Purpose:To evaluate the effect of the ischemic preconditioning and the ischemic postconditioning over the tissue injury in the intestinal mucosa of rats undergoing the procedure of mesenteric ischemia and reperfusion. Methods: Thirty Wistar rats were studied, divided in three groups: group A, undergoing mesenteric ischemia (30 minutes) and reperfusion (60 minutes); group B, mesenteric ischemia and reperfusion preceded by ischemic preconditioning; group C, mesenteric ischemia and reperfusion and, before the beginning of reperfusion, the ischemic postconditioning was performed. At the end, a segment of the small intestine was dissected for histological analysis. The results were evaluated using the CHIU et al. 6 classification followed by the statistic treatment. Results: The mean values of the tissue injury levels were: group A, 3.5; group B, 1.2; and group C, 1. The difference between the result of group A with the results of groups B and C was considered statistically significant (p < 0,05). Conclusion: The ischemic preconditioning and postconditioning are able to minimize the tissue injury in the intestines of rats that underwent the procedure of mesenteric ischemia and reperfusion. Terapêutica Medicamentosa na Isquemia e Reperfusão Mesentérica: Revisão da LiteraturaCarlos Henrique Marques dos Santos e Cols.RESUMO: Até pouco tempo atrás se acreditava que na isquemia mesentérica todas as alterações orgânicas desta afecção eram devidas à obstrução total ou parcial do fluxo arterial intestinal. Recentes descobertas quanto à fisiopatologia do processo de isquemia e reperfusão mesentérica demonstraram que os radicais livres, principalmente, atuam durante a reperfusão, levando à lesão tecidual muito mais importante do que as lesões que ocorrem na fase de isquemia isoladamente. Assim, surge uma nova possibilidade terapêutica além do tratamento cirúrgico, em que uma determinada substância poderia atuar de modo a inibir ou minimizar a cascata de alterações no nível celular que culminam na lesão e morte celular. Realizamos uma ampla revisão da literatura médica atual pelos bancos de dados LILACS e MEDLINE, visando verificar quais os fármacos estudados para este fim e os resultados obtidos nas pesquisas. Constatamos que inúmeras substâncias têm sido avaliadas em estudos experimentais, em sua maioria utilizando ratos e a maioria não apresentou resultados satisfatórios a ponto de permitirem seu emprego na prática clínica; algumas, entretanto, apresentaram resultados promissores, necessitando ainda de novos estudos a fim de se descobrir uma substância que possa ser empregada em seres humanos em situações de isquemia e reperfusão mesentérica, a fim de se evitar tratamentos intervencionistas com altos índices de morbi-mortalidade.
Estudo evolutivo da anatomia das artérias coronárias em espécies de vertebrados com técnica de moldagem em acetato de vinil (vinilite)
Introduction: The maze procedure for primary atrial fibrillation treatment, initially proposed by Cox, is somewhat complex, and it increases the morbidity risk compared to isolated mitral valve surgery. Objective: To describe the surgical technique, the concepts of electrophysiology adopted and the preliminary results of a new surgical and electrophysiological approach, including specific atrial circuit blocking as defined by Frame and optimization of the surgical tactic for atrial fibrillation treatment. Method: Eight patients with chronic atrial fibrillation and mitral valve dysfunction, associated in one case with tricuspid valve regurgitation were operated on. The following modifications of the classic Cox procedure were employed: 1-Exclusion of the left atrium appendage by an internal suture closing its left atrial ostium; 2-Exclusion of the right atrium appendage by a purse-string suture used for fixation of the superior vena cava draining cannula; 3-Single atrial incision; 4-Trans-endocardium electrocauterization in the left atrium wall around all pulmonary vein ostia; 5-Substitution of the incisions and sutures in the left atrium by trans-endocardium electrocauterization. Results: The cardiopulmonary bypass time varied from 64 to 133 min (mean 107.5 min) and the cardioplegic time from 40 to 105 min. (mean 76.7 min). At the end of the surgery all patients were in regular atrial rhythm. The postoperative period was uneventful with all patients discharged from hospital showing regular atrial rhythm, without definitive pacemaker implantation. In the six-month postoperative follow up after surgery, six patients were in regular atrial rhythm (75%) with preserved atrial contractions, and two (25%) with clinically controlled atrial fibrillation (NYHA II). Also there were no embolic complications nor thrombotic evidence in the Doppler echocardiography control. Conclusion: It is concluded in this initial series of cases that the electrophysiological approach and the surgical technique employed improved the surgical treatment of the atrial fibrillation making correction of mitral and tricuspid valve injuries possible without additional morbidity.
Results: The mean time of extracorporeal circulation was 52.5 ± 13.3 min; aortic clamping, 35.6 ± 12.9 min; atrial ablation, 3.05 ± 0.16 min. All the patients were free of atrial fibrillation after the procedure; on hospital discharge, 69.5%; at 6 months, 91.3%; at 12 months, 76.4%; at 18 months, and at 24 months, 68.4%.
