Chinese and U.S. human resource management systems differ on a number of cultural dimensions. The most important of these are described with respect to fundamental organization and work‐related assumptions about people and performance, rewards, training and development, and educational background of human resource practitioners. An appreciation of and respect for these differences is a prime requirement for effecting a successful Sino‐American venture. This is especially important given that China is the world's largest market, and because U.S. companies are recently finding that joint ventures with China are paying off. This paper helps business people and academics understand the world's fastest growing economy and the growing influence of Confucian Dynamism that affects HRM practice in Chinese ventures. The individualism‐collectivism dimension and the psychological contract also helps managers understand cultural differences and apply appropriate management techniques.
Zusammenfassung
Hintergrund In der Behandlung von Kahnbeinpseudarthrosen gelten operative
Verfahren als der „Goldstandard“. Diese sind jedoch mit möglichen Komplikationen
vergesellschaftet und bedeuten meist einen beträchtlichen operativen Aufwand.
Die hochenergetische extrakorporale Stoßwellentherapie (ESWT) stellt ein
nicht-invasives Therapieverfahren dar, welches in der Behandlung von
Pseudarthrosen eingesetzt wird. Ziel der Studie war die Untersuchung der
Konsolidierungsrate und des klinischen Behandlungsergebnisses bei verzögerter
Heilung von Kahnbeinfrakturen und bei Kahnbeinpseudarthrosen nach ESWT.
Patienten und Methoden Von 2010 bis 2017 wurden 42 Patienten (37 Männer
und 5 Frauen) mit einem Durchschnittsalter von 28,3 (15–70) Jahren aufgrund
einer verzögerten Knochenheilung einer Kahnbeinfraktur (21) bzw. einer
Kahnbeinpseudarthrose (21) mittels ESWT und anschließender Ruhigstellung im
Unterarmgipsverband mit Daumeneinschluss von 6–8 Wochen behandelt. Bei 13
Patienten war primär eine Behandlung der Kahnbeinfraktur erfolgt, 8mal mittels
alleiniger Ruhigstellung, 5mal mittels Verschraubung. Von den 29 primär nicht
behandelten Patienten waren 5 sekundär erfolglos operativ therapiert worden,
2mal mittels Verschraubung und 3mal mittels Transplantation einer medialen
Femurkondyle. Die Knochenheilung wurde nach 10–12 Wochen mittels
Feinschnitt-Computertomographie in Längsrichtung des Kahnbeines überprüft, wobei
das Kahnbein als geheilt eingestuft wurde, so sich eine Durchbauung von mehr als
50 % seines Querschnitts fand. Es wurde analysiert, welche Faktoren die Heilung
(Lokalisation der Fraktur/Pseudarthrose, skapholunärer Winkel, Alter der
Patienten, Body Mass Index (BMI), Tabak- sowie Alkoholkonsum) beeinflussen.
Zusätzlich konnte eine klinische Nachuntersuchung von 34 Patienten erfolgen,
dabei kamen der DASH-Fragebogen, der Mayo Wrist Score sowie zur Analyse der
Schmerzangaben die Visuell Analog Skala (VAS) zur Anwendung.
Ergebnisse Bei 71 % der Patienten kam es durch die Anwendung der ESWT zu
einer Konsolidierung. Bei 7 der 8 Patienten mit Ruhigstellung sowie 4 der 5
Patienten mit Kahnbeinverschraubung als Primärtherapie kam es nach der ESWT zur
knöchernen Heilung des Kahnbeines. Bei den sekundär erfolglos operativ
behandelten 5 Patienten kam es bis auf einen Patienten nach Transplantation
einer medialen Femurkondyle zur knöchernen Heilung. 15 der 24 nicht
vorbehandelten Kahnbeine heilten nach der ESWT knöchern aus. Der Zeitraum
zwischen der ESWT und der dokumentierten Heilung betrug im Mittel 8,4 Monate
(256 Tage).Hinsichtlich verschiedener Einflussfaktoren wie Frakturlage, skapholunärer
Winkel, Alkohol- und Tabakkonsum sowie biometrischen Faktoren wie Alter und BMI
konnte kein signifikanter Unterschied zwischen den mittels ESWT erfolgreich bzw.
erfolglos behandelten Patienten festgestellt werden. Die Nebenwirkungen der ESWT
begrenzten sich auf temporäre lokale Rötungen und petechiale Blutungen.
Schlussfolgerung Die ESWT ist sowohl zur Behandlung einer verzögerten
Heilung einer Kahnbeinfraktur als auch etablierten Kahnbeinpseudarthrose mit
einer Ausheilungsrate von 71 % durchaus geeignet, zumal die klinischen
Ergebnisse, was die Komplikationen, die Beweglichkeit und die Schmerzen
betrifft, befriedigend sind. Allerdings bedarf es weiterer Untersuchungen z. B.
des morphologischen Erscheinungsbildes, um herauszuarbeiten, welche
Frakturen/Pseudarthrosen das Potenzial zur knöchernen Heilung nach ESWT haben
und welche eher nicht.
