ResumoIntrodução: a cabeça é um dos fatores determinantes da posição do corpo durante o desenvolvimento da postura humana, caracterizada pelo equilíbrio entre as estruturas de suporte, envolvendo mínimo esforço e sobrecarga com máxima eficiência. As disfunções temporomandibulares (DTM's) são condições multifatoriais e, muitas vezes, estão relacionadas à má postura. A atividade alterada da musculatura mastigatória, comum nas DTM's, interfere nos músculos de contra apoio, levando ao encurtamento dos músculos posteriores cervicais e consequente desequilíbrio postural do crânio. Objetivo: verificar a associação entre flexibilidade da cadeia posterior e severidade das DTM's. Metodologia: trata-se de um estudo observacional, em corte transversal, em uma amostra não probabilística de universitários entre 18 e 50 anos. Após assinatura do TCLE, os voluntários responderam ao Questionário Anamnésico de Fonseca e, em seguida, foram submetidos ao teste de flexibilidade do Banco de Wells, com e sem calço molar, em máxima intercuspidação habitual. Resultados: foram avaliados 57 voluntários com idade média de 21 anos, sendo oito do sexo masculino e 49, do feminino. Verificou-se o predomínio de DTM leve, seguido de DTM moderada, ausência de DTM e DTM severa. Foi verificado, ainda, que a flexibilidade da cadeia posterior aumentou em todos os indivíduos quando realizado teste com o calço molar. A diferença das médias nos testes foi maior no grupo DTM moderada seguida de DMT severa, sem DTM e DTM leve. Conclusão: houve maior prevalência de DTM's do tipo leve, assim como associação entre flexibilidade da cadeia muscular posterior e grau de severidade das DTM's. Já os voluntários com DTM moderada apresentaram maior diferença entre as médias nos testes do que indivíduos com outros graus de severidade. Purpose: to investigate the possible association between the severity of the temporomandibular disorder, cervical pain, and mandibular function impairment. Methods: is a cross-sectional, descriptive study, conducted with 32 individuals with temporomandibular disorder, categorized by degree of severity, according to the Fonseca Index. Using the diagnosis criteria for temporomandibular disorder, the likely etiological factors for the disorder were established, as well as the intensity of the functional disability, resulting from cervical pain and of the mandibular impairment. The data obtained were statistically treated, adopting the significance level of 5%. Results: the mean age was 33.8 years, 90.6% being females. As for the degree of disorder, 56.3% presented severe TMD, followed by 28.1% showing a moderate one. The myogenic etiology was present in 93.7% of the patients. Cervical pain was present in 90.6% of them, of which, 59.4% presented a mild disability, and 25%, a moderate one. Considering the mandibular function, 46.9% of the patients presented a low, 40.6%, a moderate, and 12.5%, a severe impairment. There was a statistically significant association between cervical pain and mandibular function (p = 0.011). However, although there was an increase in cervical disability and in mandibular impairment as the severity of the TMD also increased, these associations were not statistically significant (p = 0.178 and p = 0.102, respectively). Conclusion: it can be stated that there is a higher prevalence of severe TMD and of myogenic origin, and that cervical pain influences, directly, the mandibular function, which is not necessarily related to the severity of the temporomandibular alteration. Likewise, such severity does not present a relationship to mandibular function impairment either.
ResumoA articulação temporomandibular (ATM) é a articulação mais complexa do corpo humano e faz parte do s istema estomatognático. Amplamente estudada pelas ciências da saúde, as alterações nesta região são conhecidas por disfunção temporomandibular (DTM) e referem-se, normalmente, a processos inflamatórios e/ou danos teciduais. O presente estudo tem como objetivo realizar uma revisão de literatura dos últimos cinco anos sobre DTMs, inflamação e dor. Foram encontrados quarenta e cinco artigos e permaneceram os vinte e cinco últimos artigos publicados de acordo com os critérios de inclusão. Os temas analisados foram: conceito de DTM, dor e inflamação nas DTMs; tratamentos e exames diagnósticos da inflamação nas DTMs; patologias associadas às DTMs. Fica claro o envolvimento de mediadores inflamatórios nas patologias da ATM assim como a relação entre dor e inflamação nesta articulação. O sistema nervoso central assim como hormônios, sexo, idade, grau e severidade da disfunção fazem parte das alterações e influenciam a dor e a inflamação. Exames radiológicos como a ressonância magnética (padrão-ouro), tomografia computadorizada e ultrassonografia devem ser realizados quando do aparecimento da sintomatologia, visando à prevenção de desgastes articulares e de piora do quadro em longo prazo. AbstractThe temporomandibular joint (TMJ) is the complex articulation of the human body and is part of the s tomatognathic system. Widely studied by the sciences of health, changes in this region are known for temporomandibular disorders (TMD) and usually refer to inflammatory processes and / or tissue damage. The present study aims to conduct a literature review of the last five years on TMD, inflammation and pain. We found forty-five articles and twenty-five published articles remained in accordance with the criteria for inclusion. The subjects discussed were: the concept of TMD pain and inflammation in TMD, treatments and diagnostic tests of inflammation in the TMDs, pathologies associated with TMD. Clearly the involvement of inflammatory mediators in TMJ disorders and the relationship between pain and inflammation in this joint. The CNS as well as hormones, sex, age, level and severity of dysfunction are part of the changes and influence pain and inflammation in TMD. Radiological examinations such as MRI (gold standard), CT and U.S. should be performed when the onset of symptoms in order to prevent joint wear and worsening of the long term.
<p>Não há um consenso na literatura quanto à associação entre postura craniocervical (PCC), disfunção temporomandibular (DTM) e oclusão dentária. Ainda assim, admite-se que a articulação temporomandibular está intimamente envolvida com a biomecânica cervical e cintura escapular. <strong>Objetivo:</strong> Traçar o perfil das associações entre PCC, DTM e oclusão dentária. <strong>Método:</strong> Estudo observacional em corte transversal realizado em universitários, de ambos os sexos, com idade entre 18 e 40 anos. Os voluntários foram avaliados pelo RDC/TMD, biofotogrametria, sendo esta realizada através do Software de Avaliação Postural (SAPO) pelos ângulos A1(tragus-horizontal), A2 (tragus-C7-horizontal), A3 (tragus- acrômio-vertical) e distância toraco cervical, além da análise quanto ao tipo de oclusão dentária. O tratamento dos dados foi feito a partir de medidas de tendência central, razão de prevalência e análise de correspondência múltipla assimétrica. <strong>Resultados: </strong>Foram avaliados 88 voluntários com mediana da idade de 22 anos, sendo 27 do sexo masculino e 62 do feminino. A amostra foi dividida em quatro grupos, de acordo com a presença de DTM, padrão de dor à palpação, cefaléia, PCC e oclusão. As maiores associações observadas foram: DTM tipo II com a distoclusão tipo dois, DTM tipo I com flexão de cabeça e retificação da coluna cervical; mesioclusão com alinhamento da coluna cervical e indivíduos sem dor com anteriorização da cabeça e distoclusão tipo um. <strong>Conclusão</strong>: Este estudo demonstrou que a DTM não interfere na postura da cabeça, mas que a má oclusão pode alterar a postura craniocervical nos planos sagital e frontal além de aumentar a lordose cervical.</p>
