We analyzed the frequency and morphological characteristics of the initial EEG manifestations of spontaneous seizures recorded from depth and subdural electrodes in 26 patients for whom pathological analysis of the area of seizure onset was available after resective surgery. Pathological features considered to be positive findings included well-defined structural lesions (hamartoma, neoplasm) or strictly defined mesial temporal sclerosis. Seizure onset was characterized by the frequency of the rhythmic discharge greater than 2 Hz in the first second and by the presence or absence of periodic low-frequency spikes (less than 2 Hz) preceding this stable change in background frequency. These features were correlated with the presence or absence of pathologic abnormalities in temporal and extratemporal locations. Although all patterns and frequencies of seizure onset were recorded in both medial temporal and extratemporal locations, medial temporal seizure onset was significantly more likely to have high frequency (greater than 13 Hz, p less than 0.00001) and tended to show periodic spikes prior to the seizure when it was associated with medial temporal sclerosis compared to when it was not. Extratemporal seizure onset associated with abnormal pathological substrate was significantly more likely to have a lower frequency (less than 13 Hz, p less than 0.05) and no periodic spikes before seizure onset (p less than 0.00001) than extratemporal seizure onset recorded from areas without pathological findings. Variability of seizure onset frequency was a characteristic of temporal, but not extratemporal, seizures (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)
The majority of patients with temporal lobe epilepsy show hippocampal sclerosis, which pathologically represents neuronal loss and gliosis. We studied volumetric neuronal density on a representative mid to mid-posterior level slice of hippocampi surgically removed from intractable temporal lobe epilepsy cases, and compared the results between 25 non-tumor epilepsy (NTE) cases and 5 tumor-associated epilepsy (TAE) cases. Eleven age-matched non-epileptic autopsy cases were studied as controls. Cells were counted in the CA1 through CA4 fields and the stratum granulosum of the dentate fascia. In NTE every hippocampal field showed statistically significant loss of neurons, the neuronal density in each field ranging from 35% to 50% of that of control. The mean neuronal density between the TAE and NTE groups also showed statistically significant differences in all hippocampal fields. The neuronal density of hippocampal fields of NTE ranged from 43% to 58% of that of TAE. Tumor-associated epilepsy cases, however, failed to show any statistically significant deviation from the control in their neuronal density. The etiology of the difference in neuronal density between the TAE and NTE groups is discussed.
INTRODUÇÃO: A paralisia cerebral (PC) é uma disfunção predominantemente sensório-motora que envolve distúrbios do tônus muscular, postura e movimentos voluntários, ocasionada por lesões cerebrais não progressivas. Esta condição de saúde interfere de forma importante na interação da criança em contextos relevantes, que influencia a aquisição e o desempenho dos marcos motores básicos, bem como nas atividades da rotina diária. É por isso que a acessibilidade é uma questão fundamental na vida das crianças com PC e um direito de todas elas para construir valores individuais e sociais, os quais eliminem os obstáculos e possibilitem a comunicação com o mundo. OBJETIVO: Verificar as dificuldades de acessibilidade, sob a ótica dos cuidadores primários, encontradas no cotidiano das crianças com PC. MÉTODO: Trata-se de um estudo qualitativo, de estudo de caso. Foram selecionados três cuidadores primários de crianças com PC que concordaram em realizar a entrevista e assinaram o TCLE. Os dados foram transcritos e categorizados. RESULTADOS E DISUSSÃO: Verificou-se que a escassez de informações sobre as potencialidades da criança com PC é comum na sociedade atual, o que contribui para existência de barreiras atitudinais, assim como para precariedade em acessibilidade geral destas pessoas. CONSIDERAÇÕES FINAIS: Conclui-se que é necessária inclusão de programas educativos nas escolas e maior apoio voltado aos cuidadores primários nas clínicas de reabilitação e outras instituições especializadas, pois mesmo diante das dificuldades as mães encontram formas e condições de cuidar e lutar por estas crianças.
OBJETIVO: Descrever um modelo experimental de vasectomia laparoscópica em ratos. MÉTODOS: Foram utilizados 35 ratos Rattus norvegicus albinus da linhagem Wistar. Após a anestesia, os ratos foram submetidos ao pneumoperitôneo através da agulha de Veress com PCO2 de 7mmHg. Depois, foram transpassados pela parede abdominal um trocater de 11mm de espessura e dois trocateres com 5 mm de espessura. Através destes foram introduzidos na cavidade abdominal à ótica (10mm) e as pinças utilizadas na dissecção, cauterização e secção do canal deferente e seus vasos. RESULTADOS: Durante o ato operatório dois animais (5,74%) apresentaram sangramento, contido com cauterização. Ocorreu um óbito na indução anestésica (2,85%) e outro (2,85%) no quarto dia pós-operatório de causa não identificada. Após 08 dias havia 33 ratos vivos (94,28%). CONCLUSÃO: Neste modelo, o material cirúrgico é o mesmo utilizado em humanos e são aplicados todos os procedimentos básicos da cirurgia laparoscópica, tornando-o factível para o treinamento de acadêmicos, médicos residentes e cirurgiões.
OBJETIVO: Desenvolver um modelo de esplenectomia laparoscópica em ratos. MÉTODOS: Foram utilizados trinta e cinco ratos machos ( Rattus Norvegicus Albinus, linhagem Wistar),pesando 250 +/- 50 g .Os animais foram anestesiados com cetamina e xylazina e foi estabelecido pneumoperitônio de CO2 com pressão de 7 mmHg, através da agulha de Veress. Após pneumoperitônio, dois trocateres de 5 mm e um de 11mm foram alocados na parade abdominal obedecendo a triangulação. O ligamento gastroesplênico foi dissecado para mobilização do baço. Os vasos hilares foram dissecados e eletrocoagulados utilizando a pinça "Maryland" e o gancho "HooK". Um "endobag" adaptado, foi utilizado para retirar o orgão da cavidade abdominal. RESULTADOS: Um animal (2,86%) morreu na indução anestésica. Após um período de observação de oito dias, trinta animais sobreviveram (85,7 %) e quatro (11,42%) Morreram no pós-operatório imediato. Em relação às complicações, Aderências intra-abdominais foram encontradas em 25,71% dos animais (n=9). CONCLUSÃO: A esplenectomia laparoscópica é um modelo factível por conta da baixa taxa de mortalidade além do baixo custo para treinamento da técnica.
