The outcome of Helicobacter pylori infection has been associated with specific virulence-associated bacterial genotypes. The present study aimed to investigate the gastric histopathology in Portuguese and Colombian patients infected with H. pylori and to assess its relationship with bacterial virulence-associated vacA, cagA, and iceA genotypes. A total of 370 patients from Portugal (n = 192) and Colombia (n = 178) were studied. Corpus and antrum biopsy specimens were collected from each individual. Histopathological features were recorded and graded according to the updated Sydney system. H. pylori vacA, cagA, and iceA genes were directly genotyped in the gastric biopsy specimens by polymerase chain reaction and reverse hybridization. Despite the significant differences between the Portuguese and Colombian patient groups, highly similar results were observed with respect to the relation between H. pylori genotypes and histopathology. H. pylori vacA s1, vacA m1, cagA+ genotypes were significantly associated with a higher H. pylori density, higher degrees of lymphocytic and neutrophilic infiltrates, atrophy, the type of intestinal metaplasia, and presence of epithelial damage. The iceA1 genotype was only associated with epithelial damage in Portuguese patients. These findings show that distinct H. pylori genotypes are strongly associated with histopathological findings in the stomach, confirming their relevance for the development of H. pylori-associated gastric pathology.
vacA s1 and cagA+ H. pylori strains are associated with duodenal ulcer, gastric ulcer or gastric carcinoma. vacA m1 is associated with gastric ulcer or carcinoma but not with duodenal ulcer. Infection with multiple H. pylori strains is remarkably high in Portugal and is more frequent in duodenal ulcer patients.
A Terapia Transfusional resulta na transfusão de componentes específicos do sangue, dos quais o paciente necessita repor, opondo-se a utilização de sangue total. Este procedimento visa o beneficio de vários pacientes, e a otimização dos estoques dos bancos de sangue. A presença de anticorpos antieritrocitários no soro de pacientes podem ocasionar reações hemolíticas. É extremamente necessária a identificação desses anticorpos para a busca de hemocomponentes compatíveis e consequentemente a prevenção de reações transfusionais. O objetivo deste trabalho é de descrever a importância das técnicas imunohematológicas pré transfusionais no processo de transfusão de hemocomponentes. Trata-se de uma revisão bibliográfica de caráter qualitativo que se baseia na produção científica a partir de estudos já publicados. A decisão de transfundir um paciente deve considerar os riscos e benefícios e avaliar se há realmente necessidade de uma hemotransfusão. A avaliação ocorre através dos valores de hemoglobina (para transfusões de concentrado de hemácias) e valores de plaquetas (para transfusões de concentrado de plaquetas) e os aspectos clínicos do paciente. A introdução de testes laboratoriais e uma melhor triagem dos doadores reduziram significativamente as taxas de mortalidade e riscos de infecções relacionadas a este procedimento e as complicações mais frequentes atualmente são relacionadas a causas não infecciosas. Pacientes que realizam transfusões sanguíneas com frequência podem sensibilizar-se, produzindo aloanticorpos, devido à presença de antígenos antieritrocitários presentes nas bolsas de sangue transfundidas. Esses pacientes apresentam um risco maior no desenvolvimento de reações transfusionais, por isso, a importância das técnicas imunohematológicas pré transfusionais que tem como função principal rastrear a presença de anticorpos irregulares nos receptores e encontrar bolsas compatíveis para a hemotransfusão, diminuindo o risco de reações transfusionais e aumentando a segurança do paciente.
A Síndrome de Guillain-Barré tem diversas consequências negativas de acordo com a gravidade de seus sintomas. A perda da independência funcional é um dos fatores que mais interferem na qualidade de vida, tornando-a deletéria; além disso, cabe destacar que se trata de uma doença multifatorial. Com isso, a presente pesquisa objetivou analisar as relações entre a Síndrome de Guillain-Barré e as infecções virais. Trata-se de uma revisão integrativa da literatura, de caráter exploratório, com abordagem qualitativa, realizada nos meses de Abril a Junho de 2020. Para a realização desse estudo, foram consultadas as bases de dados: Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Eletronic Library Online (SCIELO). Utilizou-se os Descritores em Ciências da Saúde (DeCS) de forma associada: Sindrome de Guillain Barré; COVID-19; Neurológia; Infecções por Vírus. Assim, foram considerados inicialmente 704 estudos com a temática proposta; dentre estes, foram excluídos 622 estudos, de acordo com os critérios de exclusão estabelecidos. Após a aplicação dos critérios de inclusão, restaram 9 estudos. Os estudos incluídos foram tabelados e foram feitas revisões críticas e corroborativas entre cada um deles. Conclui-se que há um aumento nos casos de Guillain-Barré durante os surtos virais, com um aumento na incidência e no número de casos/mês. No geral, observa-se que entre as principais causas de Síndrome de Guillain-Barré estão os processos infecciosos.
