RESUMEN:Los tumores de las glándulas salivales representan entre el 2 y 3% de las neoplasias de cabeza y cuello. El carcinoma mucoepidermoide (CME) es el tumor maligno más común en las glándulas salivales; su comportamiento biológico se relaciona con el grado histológico tumoral (bajo, intermedio y alto). Está formado por células mucosas, epidermoides, intermedias, columnares y claras. El objetivo del presente trabajo fue presentar la casuística de CME de nuestro laboratorio en el período 1999-2009 para determinar grado histológico y potencial proliferativo como factores pronósticos ya que éstos son importantes para guiar el tratamiento y diseñar recomendaciones para los pacientes. Cortes seriados de 13 casos de CME se colorearon con H/E, Masson, PAS, Dane/Alcian blue y Azul de toluidina. Se realizó inmunomarcación para ki67. Se analizó su grado histológico según el sistema de gradación tumoral de Auclair et al., que evalúa variables observables con microscopio óptico. 8 tumores se localizaron en parótida, 3 en paladar, 1 en submandibular y 1 en glándulas genianas. Histológicamente 3 casos fueron de alto grado, 2 de grado intermedio y 8 de bajo grado, según sus características citológicas, patrón de invasión y tipo celular. En dos casos de bajo grado se observaron cristales de colesterol, macrófagos y células gigantes de cuerpo extraño. La expresión de ki67 fue importante en tumores de alto grado. La parótida fue la más afectada seguida por paladar y submaxilar, lo que acuerda con la bibliografía. El grado histológico tumoral se asoció con la expresión de Ki 67, factores pronósticos relevantes en el CME, que deben asociarse a estudios cínicos y análisis de supervivencia. Continuaremos el presente trabajo con la realización de estudios de los residuos azúcares terminales y subterminales de los carbohidratos con lectinhistoquímica, para detectar modificaciones sutiles de la glicosilación de las proteínas celulares, rasgo común en el proceso de transformación neoplásica.
RESUMEN:El estroma juega un rol importante en los procesos tumorales de invasión y metástasis. Las fibras de colágeno tipo I son el principal componente estructural del estroma en distintos tumores. Sin embargo, hay muy pocos estudios en los tumores de glándulas salivales. Basándonos en estos antecedentes el objetivo de la presente comunicación fue estudiar las características del colágeno con Picrosirius red/polarización en tumores benignos y malignos de glándulas salivales para evaluar su posible rol en los mecanismos de progresión tumoral. Cortes histológicos de adenoma pleomórfico, carcinoma adenoide quístico y carcinoma epitelial mioepitelial se colorearon con H/E y Picrosirius red y se examinaron con microscopio de polarización. La birrefringencia del colágeno con Picrosirius/polarización resultó diferente en el estroma de los tumores malignos (carcinoma adenoide quístico y carcinoma epitelial mioepitelial), con predominio de colágeno I, en comparación con el tumor benigno (adenoma pleomórfico), con predominio de colágeno III. El diferente perfil de coloración en las fibras colágenas producidas en el estroma de los tumores analizados podría relacionarse con diferentes mecanismos de expansión tumoral, los que fueron poco estudiados en los tumores de glándulas salivales. Más estudios son necesarios para obtener resultados más concluyentes que contribuyan al diagnóstico, pronóstico y tratamiento.
RESUMEN:El carcinoma de células claras es una neoplasia rara de bajo grado que se desarrolla casi exclusivamente en glán-dulas salivales menores humanas. El objetivo de esta comunicación es analizar las características estructurales, histoquímicas e inmunohistoquímicas (IHQ) de dos casos de carcinoma de células claras de parótida y realizar el diagnóstico diferencial con otros tumores salivales primarios y metastásicos que presentan células claras. Cortes de ambos tumores fueron procesados para H/E, tricrómicos de Masson y Dane, Azul de toluidina, Azul alciano, PAS y PAS/diastasa; marcaciones IHQ para citoqueratinas de bajo y alto peso molecular, ki67, HMB45, p63 y proteína S-100. El patrón estructural de estos tumores estaba determinado por nidos y cordones de células claras delimitados por un estroma no hialinizado. En un tumor se observó una neoplasia maligna a células claras con manifiesta anaplasia. La expresión de ki67 fue importante. El otro tumor estaba constituido por células claras monomorfas sin signos manifiestos de atipia y casi nula expresión de ki67. Con PAS se demostró la presencia de glucógeno y no se observó un desarrollo importante del estroma colágeno en ambas neoplasias. En ambos casos resultó francamente positiva la inmunomarcación para citoqueratinas de bajo y alto peso molecular. Por el contrario resultó negativa para HMB45 y p63. La proteína S-100 tuvo su expresión en células aisladas. Se concluye que los casos presentados son de localización poco común (parótida) según lo descripto en la bibliografía; corresponden a la variedad no hialinizante ya que no está presente un importante estroma colágeno hialinizado, y ambos tumores son primarios de glándu-las salivales, de origen epitelial, no melanocítico ni hematopoyético (linfomas) como lo demuestran las marcaciones IHQ.
