Stone depth has a significant influence on treatment outcome after piezoelectric SWL for both renal and ureteral calculi. We recommend particular attention be given to corpulent patients presenting with ureteral stones.
Piezoelectric SWL without intravenous sedation is suitable for the treatment of upper ureteral calculi. However, such an approach is less efficient in the management of kidney stones, especially for female patients.
RI~SUMI~1127 patients infertiles prdsentant une oligospermie < 5 millions/ml ont 6td pris en charge entre mars 1987 et janvier 1998. Parmi eux, 876 (age moyen 35,2 ans) ont dt4 suivis pendant une durde minimale de 1 an (suivi moyen 3,2 ans). Durant ce suivi, tous ont effectual au minimum 2 spermogrammes ~ des intervalles de 6 mois ~ 3 ans.Nous avons dtudi4 rdvolution de la qualitd du sperme et tent4 de ddgager des critbres pronostiques pr4dictifs de l'4volution de ces spermes au cours du temps.On retrouve une tendance ~ l'aggravation des diff4rents param~tres spermatiques au cours du suivi, et ce d~s le 6bme mois. A 4 ans, plus de 2/3 des spermes se sont altdrds, aucun ne s'est normalisd. Parmi les crit~res prddictifs d'altdration, le % de formes mobiles apparait ~tre comme le plus important. Par ailleurs, la rapiditd d'alt4ration semble ~tre inversement proportionnelle au % de formes mobiles.En conclusion, m~me si le % de formes mobiles semble corrdld ~ la numdration initiale, celui-ci semble ~tre un facteur important pour apprdcier une altdration potentielle de la qualitd du sperme avec le temps. Un % de formes mobiles infdrieur ~ 20% est en faveur d'une alt4ration significative dans les 2 ans de suivi.
