elementos constitutivos. Argumenta a favor de una praxis creativa y reflexiva de Enfermería, reconociendo que ésta se configura en una articulación teórico-práctica, fundada en el conocimiento científico; praxis social específica que se constituye por la relación entre los diversos actores sociales. De esta relación depende su compromiso con la salud del ser humano y de la colectividad, con la preservación de la vida; relación históricamente determinada-aunque también determinante-por el proceso de producción en salud, insertada en una relación más amplia del modo de producción de la vida en la sociedad, a su vez, principal determinante del proceso del vivir humano.
