Entre noviembre de 1999 y abril de 2000 se presentó una epidemia de malaria en el departamento de La Guajira que alcanzó 5.687 casos, 3.401 causados por Plasmodium faiciparum, 2.256 por Plasmodium vivax y 30 casos diagnosticados como infección mixta. Dadas las caracteristicas propias del departamento de La Guajira y de la transmisión de malaria allí. se consideró que existian las condiciones favorables para usar una prueba rapida de diagnostico de campo. En este estudio se evaluó y comparó el diagnóstico por inmunocromatografía rápida con la gota gruesa como estándar de oro en los municipios de Dibulla, Manaure y Riohacha. La conducta con el paciente se basó en el resultado de la pruzba rápida. Sin embargo. los resultados obtenidos en la gota gruesa se tuvieron en cuenta para modificar la conducta inicial con el paciente, en caso de que los resultados de las pruebas fueran discordantes. Se procesaron 231 muestras sanguíneas obtenidas por puncion capilar. La sensibilidad y la especificidad generales del método inmunocromatográfico fueron de 98.7% y 99.3%. respectivamente. en tanto que para P falciparum fueron de 98.1% y 76,9%, y para P vivax de 90,9% y 100%. El índice de concordancia fue 0.98; el valor predictivo positivo, 98,7%, y el valor predictivo negativo, 99,3%. Los resultados obtenidos sugieren que la prueba inmunocromatográfica (OptiMale) es una alternativa adecuada para realizar el diagnóstico de malaria en lugares donde no se realiza el diagnóstico microscópico.Palabras clave: malaria. prueba de diagnóstico rápido, OptiMALm Evaluation of a rapid diagnostic field test for malaria Between November 1999 and April 2000, a malariaoutbreak occurred in the state of La Guajira, Colombia; 5.687 cases were diagnosed -3,401 as P falciparum, 2,256 as P vivax and 30 as mixed infections. The treatment of each patient was based on the rapid test result, but whenever the two methods disagreed, the thick bood smear results were given priority in decisions to modify the initial treatment. Two hundred thitiy-one finger-prick blood samples were processed. The general sensitivity and specificity of OptiMALm were 98.7% and 99.3%, respectively. The sensitivity and specificity for P falciparum were 98.1% and 76.9%. The sensitivity and specificity for F1 vivax were 90.9% and 100%. The concordance index was 0.98, the positive predictive value. 98.7% and the negative predictive value, 99.3%. These results suggest that the rapid imrnunocromatographic test (OptiMAL") is a reliable alternative for malaria diagnosis under circumstances where rnicroscopical diagnosis is not possible.
Introducción. El Instituto Nacional de Salud con el fin de mejorar el acceso al diagnóstico de paludismo, evaluó dos pruebas rápidas para el diagnóstico de dicha enfermedad. Objetivo. Evaluar la sensibilidad, especificidad y concordancia de dos pruebas inmunocromatográficas, NOW® ICT Malaria Pf/Pv y OptiMAL®, frente a la gota gruesa. Materiales y métodos. Se llevó a cabo un estudio descriptivo de concordancia en 214 pacientes de Tumaco, captados mediante búsqueda pasiva y brigadas de atención, que presentaran, por lo menos, uno de los síntomas de la tríada clásica. Resultados. NOW® ICT tuvo una sensibilidad general de 98,4% (intervalo de confianza de 95% (IC95%): 90,3-99,9), especificidad general de 98,0% (IC95%: 93,9-99,5). Para Plasmodium falciparum la sensibilidad fue de 98,2% (IC95%: 89,4-99,9) especificidad de 98,1% (IC95%: 94,1-99,5). Esta sensibilidad disminuyó a 80% en el rango de 200-4.000 parásitos/µl. Los valores de sensibilidad y especificidad de NOW® ICT para Plasmodium vivax fueron del 100% y no se afectó la sensibilidad en los rangos de parasitemias establecidos. OptiMAL® tuvo una sensibilidad general de 95,2% (IC95%: 85,8-98,8) y especificidad general de 99,3% (IC95%: 95,8-100,0). Para P. falciparum OptiMAL® tuvo una sensibilidad de 94,7% (IC95%: 84,5-98,6) y especificidad de 99,4% (IC95%: 96,0-100,0). La sensibilidad disminuyó a 60% en el rango de de 200-4.000 parásitos/µl. La sensibilidad de OptiMAL® para P. vivax fue 66,7% (IC95%: 24,1-94,0), pero disminuyó a 50% en el rango de 300-2.500 parásitos/µl. Conclusiones. To increase the accessibility of malaria diagnosis, the Instituto Nacional de Salud de Colombia undertook a field trial to evaluate the sensitivity and specificity of two rapid diagnostic tests. Objective. The sensitivity, specificity and concordance was compared for two rapid diagnostic tests for malaria, NOW® ICT Malaria Pf/Pv and OptiMAL®, Materials and methods. Introducción. La evaluación del desempeño es una de las actividades más importantes del aseguramiento de la calidad del diagnóstico de la malaria o paludismo. En Colombia, esta actividad ha estado a cargo de los programas de la evaluación externa del desempeño y la evaluación externa e indirecta del desempeño. Objetivos. Evaluar el desempeño de los microscopistas de malaria de los laboratorios públicos de referencia, utilizando paneles de láminas, y describir la metodología empleada para este propósito. Materiales y métodos. Se hizo un estudio retrospectivo para evaluar las concordancias entre los microscopistas sénior en la detección parasitaria, la identificación de especie y el recuento parasitario, con base en los resultados de la evaluación de competencias y utilizando dos paneles: uno de 40 láminas y otro de 17 láminas. Resultados. La concordancia en la determinación de parásitos en las muestras, fue de 96,9 % (IC 95% 96,0-97,5) y, para la identificación de especies, fue de 88,7 % (IC 95% 86,6-90,5). El porcentaje promedio de láminas concordantes en el grupo evaluado, fue de 89,7 % (IC 95% 87,5-91,6). Conclusiones. En la evaluación del desempeño con paneles de láminas se encontró que la mayoría de los microscopistas sénior de Colombia se clasificaban en las dos categorías superiores. La dificultad más común fue la identificación de la especie parasitaria. In Colombia, performance assessment of malaria microscopists has been done through the external performance assessment and indirect external performance assessment programs. Objectives: To assess the performance of malaria microscopists of public reference laboratories using slide sets, and to describe the methodology used for this purpose. Materials and methods: This was a retrospective study to evaluate the concordance of senior microscopists regarding parasite detection, species identification and parasite count based on the results of the assessment of competences using two sets, one comprising 40 slides, and another one with 17 slides. Results: The concordance for parasite detection was 96.9% (95% CI: 96.0-97.5) and 88.7% (95% CI: 86.6-90.5) for species identification. The average percentage of concordant slides in the group evaluated was 89.7% (95% CI: 87.5-91.6). Introducción. La implementación y desarrollo de actividades del sistema de gestión de la calidad del diagnóstico de malaria, permiten el adecuado funcionamiento de la red nacional de diagnóstico, necesario para fortalecer acciones de prevención y control de este evento, importante en salud pública. Objetivo. Caracterizar la Red de Diagnóstico de Malaria en Colombia, entre 2006 y 2010. Materiales y métodos. Se hizo un estudio retrospectivo mediante la revisión de los informes anuales de actividades de la Red de Diagnóstico de Malaria, enviados por los laboratorios de salud pública entre 2006 y 2010. Se analizó la cobertura de diagnóstico en la población en riesgo, las actividades de evaluación del desempeño y las capacitaciones al personal que hace el diagnóstico. Resultados. La cobertura del diagnóstico de malaria se ha incrementado en el país, pasando de 53 % de los municipios, en 2006, a 80 %, en 2010. El número de sitios que hacen el diagnóstico aumentó en 31 %, con un incremento en el número de microscopistas (56 %) y de laboratorios (30 %), para un total de 1.195 y 1.780, respectivamente, registrados en 2010. En el periodo de estudio, se mantuvo el porcentaje de laboratorios de salud pública (67 %) que llevan a cabo, al menos, tres de las actividades del sistema de gestión de la calidad a la Red de Diagnóstico de Malaria a nivel local. Conclusiones. Es necesario continuar con el fortalecimiento de la Red de Diagnóstico de Malaria, para brindar diagnóstico oportuno y con calidad con el fin de reducir la morbimortalidad por esta causa. A retrospective study was made by reviewing the annual reports of malaria diagnosis network activities that were sent by the Public Health Laboratories (PHL) between 2006 and 2010. The study included analysis of diagnostic coverage in population at risk and an evaluation of activities and training to the people responsible for malaria diagnosis. Results. Malaria diagnostic coverage has increased in Colombia, from 53% of municipalities covered in 2006 to 80% in 2010. The number of places that perform diagnosis increased by 31% with a significant increase, for the same period, of the number of microscopists (56%) and laboratories (30%), to 1,195 and 1,780 respectively, registered in 2010.
