Carcinosarcoma is a rare type of malignant tumour that possess both the elements of carcinoma and sarcoma. They may occur in various locations such as the uterus, breast, thyroid, lung and gastrointestinal system. However, very few primary mediastinal carcinosarcomas have been reported. We are presenting a case of a 75-year-old male who presented with progressive dysphagia and hoarseness for 3 weeks. Contrast-enhanced computed tomography of the neck showed features of enlarged mediastinal necrotic nodes and the biopsy of this mass showed morphology and immunohistochemical profile that are consistent with carcinosarcoma. However, the patient succumbed to his illness soon after diagnosis.
Vallecular cysts are ductal cysts that form when a mucous gland or a lingual tonsillar crypt becomes dilated due to the obstruction from inflammation, irritation or trauma. The peak incidences of vallecular cysts are bimodal; they are more frequent in children compared to the adult group. We report a case of a 22-year-old lady who presented with globus sensation for more than a year. Flexible nasopharyngolaryngoscopy revealed a huge cystic mass at the vallecula obscuring the visualisation of the laryngeal inlet. Patient successfully underwent endoscopic excision of the vallecular cyst with no intra-and postoperative complications.
Submassteric abscess is a rare head and neck abscess. It may mimic a parotid abscess due to very close anatomical relationship. We present a 14-month-old child presented with fever, poor oral intake, irritability, and left-sided cheek swelling. Contrast-enhanced computed tomography of the neck showed an abscess formation in left submasseteric space. Intraoral drainage of abscess was successfully performed under general anesthesia. Patient recovered well and was discharged on day three post operatively. Lipoma is a benign mesenchymal tumour of adipose tissue and relatively rare occurrence in head and neck area. Mostly lipoma have a diameter of 2 cm and rarely grow beyond 10 cm. However, it may occasionally enlarge rapidly and become huge to become a giant lipoma which measures at least more than 10 cm in one dimension or weighing at least 1000g. The presentation of the anterior neck swelling is mostly related to the thyroid gland. However, it also can be the other soft tissue swellings, such as lipoma, which is the commonest mesenchymal tumours in adults. The lipomas however, are commonly involving the head and posterior neck region, which estimate about 15-20% of the cases and mostly small. We present the case of a 80-yearold man with huge anterior neck swelling for thirty years. The complete excision was performed with a good post-operative healing and cosmesis.

Schwannomas are benign tumors arising from Schwann cells of myelinated nerve sheath. Schwannomas of the nasal cavity and paranasal sinuses are rare, with only 4% occurrence. We report a case of a 48-year-old lady who presented with 1-year history of progressively worsening left nasal blockage. Rigid nasoendoscopy showed a smooth, globular mass occupying the left nasal cavity. The mass arose from the left nasal septum and impinged on the anterior part of the middle turbinate posteriorly and inferior turbinate anteriorly. Computed tomography of the paranasal sinuses showed a 3.8 × 1.8 x 3.7- cm enhancing soft tissue density in the left nostril. The mass obliterated the left nasal cavity and caused deviation of the septum to the right. The patient underwent an endoscopic excision and histopathologic examination confirmed the diagnosis of Schwannoma. Postoperative recovery was uneventful and no recurrence was seen in the follow-up period.
