Від 40 до 70 % хворих на ВІЛ мають неврологічні захворювання. Токсоплазмоз – одна з найчастіших причин уражень головного мозку в пацієнтів із ВІЛ, що часто призводить до летальності.Мета дослідження – визначити та порівняти основні характеристики токсоплазмозу центральної нервової системи у ВІЛ-позитивних пацієнтів, які мали різний результат захворювання (вижили або померли внаслідок хвороби).Матеріали і методи. Проведено ретроспективний когортний аналіз 135 випадків церебрального токсоплазмозу у ВІЛ-позитивних пацієнтів Дніпропетровського регіону за період з 2010 до 2017 року. Наявність токсоплазмозу було підтверджено виявленням ДНК Toxoplasma gondii в спинномозковій рідині, а у померлих хворих дослідженням автопсійного матеріалу. Статистичну обробку результатів дослідження проводили з використанням програм STAT IST ICA v.6.1® і SPSS. Для порівняння показників у групах хворих, які померли та які вижили, визначали відношення шансів (odds ratio – OR) з довірчим інтервалом (95 % CI).Результати досліджень та їх обговорення. У 30 % ВІЛ-позитивних пацієнтів із захворюваннями ЦНС було діагностовано токсоплазмоз. Більшість хворих склали чоловіки – 71 (52,6 %), середній вік становив (37,79±0,64) року. Кількість летальних випадків – 29 (21,5 %). Групи пацієнтів, які вижили або померли, суттєво не відрізнялись за віком (37,47±0,72) проти (38,93±1,46) року (p>0,05) та гендерним складом (чоловіків було 53,8 % проти 48,3 % жінок). Інфікування статевим шляхом мало місце в 40,0 % випадків, через споживання ін’єкційних наркотиків (СІН) – 34,8 %, шлях інфікування був невизначений у 25,2 %. У померлих невизначений шлях інфікування склав 51,7 % проти 17,9 % (р<0,001) пацієнтів, які вижили. У 43 хворих (31,9 %) на церебральний токсоплазмоз ВІЛ-статус було визначено разом із розвитком токсоплазмозу. Частота пізнього виявлення ВІЛ (менше року до маніфестації токсоплазмозу ЦНС) у хворих, які померли, була в 6,49 раза вищою (95 % CI 2,64–15,95), порівняно з тими, які вижили. 76 (56,3 %) пацієнтів мали рівень CD4 менший 50 клітин/мм3, у тому числі серед померлих пацієнтів цей показник був у 1,5 раза вищим – 75,9 % проти 50,9 % (р<0,05) у тих, хто вижив. Медіана CD4 в період розвитку неврологічних захворювань у хворих із токсоплазмозом ЦНС склала 44,5 (IQR 21–100) клітин/мм3, з них у пацієнтів, які померли, – 33 (IQR 15–44) клітин/мм3 проти 52 (IQR 25–106) клітин/мм3 в осіб, які вижили (р<0,05). Медіана вірусного навантаження (Lg ВН) у період розвитку неврологічних симптомів склала 4,96 (IQR 1,60–5,80) копій/мл, а у пацієнтів, які померли, дорівнювала 6,20 (IQR 6,05–6,31) копій/мл і значно перевищувала аналогічний показник у тих, кого виписали з поліпшенням стану, – 4,89 (IQR 1,60–5,72) копій/мл (р<0,05). Серед клінічних симптомів найрозповсюдженішими у пацієнтів із токсоплазмозом ЦНС були: головний біль – у 86 (63,7 %) пацієнтів, запаморочення та порушення координації – у 81 (60,0 %), судоми – в 43 (31,9 %), моно- та геміпарези – у 57 (42,2 %), порушення ЧМН – у 37 (27,4 %), дизартрія – у 27 (20,0 %), когнітивні порушення – у 29 (21,5 %), порушення свідомості – у 14 (10,4 %), порушення зору – в 7 (5,2 %) випадках. У 30 (22,2 %) пацієнтів мало місце поєднання токсоплазмозу з іншими захворюваннями ЦНС. Розвиток ко-інфекції токсоплазмозу з туберкульозом у пацієнтів із летальним наслідком хвороби спостерігався у 4,5 раза частіше (95 % СI 1,42–14,29), ніж у пацієнтів, які вижили (р<0,05). Поширення вірусного інфікування Епштейна–Барр серед померлих було у 4,21 раза більшим (95 % СI 1,11–15,90) (р<0,05).Висновки. Церебральний токсоплазмоз складає 30,0 % усієї ВІЛ-асоційованої інфекційної патології ЦНС в пацієнтів у Дніпропетровському регіоні; у 56,3 % пацієнтів токсоплазмоз діагностують на тлі глибокої імуносупресії, у 21,5 % первиннодіагностований токсоплазмоз закінчується летально. Шанси летального наслідку хвороби не залежить від статі, віку, але значно зростають при відсутності АР Т, кількості CD4 клітин/мм3 менше 50 та наявності ко-інфекцій ЦНС, найвагомішим з яких у плані ризику є туберкульоз ЦНС. Отримані результати важливі для прогнозування перебігу хвороби та виявлення можливих ризиків, для удосконалення профілактичних заходів.
A retrospective analysis of medical records of 451 patients with HIV infection showed that the most common clinical manifestations of cerebral tuberculosis are as follows: headache, fever ≥ 38.0 °C, meningeal symptoms; in toxoplasmosis encephalitis these are headache, paresis of cranial nerves and extremities, incoordination, cognitive disorders, dysarthria, fever ≥ 38.0 °C and convulsions; in mycotic lesions of the CNS they are headache, coordination disorders, positive meningeal symptoms, disturbances of consciousness, cognitive disorders, visual disorders, cranial nerve deficiency, convulsions and dysarthria. The clinical picture of bacterial (exacted) meningitis was dominated by positive meningeal symptoms, headache, fever, impaired consciousness. At the same time, in such neurological manifestations of central nervous system lesions as cranial nerve paresis, mono− and hemiparesis, visual, coordination and speech disorders, which mostly accompany cerebral toxoplasmosis, cerebrovascular diseases, brain tumors, encephalitis of unspecified etiology. Thus, the analysis suggests that the clinical manifestations of various etiologies of lesions of the central nervous system are often nonspecific and probably are stipulated with common morphological changes in brain tissue, which may be associated with the neuropathological effects of HIV, as well as a large impact co−infection. Objective — to determine the association of basic demographic, clinical, and routine laboratory characteristics with the risk of adverse outcomes in patients with COVID-19. Materials and methods. Data of 320 patients hospitalized in the City Clinical Hospital No. 21 named after Prof. Ye. G. Popkova with a diagnosis of coronavirus disease19 (SARS-CoV2+ RNA)) were studied. Women — 156 (48.8 %), men — 164 (51.2 %), the mean age of the patients — (60.3 ± 13.6) years. Patients were divided into 2 groups: Group I — 270 (84.4 %) people who were discharged from the hospital with recovery or significant improvement in their condition, Group II — 50 (15.6 %) patients who died as a result of the disease (p = 0.010). The average period from the onset of the disease to hospitalization was 9 (7; 11) days. Statistical processing of the research results was carried out using the Statistica v. 6.1 software package. Results and discussion. Most of the deceased patients were older than 60 years — 36 (72.0 %) (p < 0.001). The maximum number of fatalities is in the age range of 60—69 years. Mortality among men was 1.5 times higher than among women — 30 (60.0 %) vs 20 (40.0 %) (p = 0.046). In the II group, the following were observed significantly more often: ischemic heart disease and hypertension — respectively 54.0 vs 20.7 % (p < 0.001) and 70.0 vs 50.0 % (p = 0.009); other heart diseases (OR — 7.23, rv = 0.202, p < 0.001), obesity (OR — 2.97, rv = 0.186, p < 0.001), diabetes (OR — 2.38, rv = 0.146; p = 0.009).The risk of mortality increased in the presence of cough (OR — 11.87, p = 0.012), fever (OR — 3.44, p < 0.001); lymphopenia (OR — 3.82, p < 0.001); hyperglycemia (OR — 4.69, p < 0.001); increased C-reactive protein (CRP) (OR — 5.96, p < 0.001), especially above 100 mg/l (OR from 39.86 to 52.35 at p < 0.01 and p < 0.001). However, thrombocytosis and lymphomonocytosis were more often observed during the benign course of the disease (respectively: p < 0.001, p = 0.051). A direct relationship between the probability of mortality and age (rs = 0.143, p = 0.011), respiratory rate (rs = 0.332, p < 0.001) and heart rate (rs = 0.159, p = 0.004) was revealed; fever (rs = 0.152, p = 0.006), quantitative indicators: segmented neutrophils (rs = 0.275, p < 0.001), urea (rs = 0.309, p < 0.001), urea nitrogen (rs = 0.300, p < 0.001), residual nitrogen (rs = 0.288, p < 0.001), creatinine (rs = 0.111, p = 0.047), glucose (rs = 0.273, p < 0.001), CRP (rs = 0.348, p < 0.001), prothrombin time (rs = 0.149, p = 0.008), international normalized ratio (rs = 0.193, p = 0.001). A inverse relationship was determined with blood oxygen saturation (rs = – 0.431, p < 0.001); levels of lymphocytes, bands, platelets and prothrombin index (respectively: rs = – 0.278, p < 0.001; rs = – 0.118, p = 0.034; rs = – 0.223, p < 0.001; rs = – 0.170, p = 0.002). Conclusions. The following are associated with in-hospital mortality in patients with COVID-19: older age, coronary heart disease, stage III hypertension, diabetes, stage III obesity, clinical and laboratory signs: fever, shortness of breath, cough, lymphopenia, CRP content in the blood more than 100 mg/l, hyperglycemia, increased markers of kidney damage and hypercoagulation. The obtained results may be useful for predicting the course of the coronavirus disease.
