L'influence du carbacetam sur les réactions comportementales dans la maladie d'Alzheimer expérimentale Introduction. Compte tenu de l'efficacité insuffisante des traitements modernes, la recherche de nouveaux moyens de la thérapie pathogénique de la maladie d'Alzheimer est pertinente. Le but de l'étude est d'évaluer l'efficacité du nouveau modulateur original du système GABA-ERGIQUE du carbacetam en ce qui concerne la dynamique des modifications des indicateurs des réactions comportementales des rats atteints du modèle de la maladie d'Alzheimer. Matériaux et méthodes. Le modèle de la maladie d'Alzheimer a été créé par l'administration intra-abdominale pendant 27 jours de chlorhydrate de scopolamine (Sigma, USA) à une dose de 1 mg/kg. à partir du 28e jour de l'expérience ; le carbacetam a été administré par voie intra-péritonéale à une dose de 5 mg/ kg dans 1 ml de solution saline physiologique, une fois par jour pendant 14 jours. Актуальність. Одним з органів-мішеней при цукровому діабеті (ЦД) є центральна нервова система, гіперглікемічні пошкодження якої проявляються прогресуючими когнітивними порушеннями та зниженням якості життя хворих. Значущою патогенетичною ланкою ЦД і його ускладнень є активація процесу вільнорадикального окиснення біомолекул, що у ЦНС залежить від стану центральних ГАМКергічних регуляторних механізмів. Мета дослідження: вивчення впливу нового модулятора активності ГАМК карбацетаму на стан прооксидантно-антиоксидантної системи та системи оксиду азоту (NO) кори головного мозку (ГМ) і гіпокампа щурів при експериментальному ЦД 2-го типу. Матеріали та методи. Експерименти проводили на нелінійних лабораторних білих щурах-самцях масою 0,18–0,20 кг. ЦД 2-го типу моделювали шляхом введення стрептозотоцину (30 мг/кг) в поєднанні зі збагаченою жирами дієтою. Карбацетам вводили внутрішньоочеревинно в дозі 5 мг/кг протягом 14 днів. Інтенсивність перекисного окиснення ліпідів оцінювали за вмістом продуктів, що реагують із 2-тіобарбітуровою кислотою (ТБКАП). Антиоксидантний захист оцінювали за активністю супероксиддисмутази (СОД) та каталази. Стан системи NO визначали за вмістом стабільних метаболітів монооксиду азоту — нітрит-аніонів (NO2–) та активністю NO-синтази (NOS). Для статистичної обробки даних застосовували програму Statistica 10 (StatSoft, Inc., США). Результати. У корі ГМ і гіпокампі щурів із ЦД збільшувався вміст ТБКАП, зменшувалась активність СОД та каталази. Водночас в обох структурах підвищувались вміст NO2– та активність NOS. Після введення карбацетаму порівняно з контролем у щурів з діабетом вміст ТБКАП знижувався як у корі ГМ, так і в гіпокампі, зростала активність СОД. Однак активність каталази збільшувалася у корі ГМ, а у гіпокампі спостерігалась лише тенденція до зростання даного показника. Вміст NO2– зменшувався в обох досліджуваних структурах. Активність NOS знижувалась після введення карбацетаму лише в гіпокампі. Висновки. Наявність корегувального впливу на прооксидантно-антиоксидантний баланс та систему оксиду азоту в корі головного мозку та гіпокампі щурів вказує на нейропротекторні властивості карбацетаму при експериментальному ЦД 2-го типу.
Purpose of the study. Accentuate attention to the need to inform students about the achievements of evidence-based medicine in the field of rational pharmacotherapy when considering issues related to the choice of treatment and prevention of problems associated with taking medicines. Methodology. Descriptive and comparative methods were used in the study. Scientific novelty. The advantages of teaching the basics of pharmacotherapy of pathological processes using evidence-based medicine data for the formation of skills to critically evaluate information of scientific practical and make the right choices for the benefit of the patient. Conclusions. This approach increases the level of competence of medical students in matters of rational pharmacotherapy. Prospects for further research. Analysis of the effectiveness of teaching pharmacotherapy taking into account the results of student questionnaires.
A progressive increase of complications of type 2 DM makes the problem very important for clinical andfundamental medicine. In particular, neurodegenerative processes in case of type 2 DM aggravate the courseof the disease, change a usual life rhythm, and are a valuable cause of high rates of disability and lethality.Deterioration of cognitive functions is one of the main complications of diabetes. In certain literary sourcesinhibitors of angiotensin-transforming enzyme are considered as the means to prevent and treat complicationsof diabetes mellitus. Their role as modulators of gama-aminobutyric acid with development of central diabeticneurodegeneration is not completely determined yet.Therefore, the objective of the study is to assess efficacy of pharmacological correction of cognitive disordersby carbacetam, a GABA modulator, and enalapril, a RAS blocker, in case of experimental neurodegenerationcaused by type 2 DM.Changes of the cognitive functions under carbacetam and enalapril effect in nonlinear laboratory albinomale rats with neural degeneration under conditions of type 2 diabetes mellitus simulated by streptozotocin andhigh fat diet are studied. The cognitive ability was assessed by behavioral reactions of rats in the tests «openfield» and «the conditional reflex reaction of passive avoidance» by Butler formula.Behavior of rats with type 2 diabetes mellitus after administration of carbacetam and enalapril showed ofcognitive functions in the groups. At the same time, carbacetam was found to possess better correcting effect onadaptation, cognitive reactions and anti-amnestic activity, while enalapril improved emotional condition of therats mostly. Analysis of anti-amnestic properties found higher values of the parameters on the 14th day than onthe 1st day of correction, which is indicative of the efficacy of a course administration of carbacetam and enalapril under conditions of neural degeneration stipulated by type 2 DM.
Neurodegenerative disorders in the cerebral cortex and hippocampus are one of the most common causes of disability and mortality in patients with diabetes. Excessive glucose concentration causes a toxic effect due to an increased amount of glycolysis products, lipid and protein peroxide oxidation, decreased membranous potential of the mitochondria and deficiency of neuron energy supply due to mitochondrial dysfunction. Gammaamino butyric acid is known to localize to the mitochondria, and its functional cycle is closely linked to glucose metabolism. Objective of the study: experimental investigation of сarbacetam effect with cerebral mitochondrial dysfunction of rats with type 2 diabetes mellitus. The experiments were conducted on laboratory nonlinear albino male rats with the body weight 0.18–0.20 kg. Type 2 diabetes is modeled on streptozotocin and a high-fat diet. Carbacetam was administered intraperitoneally at a dose of 5 mg/kg, once daily for 14 days. Under conditions of central nervous system damage induced by type 2 diabetes mellitus, lipid and protein peroxide oxidation increases in the mitochondrial fraction of the cerebral cortex and hippocampus of rats; activity of superoxide dismutase, catalase, α-ketoglutarate dehydrogenase, succinate dehydrogenase decreases; a relative rate of mitochondrial swelling increases. After carbacetam administration during 14 days the content of products reacting with 2-thiobarbituric acid and protein oxidation modification decrease in the mitochondria of the brain and hippocampus of rats with type 2 diabetes mellitus; activity of catalase in the cerebral cortex and α-ketoglutarate dehydrogenase in the hippocampus increases, activity of succinate dehydrogenase increases in both structures examined which is indicative of its antioxidant properties. Decrease of a relative rate of mitochondrial swelling in the cerebral cortex and hippocampus of rats confirms a protective effect of carbacetam under conditions of mitochondrial dysfunction.
