Мета дослідження: оцінити ефективність застосування трансплантації автологічних хондроцитів (АХ) у хворих після проведеної поперекової мікродискектомії у віддаленому періоді. Матеріали та методи. Проведено трансплантацію АХ 6 пацієнтам через 3 міс. після поперекової мікродискектомії. Оцінку клінічного стану проводили за шкалами VAS і NASS до операції та через 3 міс. після мікродискектомії, через 1 і 2 роки після трансплантації АХ. Також у ці періоди оцінювали магнітно-резонансні характеристики гідратації міжхребцевого диска. Результати. За даними магнітно-резонансної томографії встановлено відновлення гідрофільності пульпозного ядра у 4 із 6 пацієнтів (66,7 %), яким проводили АХ. Інтенсивність болю істотно знизилася вже в ранньому періоді після операції мікродискектомії, а через 3 міс. (на час проведення трансплантації АХ) зменшилася майже у 6 разів. Через 1 рік та 2 роки після трансплантації АХ цей показник продовжував поступово знижуватись. Функціональний статус та якість життя після операції мікродискектомії значно покращилися на момент проведення трансплантації АХ, а після даної процедури істотно не змінились. Висновки. Автотрансплантація хондроцитів у пацієнтів після виконання поперекової мікродискектомії є безпечним методом хірургічного лікування дегенеративних захворювань хребта, що позитивно впливає на біомеханічні властивості оперованого міжхребцевого диска.
Мета: проаналізувати результати моніторингу ефективності пункційної вертебропластики (ПВП) за даними комплексної оцінки якості життя (ЯЖ) у найближчий та віддалений післяопераційний період у хворих різних вікових груп з ізольованими травматичними компресійними переломами тіл хребців. Матеріали та методи. Проаналізовано показники ЯЖ у 160 хворих, прооперованих методом ПВП у клініці малоінвазивної та лазерної спінальної нейрохірургії ДУ «Інститут нейрохірургії ім. акад. А. П. Ромоданова НАМН України» у 2002-2016 рр. Вік хворих на момент проведення ПВП-від 15 до 78 років, середній вік-(50,3±13,1) року. У віці <60 років прооперовано 106 хворих, у похилому і старечому (≥60 років)-54. Тривалість періоду від моменту травми до операції становила 2,2 тиж у цілому по групі, 1,9 та 3,6 тиж-у хворих віком <60 років і ≥60 років відповідно. Усі хворі, залучені в дослідження, повідомили по значну силу травмувального агента (падіння з висоти, дорожньо-транспортна пригода). Критерієм вилучення із дослідження була низькоенергетична травма. Найбільший термін збору катамнезу становив 12,5 року. Результати. Індекс Освестрі (Оswestry Disability Index (ODI)), який до операції в цілому по групі становив (57,7±1,6)%, що відповідало інвалідизації тяжкого ступеня, через 3 дні після операції істотно (p<0,001) знизився і становив (31,2±1,3)%, що відповідало помірній інвалідизації. Через 3 міс ODI знизився до (18,8±1,4)% (p<0,05), що відповідало мінімальній інвалідизації. Через 6 міс спостерігали тенденцію до подальшого зниження ODI ((18,5±1,4)%). У віддалений післяопераційний період істотних змін ЯЖ і, відповідно, ступеня інвалідизації не відбувалось: у загальній групі ODI в середньому становив (18,4±1,3)% через 1 рік, (18,2±1,3)%-через 3 роки, (18,3±1,3)%-через >3 роки, що відповідало мінімальній інвалідизації. Клінічні результати ПВП у найближчий та віддалений період у хворих різних вікових груп істотно не відрізнялись. Висновки. Методика ПВП дає змогу в короткі терміни суттєво знизити ступінь інвалідизації хворих будь-якого віку з ізольованими травматичними компресійними переломами тіл хребців та значно поліпшити їх ЯЖ. Функціональні результати лікування цієї патології методом ПВП залишаються позитивними протягом тривалого (до 12,5 року) терміну спостереження. Objective — to analyze the effectiveness of percutaneous vertebroplasty (PV) in the short term postoperative period in patients with isolated traumatic vertebral compression fractures (IT VCF). The analysis of short term results of PV for IT VCF in patients (n=160) of two age groups: I — up to 60 years (n=106) and II — 60 years and older (n=54). The criteria for inclusion in the study were: high-energy spinal injury resulting a road accident or a high level fall; absence of signs of osteoporosis. The average pain intensity (M ± σ) before PV matched a severe pain (8.21±1.41 points according VAS in the general group). At 3 days after surgery, the pain radically decreased to insignificant (0.91±0.98 points), gradually decreased to 0.76±0.82 points during the first 3 months; there was a further tendency for its decreasing within the insignificant (up to 0.71±0.76 points after six months). At the same time, before the operation and during the nearest postoperative period, patients of group I had a somewhat lower intensity of pain than in group II. Dependence on painkillers observed in 100% of patients before surgery, in the first 3 days after PV 78.1% of all patients have lost it (80.2% — in group I, 74.0% — in group II); in 3 months — 95,6%, 96,2%, 94,5%, respectively; after 6 months — 96.2%, 97.2%, 94.5%, respectively. Motor activity before surgery was reduced in all patients (45.0% of them to the degree of bedbound). In 3 days after PVP, 60.6% of all patients returned to usual motor activity at home (65.1% in group I, 51.8% in group II), 93.7%, 3 months later, respectively, 2%, 87.0%; in 6 months — 96,3%, 99,1%, 90,7%. The persons of physical work who were employed at the time of injury, remained disabled 3 days after the PVP. Within 3 months, they all returned to work: to their usual work 83.7% in the former group, 90.7% in the first group and 52.9% in the second group; 16,3%, 9,3% and 47,1% of the patients, respectively, for work with facilitated conditions. After 6 months, the number of patients with full physical recovery in these groups reached 94.6%, 97.3% and 82.4%, respectively. Oswestry disability index (ODI), which before the operation was (57.7±20.1)% in the whole group, (55.7±20.85)% — in the first group and (59.6±18.54)% — in group II, and responded to severe disability, 3 days after surgery significantly (p<0.001) decreased and amounted to (31,2±15,89)%, (30,3±16,62)% and (31, 9±14,46)%, respectively, indicating moderate disability. After 3 months, ODI decreased to (18.8±17.45)% (p<0.05) in the general group and to (18.2±18.56)% and (19.1±15.18)% — in the relevant age groups, indicating a minimal disability. After 6 months, there was a tendency for further decrease of ODI (to (18.5±17.1)%) and (17.9±18.08)% and (19.0±15.24)%, respectively). So, in the short term after PV, the degree of disability of patients of both age groups decreased significantly. Thus, PV in patients with IT VCF provides rapid pain regression and improvement in the functional status of patients of both age groups — up to 60 years and 60 years and older. The most radical changes occur within the first 3 days after surgery. Further improvement of most options during the first 6 month is progressing gradually. The rates and completeness of the functional recovery are slightly higher in patients of the age group up to 60 years, compared with the group of 60 years and older.
Objective: To perform a comparative analysis of MRI data obtained in the early postoperative period after repeated lumbar microdiscectomies in patients with and without epidural injection of “Nubiplant” polyacrylamide hydrogel (HG). Material and methods: The MRI data of the lumbar spine in the early postoperative period after repeated removal of herniated disc (on the 3-15th day) in 84 (100%) patients were analyzed: 30 (35,7%) patients were injected intraoperatively epidurally with “Nubiplant” HG to prevent epidural fibrosis (main group (MG) and in 54 (64,3%) patients the HG was not injected (control group (CG). Results: Comparative analysis of MRI data on the 3-15th day after surgery showed that the frequency of epidural edema and hemorrhage signs within the postoperative area in the MG was significantly lower as compared to the CG (p = 0,0444 and p = 0,0288 respectively). To assess the accuracy of the epidural administration of an artificial biopolymer Nubiplant during lumbar microdiscectomy, in the early postoperative period the following MRI criteria could be helpful: i) absence of the dural sac deformation and dislocations of the spinal root; ii) well-defined margin of the adjacent spinal root; iii) homogeneous MRI signals of the Nubiplant zone; iv) absence of Nubiplant areas outside the postoperative area; v) sufficient sectoral coverage of the adjacent root with epidurally administered Nubiplant (optimally >1800). Nubiplant” HG in the patients of the MG was evaluated, and MRI criteria for assessing the correctness of its introduction were proposed. Conclusions: In the early period after repeated lumbar microdiscectomies (on the 3-15th day), intraoperative epidural injection of “Nubiplant” HG was accompanied by a significant decrease of epidural edema and hemorrhage signs within the postoperative area. The proposed criteria of correctness of HG “Nubiplant” introduction allow unifying the approaches in radiological assessment of this patients.
Background. Epidural fibrosis (EF) is one of the reasons of the failed back surgery syndrome manifesting in back pain and pain in extremities as well as other signs of the compression or irritation of the nerve structures. The numerous approaches to prevent EF have been elaborated. Nowadays, many years of experience in the usage of different barrier materials in microdiscectomy such as polyacrylamide hydrogel (PAH) have been accumulated. Nevertheless, the results of such treatment are rather controversial. The aim of the study was to assess the radiological data on the state of the backbone according to MRI findings and the clinical symptoms of the patients following the surgery as well as to determine the long-term strength of the interaction between the factors under study after the repeated lumbar microdiscectomies performed with intraoperative epidural PAH administration. Materials and methods. The results of the clinical and radiological assessment of 96 patients (mean age 45.7 years; 95 % CI [43.5; 47.9]; 59 males and 37 females) enrolled into the single-center prospective cohort study performed at the State Institution «Romodanov Neurosurgery Institute National Academy of Medical Sciences of Ukraine» have been presented. The patients were divided into two groups. The main group (MG) consisted of 35 (36.5 %) patients with recurrent intervertebtral disc (IVD) herniation to whom PAH was administered epidurally at the final step of the surgery comprising disc removal and decompression of the nervous structures with the aim of preventing cicatrical adhesive EF. In control group (61 patients, 63.5 %), barrier materials were not used. The primary end-points of the study were the long-term outcomes of the repeated lumbar microdiscectomies: EF rate at the postoperative site; the rate of the displacement of spinal root at the postoperative site; the rate of the repeated recurrent IVD herniation and de novo IVD herniation; the rate of the cases with the residual radicular pain localized in the area of the surgical intervention and the extent of the worsening of life quality in the treated patients. The secondary end-points of the study were the following: the analysis of the associations with delineation of the strength of such factorsaffecting the residual radicular pain as EF, the displacement of spinal root at the postoperative site, the repeated recurrent IVD herniation the repeated recurrent IVD herniation and de novo IVD herniation. Results. In 12 months following the surgical treatment, EF frequency was significantly lower in MG as compared with CG where PAH was not used (p = 0.02936). Nevertheless, the difference between groups was not significant when MRI findings of the displacement of spinal root at the postoperative site (р = 0.46759), the frequency of the repeated recurrent IVD herniation (p = 0.90904) and de novo IVD herniation (p = 0.60385) were compared. In study group, there were no association between MRI signs of postoperative EF, the displacement of spinal root or new recurrent IVD herniation on the one hand and the frequency of the radicular pain localized at the area of the surgical intervention on the other hand (p = 0.66482; p = 0.09515; p = 0.22857, respectively). In CG, the significant association between the displacement of spinal root at the postoperative site and the clinical symptoms above was revealed (p = 0.00222) with association coefficient φ = 0.41 indicating the moderate strength of this association. Conclusions. The data obtained generally suggest the reasonability of applying PAH manufactured in Ukraine as a barrier material at least for the treatment of the patients with recurrent IVD herniation representing the challenging category of spinal patients.
