Background. The relative overgrowth of the greater trochanter is one of the most common deformities of the proximal femur in association with several disorders of the hip joint. Aim. To analyze the dynamics of proximal femoral growth after trochanteric epiphysiodesis as well as to determine the options for using this method in the complex treatment of children with hip pathology. Materials and methods. We analyzed the data of clinical and radiological examinations and surgical treatment (permanent trochanteric epiphysiodesis with metal fixation) outcomes for 43 (52 joints) patients aged 412 years with a developing high position of the greater trochanter. Results. The surgery enabled slowing down of the growth of the greater trochanter on the side of intervention by (average) 50% (p 0.05), although the values of the neck-shaft angle both on the affected side and the side opposite to it did not change (p 0.05). Conclusion. In moderate disorders of the growth plate of the femoral head epiphysis, trochanteric epiphysiodesis can prevent the progression and, in some cases, correct disturbed ratios of the hip joint, thereby avoiding the need for larger surgical interventions.
Актуальность. Дисплазия тазобедренных суставов и врожденный вывих бедра -часто встречающаяся ано-малия развития у новорожденных. По-прежнему не редки случаи позднего выявления заболевания, неудачи в лечении и развитие осложнений, что не находит должного отражения в современной литературе. Цель исследования -выявить наиболее распространенные ошибки на этапах диагностики и начала лечения детей первого года жизни с врожденным вывихом бедра. Материалы и методы. Работа основана на ретроспективном анализе данных анамнеза: истории диагности-ки и предшествующего лечения, а также клинического, сонографического, лучевого, магнитно-резонансно-го методов обследования 250 пациентов от 5 мес. до 2 лет, поступивших в специализированное отделение патологии тазобедренного сустава ФГБУ «НИДОИ им. Г.И. Турнера» с диагнозом «врожденный вывих бедра» в 2005-2015 гг. Результаты. Наиболее распространенными причинами ошибок на этапе диагностики врожденного вывиха бедра у детей являются отсутствие осмотра новорожденного ортопедом в роддоме и проведения сонографи-ческого скрининга, а также погрешности в его выполнении. При начале лечения отмечалось использование конструкций, не обеспечивающих оптимальное положение для вправления головки бедра, а также поздняя установка показаний к первичному хирургическому лечению в случаях невправимых вывихов бедра.Ключевые слова: врожденный вывих бедра, дисплазия тазобедренных суставов, диагностика, лечение.
Introduction. In standard planning for corrective hip osteotomy, a surgical intervention scheme is created on a uniplanar paper medium on the basis of X-ray images. However, uniplanar skiagrams are unable to render real spatial configuration of the femoral bone. When combining three-dimensional and uniplanar models of bone, human errors inevitably occur, causing the distortion of preset parameters, which may lead to glaring errors and, as a result, to repeated operations.Aims. To develop a new three-dimensional method for planning and performing corrective osteotomy of the femoral bone, using visualizing computer technologies.Materials and methods. A new method of planning for corrective hip osteotomy in children with various hip joint pathologies was developed. We examined the method using 27 patients [aged 5–18 years (32 hip joints)] with congenital and acquired femoral bone deformation. The efficiency of the proposed method was assessed in comparison with uniplanar planning using roentgenograms.Conclusions. Computerized operation planning using three-dimensional modeling improves treatment results by minimizing the likelihood of human errors and increasing planning and surgical intervention accuracy.
Background. Hip dysplasia, characterized by pronounced anatomical changes, continues to be one of the leading positions in the structure of all congenital malformations of the musculoskeletal system. Late diagnosis or ineffective conservative treatment leads to the need for surgical correction of congenital deformities of the proximal femur and acetabulum. The choice of the method of operational reconstruction of the latter remains a subject of discussion over the past decades. Aim of the study a comparative X-ray analysis of surgical treatment outcomes of children with hip dysplasia type IHDI IIIIV after Salter and Pemberton pelvic osteotomies. Methods. The study included 80 patients (80 hip joints) aged 2 to 4 years (3.10.45) with hip dysplasia of the IIIIV degree according to the IHDI. Patients were divided into two groups: group I consisted of 40 patients who underwent modified Salter osteotomy, group II 40 patients who underwent Pemberton pericapsular acetabuloplasty. Radiometry of the following parameters was performed: acetabular index (AI), Wiberg angle, neck-shaft angle, anteversion angle of the proximal femur, degree of bone coverage, the depth of the acetabulum (AD) and pelvic height (PH) Results. The values of AI and Wiberg angle in patients in I group indicate that a greater correction was achieved (p0.05) in contrast to patients who underwent Pemberton pericapsular acetabuloplasty. At the same time, the values of AD and PH in I group patients had pronounced differences (p0.05) from those in II group patients, whose values were close to similar indicators in the contralateral hip joint. Conslusions. In the treatment of children with hip dysplasia IHDI III-IV degree performing a modified Salter osteotomy leads to a significant decrease in the values of AI and an increase in the values of the Wiberg angle, which corresponds to hypercorrection of the position of the acetabulum, does not affect the AD and contributes to the elongation of the hemipelvis by an average of 13.8 mm. Pemberton acetabuloplasty allows to achieve values of AI and Wiberg angle close to the age-related indicators of the norm, leads to an increase AD, approaching the contralateral joint in its value and does not significantly affect the PH.
Background. Both in Russia and across the world, the annual frequency of total hip and knee replacement has increased, which has inevitably led to an increase in the number of patients with unsatisfactory treatment outcomes. This failure can primarily be attributed to the complications that arise, some of which arise due to the wrong choice of treatment approach. Aim. To analyze the publications on the results of treatment of patients with hip osteoarthritis and knee osteoarthritis by total hip and knee replacements. Materials and methods. The literature data were searched in open electronic databases of scientific literature PubMed and eLibrary using the following keywords and phrases: total hip/knee replacement, complications, patient satisfaction, age, obesity. The search depth was 20 years. Results. The data indicated a rise in the total number of such patients recorded annually. A significant number of publications, both in Russian and foreign literature, are devoted to the analysis of complications that mainly determine the patients satisfaction with an operation. In addition, these articles analyze the issues of determining the indications and contraindications in applying these methods of treatment. However, it is not uncommon for orthopedists to form a decision for performing an operation based on their subjective experience. At the same time, one of the circumstances that influences the decision-making is the lack of a stable system in the conservative treatment of degenerative joint diseases. According to the data from health care organizations, hip and knee replacements are the only available method for treating osteoarthritis. This circumstance may lead to an unjustified expansion of indications for arthroplasty, which would definitely affect the outcomes of treatment. In this situation, it is critical to search for precise criteria to identify the indication for total hip and knee arthroplasty. Conclusion. Unjustified expansion of indications for endoprosthesis leads to an increase in the number of complications, which in turn leads to an increase in the number of patients who remain unsatisfied with the outcomes of hip and knee arthroplasty in the long term. This circumstance encourages special scientific research to clarify the indications and contraindications for hip and knee replacements, especially in young and middle-aged people.
Актуальность. Транспозиция вертлужной впадины является распространенным методом хирургического лече-ния диспластической патологии тазобедренного сустава. Подвздошная остеотомия таза по Salter широко при-меняется при хирургическом лечении детей до 7-8-летнего возраста. У пациентов старшего возраста лонное сочленение и связки таза становятся ригидными, что значительно ограничивает степень ротации вертлужной впадины. В этих случаях для повышения мобильности ацетабулярного фрагмента применяют двойную (лонно-подвздошную) или тройную остеотомию таза. При этом остеотомия лонной кости производится в непосред-ственной близости от бедренного сосудисто-нервного пучка, что чревато его повреждением. Цель. С целью снижения травматичности хирургического вмешательства, профилактики интраоперацион ных сосудистых осложнений и повышения послеоперационной стабильности тазового кольца была разработана технология подвздошно-седалищной остеотомии таза. Материалы и методы. В статье описан разработанный авторами способ транспозиции вертлужной впади-ны после подвздошно-седалищной остеотомии таза. Прослежены результаты 99 хирургических вмешательств с применением предложенного способа у 89 детей с диспластической патологией тазобедренного сустава. Заключение. Подвздошно-седалищная остеотомия таза является эффективным методом лечения детей в воз-расте от 9 до 16 лет с нарушением соотношений в тазобедренном суставе диспластического генеза и может применяться, когда необходимо выполнить ротацию ацетабулярного фрагмента более чем на 25° и при этом нет необходимости медиализировать тазобедренный сустав.Ключевые слова: дети, диспластический коксартроз, остеотомия таза. Введение. Все чаще в различных отраслях хирургии при планировании и выполнении оперативных вмеша-тельств применяются 3D-моделирование и прототипирование. В ортопедии данная технология впервые по-явилась в 1990 г. при выполнении операции на коленном суставе. С тех пор началась разработка протоколов создания и применения индивидуальных шаблонов для навигации при хирургических вмешательствах на раз-личных костях скелета. Цель исследования -разработка нового трехмерного метода планирования и выполнения корригирующей остеотомии бедренной кости с использованием индивидуального шаблона, а также выявление преимуществ предложенного метода по сравнению со стандартной методикой планирования и выполнения оперативного вмешательства. Материалы и методы. Представлен новый способ планирования и выполнения корригирующей остеотомии бедра у детей с различной патологией тазобедренного сустава. Проведен анализ результатов планирования и выполнения корригирующей остеотомии бедра у 27 пациентов в возрасте от 5 до 18 лет (32 тазобедренных сустава) с врожденной и приобретенной деформацией бедренной кости. Заключение. Использование компьютерного 3D-моделирования при планировании и выполнении корригиру-ющих вмешательств на бедренной кости улучшает результаты лечения за счет практически идеальной точно-сти выполнения; минимизации возможных субъективных ошибок; сокращения времени операции; уменьше-ния лучевой нагрузки на пациента.Ключевые слова: дети, корригирующая остеотомия бедра, 3D-моделирование, индивидуальный шаблон. Introduction. Three-dimensional (3D) modeling and prototyping are increasingly being used in various branches of surgery for planning and performing surgical interventions. In orthopedics, this technology was first used in 1990 for performing knee-joint surgery. This was followed by the development of protocols for creating and applying individual patterns for navigation in the surgical interventions for various bones. Aim. The study aimed to develop a new 3D method for planning and performing corrective osteotomy of the femoral bone using an individual pattern and to identify the advantages of the proposed method in comparison with the standard method of planning and performing surgical intervention. PLANNING CORRECTIVE OSTEOTOMY OF THE FEMORAL BONE USING THREE-DIMENSIONAL MODELING. PART II
Background. The formation of multiplanar deformities of the proximal femur, in most cases combined with hypertrophy of the greater trochanter (relative overgrowth of the greater trochanter (ROGT)) and its high position relative to the femoral head, up to the development of pelvic and pelvic spine syndrome (trochanteric-pelvic impingement), has been considered one of the most common problems in the treatment of children with hip joint pathology of various etiologies. Aim. The aim of this study was to determine the causes of and characterize the X-ray anatomical changes in children with ROGT. Materials and methods. This study is based on an analysis of the survey results of 350 children 3 to 17 years old with an emerging high position of the greater trochanter due to various diseases of the hip joint. Details of the radiological indicators characterizing the change in the growth of the greater trochanter relative to the head and neck of the thigh were examined in 68 of these children (112 joints). Results. Most often, hypertrophy of the greater trochanter was observed in children with the sequele of ischemic disorders that occurred during the conservative treatment of hip dysplasia and developmental hip dislocation, as well as due to previous hematogenous osteomyelitis. It was revealed that in the affected hip joints, there was a regular decrease in the articulo-trochanteric distance index; simultaneously, TTD values, which characterize the isolated growth of the greater trochanter, were almost the same in normal and pathological conditions (p 0.05). Conclusion. Damage to the growth plates of the pineal gland and neck of the femur of various etiologies was the reason for ROGT formation. The X-ray anatomical changes include progressive shortening of the femoral neck. Moderately pronounced in preschool-age children, they progress with the childs growth and become the cause of chronic trauma injuries of the components of the hip joint.
