The aim of this study was to investigate whether there is a difference between Alzheimer disease (AD) dementia and pure vascular dementia (VaD) in relation to nitrosative stress levels. We determined serum nitric oxide (NO), oxidized low-density lipoproteins (ox-LDL), and 3-nitrotyrosine levels (3-NT) in healthy elderly individuals (controls, n=15, mean age=73.4+/-7.9 y), AD patients (n=30; mean age=71.2+/-12.7 y) and VaD patients (n=20; mean age=75.2+/-7.5 y). Patients were under anticholinesterase drug therapy. Our results showed that serum NO and ox-LDL levels in AD patients group were significantly higher than in both controls (P<0.001 and P<0.001) and VaD patients (P<0.01 and P<0.001). However, no significant differences in plasma NO and ox-LDL levels were found between VaD and controls. Our study did not reveal significant differences in plasma 3-NT values of dementia patients when compared with controls. There was a significant negative correlation between Mini-Mental State Examination score and serum NO levels in dementia patients (r=-0.349, P<0.01). Serum NO levels were also positively correlated with serum ox-LDL levels (r=0.358, P<0.01). In conclusion, elevation in serum levels of NO was present in probable AD patients but not in VaD patients, and this seems to be related to both ox-LDL levels and cognitive status.
Repetitive nerve stimulation (RNS) is a highly sensitive electrophysiological test used for diagnosing myasthenia gravis (MG). Here, we evaluated electrodiagnostic value of RNS using facial nerve and occipitalis muscle. Patients with generalized MG were included. Clinical findings were assessed. RNS test was performed on occipitalis, nasalis, trapezius, abductor policis brevis (APB) and abductor digiti minimi (ADM) muscles. Twenty-five (78%) patients had relevant decrement during RNS test of at least one muscle. Nasalis muscle had the highest diagnostic potential (68.8%), followed by trapezius (53.3%), occipitalis (50%), APB (30%) and ADM (16.7%) muscles. There was not a significant relationship between clinical symptoms and facial RNS recorded on occipitalis muscle. In conclusion, we suggest that facial RNS recording over occipitalis muscle can be added in electrodiagnosis of MG because of cranial nerve innervation and proximal location. Facial RNS recording over occipitalis muscle provides a good choice under the conditions such as atrophy, cosmetic surgery, or botulinum toxin application in which nasalis muscle is unavailable for use.
Amaç: Bu çalışmada, Tip 2 diyabetli hastalarda serebrovasküler hastalık ile HbA1seviyeleri arasındaki ilişkiyi araştırmayı amaçladık.Yöntemler: HbA1c düzeylerini etkilediği bilinen demir eksikliği anemisi, trigliserid düzeyleri yüksekliği, üremisi, bilirubin düzeyleri yüksekliği saptanan ve kronik salisilat, alkol ve opiat kullanımı olan Tip 2 diyabetik hastalar dışlanarak; iskemik inme tanısı ile yatırılan diabetik hastalarda HbA1c ölçümü yapılan 60 hasta çalışmaya alındı. 34 hasta kadın, 26 hasta erkekti. HbA1c ölçümlerinde yüksek performanslı sıvı kromatografi metodu kullanıldı.Bulgular: Hastanemiz laboratuarının referans değerleri kullanılarak yapılan ölçümde hastaların %63'ünde yüksek HbA1c seviyeleri saptandı. Ancak bir kaynağa göre HbA1c'nin normal değeri üst sınırın 1,5 üstü kabul edilmektedir. Bu şekilde değerlendirme yapıldığında hastaların %43'ünde yüksek değerler bulunmaktadır. Bizim hastalarımızdaki HbA1c değerleri 60 hastanın 34'ünde %4,2-8, 21'inde %8-12,5'inde %12 ve üstünde idi.Sonuç: Glisemi kontrolü ile serebrovasküler hastalık gelişmesi arasında bir ilişki kurulamamıştır. Daha net sonuç elde edebilmek için daha fazla hasta grubu ile çalışmalar yapılmasının doğru olacağının kanısındayız.
Diyabetes Mellitus (DM) ülkemizde ve dünyada en sık görülen endokrin hastalıktır. Dünya Sağlık Örgütü (WHO) verilerine göre DM ve/veya komplikasyonlarından etkilenen kişi sayısı 1995 yılında 135 milyon iken, 2025 yılında 300 milyon olacağı öngörülmektedir (1).Diyabetes Mellitus'un en sık görülen nörolojik komplikasyonu diyabetik periferik nöropatidir ve DM'li hastaların yaklaşık %30'unu etkiler (2). DM arterioskleroz gelişme riskini arttırarak mikrovasküler ve nörolojik komplikasyonlara neden olur. Distal simetrik diyabetik polinöropati, gelişmiş ülkelerde periferik nöropatinin en yaygın görülen klinik formudur. Çok yavaş ilerleyen ve uzun süre asemptomatik ve sinsi kalan bir nöropati tipidir. Hastalarda polinöropatiyi gösteren ve en sık karşılaşılan belirtiler bacaklarda, distallerde belirgin karıncalanma, soğukluk hissi, ağrı ve parestezilerdir (3, 4).Diyabetik periferik nöropatinin erken dönemde saptanması, gelişebilecek komplikasyonların önlenmesi açısından büyük önem taşır. Bu çerçevede değerlendirildiğinde, tanıda elektrofizyolojik incelemeler bir adım öne çıkar. Nöropatinin erken dönemde tanınması, nöropatiye bağlı morbidite ve mortaliteyi azaltmasının yanında, bu hastaların yaşam kalitelerini arttırması açısından da önemlidir (5).Diyabetik nöropatinin progresyonu da glisemik kontrol ile yakından iliskilidir. Glikozile hemoglobin (HbA1c) düzeyi son 8-12 haftalık dönemdeki kan şekeri kontrolünün iyi bir göstergesi olup, diyabetik hastalarda uzun süreli glisemik kontrolün değerlendirilmesinde "altın standart" olarak kabul edilmektedir (6, 7).Çalışmamızın amacı HbA1c seviyesi ile distal simetrik diyabetik polinöropati arasındaki ilişkiyi saptamaktır.
IntroductionIn this study, we aimed to evaluate the effects of the development of aspiration pneumonia (AP) on the intensive care unit (ICU) requirements and in-hospital mortality of patients hospitalised in the neurology ward due to an acute cerebrovascular accident (CVA).Material and methodsFive hundred and three patients hospitalised in the neurology ward following an acute CVA were retrospectively analysed. The patients were divided into two groups: those with AP (group 1) and those without AP (group 2). Demographic characteristics and physical and radiological findings, including the localisation, lateralisation and aetiology of the infarction, in addition to ICU requirements and mortality, were evaluated.ResultsAspiration pneumonia was detected in 80 (15.9%) patients during the in-hospital stay. Transfer to the ICU for any reason was required in 37.5% of the patients in group 1 and 4.7% of those in group 2 (p < 0.001). In-hospital mortality occurred in 7.5% and 1.4% of the patients in group 1 and group 2, respectively (p = 0.006). The incidence of AP was highest in patients with an infarction of the medial cerebral artery (MCA) (p < 0.001). The AP was associated with older age (p < 0.001), hypertension (p = 0.007), echocardiography findings (p = 0.032) and the modified Rankin Scale (mRS) score (p < 0.001).ConclusionsOur findings suggest that the requirement rate for transfer to the ICU and the mortality rate appear to be significantly higher in patients with a diagnosis of AP. Precautions should be taken, starting from the first day of hospitalisation, to decrease the incidence of AP in patients with acute CVA, focusing especially on older patients and those with a severe mRS score.
