Мета роботи -розробка моделі проведення занять за темою "ВІЛ-інфекція" на основі інтерактивних методів навчання задля позитивного впливу на засвоювання практичного матеріалу, підвищення зацікавленості, задоволеності навчанням та якості підготовки лікаря-інтерна.Основна частина. Запропонована нами модель інтерактивного навчання розроблена відповідно до типової програми під-готовки лікарів-інтернів з інфекційних хвороб та включає в методологію проведення такі інтерактивні методи проведення за-нять, як: робота в малих групах; дискусія; проведення інтерв'ю; тренінг із використанням ділової та рольової гри; самостійне складання студентами опитувальника; екскурсія та робота в лабораторії; case-навчання. Модель складена на прикладі вивчення інтернами тем розділу навчальної типової програми, що стосуються ВІЛ-інфекції, як а є одною з найбільш актуальних проблем сучасної інфектології. Методологія включає лекції, практичні заняття, семінари та самостійну підготовку інтерна. Кількість годин у моделі розрахована для спеціальності "Інфекційні хвороби". Запропоновані два етапи підготовки інтернів: вхідний та головний етап -виконання міжпрофесійного проекту. Вхідний етап виконання міжпрофесійного проекту включає лекції, проведення практичних занять з різними видами участі лікарів-інтернів (роботу в малих групах; дискусію в групі; тренінг із використанням рольової та ділової гри; підготовку інтернами запитань (опитувальник, підготовлений у групі) та проведення інтерв'ю з пацієнтами Центру боротьби та профілактики зі СНІД, аналіз отриманих відповідей; рольову та ділову гру: "лікар -пацієнт"; екскурсію в лабораторію; роботу в малих групах, "case studies"), самостійну роботу з нормативними документами з організації та проведення добровільного консультування і тестування.До головного етапу виконання завдань міжпрофесійного проекту залучені такі спеціалісти: фахівці соціальної медицини, СНІД-центру, медичний психолог, фтизіатр, невролог, онколог, дерматовенеролог, а також представник суспільного фонду "До-рога життя" -один із пацієнтів центру СНІД. Лікар-інтерн є активним учасником проекту та входить до складу команди. На цьому етапі вирішується ряд завдань, головним із яких є сумісна робота команди фахівців у наданні кваліфікованої допомоги пацієнтам із ВІЛ.Висновок. Інтерактивна методика навчання є найбільш актуальною в сучасних умовах викладання, які передбачають підго-товку висококваліфікованих спеціалістів, здатних інтегрувати теоретичні знання і практичні уміння в цілісну систему, володіти новими технологіями.Ключові слова: інтерактивні методи навчання; лікарі-інтерни; методологія; етапи заняття; ВІЛ-інфекція.
Резюме. Пацієнти 4 клінічної стадії ВІл-інфекції із неврологічними захворюваннями мають тенденцію до збільшення рівня сироваткового β 2 -мікроглобуліну (β 2 -МГ) та значні відмінності основних показників гематологічного та імунологічного про-філю порівняно з групою пацієнтів без захворювань нервової системи. Мета дослідження -визначити рівень β 2 -мікроглобуліну та його зв'язок з основними гематологічними та імунологічними показниками у пацієнтів 4 клінічної стадії ВІл-інфекції при наявності та відсутності захворювань центральної нервової системи. Матеріали і методи. У дослідження включено 77 пацієнтів із 4 клінічною стадією ВІл-інфекції віком від 20 до 56 років, яких поділили на дві групи: першу групу склали 38 пацієнтів із характерними для відповідної клінічної стадії захворюваннями, окрім неврологічних, другу -39 пацієнтів, які мали ВІл-асоційовані неврологічні захворювання. Групу контролю становили 15 здо-рових донорів відповідної статі та віку. Визначення β 2 -мікроглобуліну проводилось за методом ІФа зі стандартними тест-системами (вир. сша). статистичну обробку результатів проводили за допомогою ліцензійного програмного продукту statIstICa v.6.1®. Результати досліджень та їх обговорення. Встановлено значну відмінність між маркерами імунного статусу та показ-никами гемограми. серед тих, хто мав неврологічні захворювання (друга група), визначалась лейкопенія на рівні (3,88±0,27) Г/л проти (5,07±0,27) Г/л (р<0,01), середня кількість тромбоцитів була нижче в 1,2 раза (р<0,05), а показник шОе -біль-шим у 2,7 раза (р<0,001), ніж у пацієнтів 4 клінічної стадії ВІл без неврологічних захворювань (перша група). Відносна та абсолютна кількість Т-лімфоцитів хелперів (Тл) у пацієнтів другої групи була у 1,9 і 3 раза меншою за відповідні показ-ники у хворих першої групи (р<0,001). суттєві відмінності мала загальна кількість лімфоцитів (р<0,001) і Т-лімфоцитів (р<0,01). середній показник вірусного навантаження (ВН) рНК ВІл у хворих другої групи (log10 ВН -5,02 коп/мл) пере-вищував показник першої групи більше, ніж на порядок -3,90 коп/мл (р<0,01). Дослідження маркера імунної активації β 2 -МГ виявило стійку тенденцію до збільшення рівня цього білка у пацієнтів із ВІл-інфекцією при наявності неврологічних захворювань (р<0,05 порівняно з групою пацієнтів без неврологічних захворювань). За результатами кореляційного аналізу, в пацієнтів із 4 клінічною стадією ВІл-інфекції, незалежно від наявності неврологічних захворювань, виявлено достовірні зв'язки між рівнем β 2 -МГ та ВН і імунологічними та гематологічними показниками. Так, рівень цього білка пря-мо корелював з шОе (rs=0,42; р<0,001) та показником ВН рНК ВІл (rs=0,23; р<0,05). Зростання рівня β 2 -МГ було пов'язане зі зниженням абсолютної кількості Тл (rs=-0,40; р<0,001) і Тлх (rs=-0,39; р<0,001), загального числа лімфоцитів (rs=-0,43; р<0,001) і відносної кількості Тлх у сироватці крові (rs=-0,36; р<0,01). більш виразними були кореляції у пацієнтів із ВІл-асоційованими неврологічними захворюваннями.
