Від 40 до 70 % хворих на ВІЛ мають неврологічні захворювання. Токсоплазмоз – одна з найчастіших причин уражень головного мозку в пацієнтів із ВІЛ, що часто призводить до летальності.Мета дослідження – визначити та порівняти основні характеристики токсоплазмозу центральної нервової системи у ВІЛ-позитивних пацієнтів, які мали різний результат захворювання (вижили або померли внаслідок хвороби).Матеріали і методи. Проведено ретроспективний когортний аналіз 135 випадків церебрального токсоплазмозу у ВІЛ-позитивних пацієнтів Дніпропетровського регіону за період з 2010 до 2017 року. Наявність токсоплазмозу було підтверджено виявленням ДНК Toxoplasma gondii в спинномозковій рідині, а у померлих хворих дослідженням автопсійного матеріалу. Статистичну обробку результатів дослідження проводили з використанням програм STAT IST ICA v.6.1® і SPSS. Для порівняння показників у групах хворих, які померли та які вижили, визначали відношення шансів (odds ratio – OR) з довірчим інтервалом (95 % CI).Результати досліджень та їх обговорення. У 30 % ВІЛ-позитивних пацієнтів із захворюваннями ЦНС було діагностовано токсоплазмоз. Більшість хворих склали чоловіки – 71 (52,6 %), середній вік становив (37,79±0,64) року. Кількість летальних випадків – 29 (21,5 %). Групи пацієнтів, які вижили або померли, суттєво не відрізнялись за віком (37,47±0,72) проти (38,93±1,46) року (p>0,05) та гендерним складом (чоловіків було 53,8 % проти 48,3 % жінок). Інфікування статевим шляхом мало місце в 40,0 % випадків, через споживання ін’єкційних наркотиків (СІН) – 34,8 %, шлях інфікування був невизначений у 25,2 %. У померлих невизначений шлях інфікування склав 51,7 % проти 17,9 % (р<0,001) пацієнтів, які вижили. У 43 хворих (31,9 %) на церебральний токсоплазмоз ВІЛ-статус було визначено разом із розвитком токсоплазмозу. Частота пізнього виявлення ВІЛ (менше року до маніфестації токсоплазмозу ЦНС) у хворих, які померли, була в 6,49 раза вищою (95 % CI 2,64–15,95), порівняно з тими, які вижили. 76 (56,3 %) пацієнтів мали рівень CD4 менший 50 клітин/мм3, у тому числі серед померлих пацієнтів цей показник був у 1,5 раза вищим – 75,9 % проти 50,9 % (р<0,05) у тих, хто вижив. Медіана CD4 в період розвитку неврологічних захворювань у хворих із токсоплазмозом ЦНС склала 44,5 (IQR 21–100) клітин/мм3, з них у пацієнтів, які померли, – 33 (IQR 15–44) клітин/мм3 проти 52 (IQR 25–106) клітин/мм3 в осіб, які вижили (р<0,05). Медіана вірусного навантаження (Lg ВН) у період розвитку неврологічних симптомів склала 4,96 (IQR 1,60–5,80) копій/мл, а у пацієнтів, які померли, дорівнювала 6,20 (IQR 6,05–6,31) копій/мл і значно перевищувала аналогічний показник у тих, кого виписали з поліпшенням стану, – 4,89 (IQR 1,60–5,72) копій/мл (р<0,05). Серед клінічних симптомів найрозповсюдженішими у пацієнтів із токсоплазмозом ЦНС були: головний біль – у 86 (63,7 %) пацієнтів, запаморочення та порушення координації – у 81 (60,0 %), судоми – в 43 (31,9 %), моно- та геміпарези – у 57 (42,2 %), порушення ЧМН – у 37 (27,4 %), дизартрія – у 27 (20,0 %), когнітивні порушення – у 29 (21,5 %), порушення свідомості – у 14 (10,4 %), порушення зору – в 7 (5,2 %) випадках. У 30 (22,2 %) пацієнтів мало місце поєднання токсоплазмозу з іншими захворюваннями ЦНС. Розвиток ко-інфекції токсоплазмозу з туберкульозом у пацієнтів із летальним наслідком хвороби спостерігався у 4,5 раза частіше (95 % СI 1,42–14,29), ніж у пацієнтів, які вижили (р<0,05). Поширення вірусного інфікування Епштейна–Барр серед померлих було у 4,21 раза більшим (95 % СI 1,11–15,90) (р<0,05).Висновки. Церебральний токсоплазмоз складає 30,0 % усієї ВІЛ-асоційованої інфекційної патології ЦНС в пацієнтів у Дніпропетровському регіоні; у 56,3 % пацієнтів токсоплазмоз діагностують на тлі глибокої імуносупресії, у 21,5 % первиннодіагностований токсоплазмоз закінчується летально. Шанси летального наслідку хвороби не залежить від статі, віку, але значно зростають при відсутності АР Т, кількості CD4 клітин/мм3 менше 50 та наявності ко-інфекцій ЦНС, найвагомішим з яких у плані ризику є туберкульоз ЦНС. Отримані результати важливі для прогнозування перебігу хвороби та виявлення можливих ризиків, для удосконалення профілактичних заходів.

Проведений аналіз на основі спостереження та моніторингу 450 хворих на ВІЛ-інфекцію у Дніпропетровському обласному центрі СНІДу, яким було призначено антиретровірусну терапію (АРТ) у 2010 році, на протязі 5 років. Виявлені додаткові фактори та недоліки які можуть впливати на результати АРТ.Матеріали і методи. Під нашим спостереженням в Дніпропетровському обласному центрі СНІДу перебувало 450 хворих, яким була призначена АРТ в 2010р. Спостереження проводилося протягом 5 років.Результати та обговорення. Проведений аналіз на основі спостереження і моніторингу 450 хворих на ВІЛ-інфекцію в Дніпропетровському обласному центрі СНІДу, яким було призначено антиретровірусну терапію (АРТ) в 2010 році, протягом 5 років. Виявлено додаткові чинники і недоліки, які можуть впливати на результати АРТ.Висновки. Більшість хворих початок терапію в терміни до 5 років встановлення діагнозу ВІЛ-інфекція, з яких 51% в стадії СНІД, що свідчить про певні недоліки щодо виявлення хворих на ВІЛ-інфекцію на перших стадіях захворювання, що може призводить до прогресування хвороби і смертності від проявів опортуністичних хвороб та СНІДу.
Токсоплазмоз ЦНС є однією з найрозповсюдженіших причин ураження мозку у пацієнтів зі СНІДом. Поліморфізм локусів HLA DRB1 пов’язаний із захистом проти ВІЛ-1 та може впливати на розвиток опортуністичних захворювань. Мета роботи – вивчити варіації алелей HLA DRB1 при церебральному токсоплазмозі та порівняти їх з даними пацієнтів при інших опортуністичних захворюваннях IV клінічної стадії ВІЛ-інфекції для визначення протекторних можливостей.Пацієнти і методи. Обстежено 70 пацієнтів з ВІЛ-інфекцією та опортуністичними захворюваннями, індикаторними для IV клінічної стадії, віком від 24 до 61 років, серед яких чоловіків було 41 (58,6 %), жінок – 29 (41,4 %). 66 (94,3 %) пацієнтів були виписані з лікарні з покращенням стану, 4 (5,7 %) хворих померли внаслідок захворювання. Пацієнти були розподілені на дві групи: основну групу склали 23 хворих (32,9 %) з токсоплазмозом головного мозку, підтвердженим детекцією ДНК Toxoplasma gondii в лікворі, серологічними дослідженнями та характерними ознаками на серії томограм, а у померлих пацієнтів дослідженням автопсійного матеріалу тканини мозку. Групу порівняння склали 47 хворих (67,1 %), які мали інші опортуністичні захворювання.Типування DRВ1 алелів проводилось методом ПЛР, варіантом сіквенс-специфічних праймерів (PCR-SSP). Дослідження здійснювалося наборами «HLA-ДНК-ТЕХ» для типування генів DRВ1 виробництва «ДНК-Технологія» (РФ).Статистична обробка результатів дослідження проводилась з використанням пакету прикладних програм STATISTICA v.6.1®. Результати. Аналіз варіацій алелей локусів DRB1 системи HLA II класу у 70 хворих з IV клінічною стадією ВІЛ-інфекції довів, що в більшості випадків визначались алелі локусів DRB1*01 (21,4 %), DRB1*04 (18,6 %), DRB1*07 (25,7 %), DRB1*11 (35,7 %), DRB1*13 (18,6 %), DRB1*15 (17,1 %), DRB1*16 (21,4 %). Алелі DRB1*02, DRB1*05, DRB1*06, DRB1*09, DRB1*18 не виявлялись в жодному випадку. Встановлено тенденції до збільшення серед хворих з токсоплазмозом носіїв алелей DRB1*01 (30,4 %), DRB1*15 (26,1 %), DRB1*16 (30,4 %), порівняно з іншими опортуністичними захворюваннями – 17,0 %, 12,8 % і 17,0 % відповідно, що при більшій кількості спостережень в означених групах може досягти критичного рівня статистичної значимості (p<0,05) для прогнозування церебрального токсоплазмозу. Водночас ризик розвитку токсоплазмозу серед ВІЛ-позитивних пацієнтів з алелями DRB1*13 був вірогідно нижчим, ніж при інших захворюваннях (rs=-0,26; p<0,05). Так, алель гена DRB1*13 виявлявся в 1 (4,3 %) хворого з токсоплазмозною інфекцією і у 12 (25,5 %) хворих з опортуністичними захворюваннями іншої етіології (р=0,048 FET), що свідчить про можливу захисну дію означених алелей. Проведений аналіз варіацій алелей DRB1 системи HLA II класу при токсоплазмозі окремо залежно від статі показав наявність вірогідних асоціацій з токсоплазмозом у жінок, носіїв алелей гена DRB1*16 (46,2 %; p<0,01), в той час як у чоловіків такий варіант частіше асоціювався з опортуністичними захворюваннями іншої етіології (25,8 проти 10,0 %; p>0,05). Протилежні тенденції встановлені відносно алелей DRB1*04: у жінок вони були виявлені лише у пацієнток без токсоплазмозної інфекції (31,3 %; p<0,05), у чоловіків, навпаки, носіїв алелей DRB1*04 було більше серед хворих з токсоплазмозом (30,0 проти 16,1 %; p>0,05). Висновки. Експресія варіанта HLA-DRB1 має певний зв’язок зі статтю, впливає на прогресування ВІЛ та розвиток токсоплазмозу. Тенденція до збільшення носіїв алелей DRB1*01 (30,4 %), DRB1*15 (26,1 %), DRB1*16 (30,4 %) серед пацієнтів з токсоплазмозом свідчить про підвищену чутливість до токсоплазмозу та необхідність посилення профілактичних заходів у групі. Нижчий ризик розвитку токсоплазмозу серед ВІЛ-позитивних пацієнтів з алелями DRB1*13, ніж інших захворювань (rs=-0,26; p<0,05), свідчить про протекторну дію даного генетичного варіанту відносно токсоплазмозу.
