The present study aimed to evaluate proinflammatory cytokine and vitamin D levels in rheumatoid arthritis (RA) and chronic periodontitis (CP) patients and healthy individuals before and after initial periodontal treatment. Overall, 17 CP patients with RA (RA + CP), 18 systemically healthy CP patients (CP), and 18 healthy controls (C) were included. Clinical periodontal measurements were recorded and gingival crevicular fluid (GCF) and blood samples were recorded. RA + CP and CP patients received nonsurgical periodontal treatment. Vitamin D, tumor necrosis factor (TNF)-α, receptor activator of nuclear factor-KB ligand (RANKL), and OPG levels were determined in GCF and serum. Baseline clinical parameters were similar in all periodontitis groups (P > 0.05) but were higher than that in controls (P < 0.05). Periodontal treatment improved clinical parameters in all periodontitis groups (P < 0.05). GCF vitamin D levels were higher in RA + CP and CP groups than in healthy controls, but these levels decreased in the RA + CP group after periodontal treatment (P < 0.05). Serum RANKL and GCF TNF-α levels in RA patients decreased after periodontal treatment (P < 0.05). Within the limitations of this study, the results suggested that GCF vitamin D levels are increased in RA patients and decrease after periodontal treatment; therefore, local vitamin D levels might be an important indicator of periodontal bone loss.

Aim The purpose of this randomized controlled clinical study was to evaluate the effect of non‐surgical mechanical periodontal therapy on the inflammatory status and severity of psoriasis in subjects with psoriasis. Material and methods The study population consisted of 92 periodontitis patients with psoriasis vulgaris suffering from an untreated periodontal disease. Two randomized groups were formed from these patients: immediate periodontal therapy (test group, n = 46) and delayed periodontal therapy (control group, n = 46). Periodontal clinical measures, on salivary interleukin 2, interleukin 6 and secretory immunoglobulin A levels and the Psoriasis Area and Severity Index (PASI) scores were evaluated at baseline and on the 8th week in control and test groups. Results Eight weeks after completion of non‐surgical periodontal therapy (test group) or initial examination (control group), a significant decrease was observed in interleukin 2, interleukin 6 level and in PASI score, whereas a significant increase was observed in secretory immunoglobulin A levels in the test group (p < .05). Conclusion Within the limits of this study, the results suggest that effective periodontal therapy improves the psoriasis condition in patients afflicted by both diseases.

ObjectivesThe purpose of this study was to determine the impact of nonsurgical periodontal therapy considering the salivary stress-related hormone and cytokine levels in the gingival crevicular fluid (GCF) on pregnant and nonpregnant women.Material and MethodsThirty non-pregnant (control group) and 30 pregnant women (test group) that met the study inclusion criteria were chosen. Only participants with gingivitis were included. Clinical data and samples of GCF and saliva were collected at baseline and after periodontal therapy. The levels of interleukin-1 beta (Κ-1β) and IL-10, and concentration of salivary chromogranin A (CgA) hormone were analyzed by enzyme-linked immunosorbent assay (ELISA). The repeated measures analysis of variance was used for intragroup and intergroup analyses.ResultsA major decrease in the gingival inflammation was observed in both groups after periodontal therapy (p<0.05). Periodontal treatment decreased the level of IL-1β in GCF (p<0.05) in control group, but no statistical difference was determined for GCF IL-1β in the test group. However, after periodontal therapy, the CgA hormone concentration was reduced in both groups (p<0.05). However, there was no difference in salivary CgA concentration, GCF IL-10 levels, and perceived stress scale (PSS)-10 between the groups (p>0.05).ConclusionsWithin the limitations of this study, periodontal therapy significantly improved the periodontal status and stress level. In addition, the severity of the gingival inflammation during pregnancy was related to stress. However, further studies will be needed to substantiate these early findings.

Psychosocial stress may increase the risk of periodontal disease by altering the behavioral and immune responses of the individual. Therefore, the levels of stress should be taken into consideration in order to increase the efficacy of periodontal therapy in pregnant patients.

This reliable, valid and short questionnaire specific to aesthetic conditions perceived by patients with periodontal problems may serve as a valued instrument in future clinical trials.

Psikososyal faktörler periodontal hastalık için önemli bir risk faktörü olarak tanımlanır, konak direncini değiştirerek ve endokrin dengesizliklere neden olarak periodontal dokuları etkileyebilir. Bu çalışmada algılanan stres ve strese bağlı hormon düzeyiyle periodontal durum arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmaya 42 gingivitisli ve 30 periodontal olarak sağlıklı, toplam 72 birey dahil edildi. Hastaların klinik ve radyolojik periodontal değerlendirmesi yapıldı. Stres düzeyleri tükürük kortizol seviyesi ve Cohen Algılanan Stres Ölçeği (ASÖ-10) değerleri kullanılarak incelendi. Bireylerin tükürükteki kortizol seviyesi ELİSA yöntemi kullanılarak ölçüldü. Elde edilen veriler p<0.05 anlamlılık düzeylerine göre istatistiksel olarak analiz edildi. Bulgular: Sosyodemografik veriler değerlendirildiğinde fırçalama sıklığının kontrol grubunda daha yüksek olduğu görüldü (p<0.05). Periodontal olarak sağlıklı gruptaki tükürük kortizol seviyesi ve ASÖ-10 skorları, gingivitis grubuna kıyasla daha düşüktü (p<0.05). Tükürük kortizol seviyesi ve algılanan stres ölçeği skorları ile periodontal klinik parametreler arasında pozitif korelasyon gözlendi. Sonuç: Gingivitisli bireylerde daha yüksek stres seviyesi görülmektedir. Çalışmamızın sonuçları dahilinde stres ile periodontal hastalık arasında pozitif bir ilişki olduğu söylenebilir. Bununla birlikte, stres ve periodontal hastalık arasındaki ilişkiyi açıklamak için daha ileri araştırmalara ihtiyaç vardır.

The aim of this study was to evaluate incidence of root canal treatment depending on the gender, age, systemic diseases, and periodontal status. Method: 11-61+ year old 1547 teeth of 1060 patient who applied to the Department of Endodontics and Periodontology were included in the study. Gender, age, systemic diseases, and periodontal status of patients were recorded. Results: Female patients (52.4%) were received more root canal treatment than male patients (47.6%).In the age group between11-20 was needed the most frequency for root canal treatment. Cardiovascular disease (5.6%) was the most common disease seen in the patients participating in this study. 40.4% of patients including in the study had gingivitis. Conclusions: A detailed medical history of all patients requiring root canal treatment should be obtained and an appropriate periodontal therapy planning should not be forgotten accordingly.
