Bệnh lý nặng cùng lúc ở toàn bộ động mach chủ ngực, từ đoạn lên đến đoạn xuống là một trong những bệnh lý động mạch chủ phức tạp nhất. Phẫu thuật kinh điển thay toàn bộ động mạch chủ sẽ rất phức tạp và nặng nề. Kỹ thuật vòi voi là một giải pháp tương đối triệt để và an toàn hơn phẫu thuật kinh điển. Kỹ thuật vòi voi cải tiến bằng phương pháp can thiệp nội mạch phổi hợp, nhờ tính hiệu quả và an toàn cao, nên đã phát triển mạnh trên thế giới từ hơn 10 năm nay, tuy nhiên có chi phí lớn nên chưa phổ biến ở Việt Nam. Bệnh viện Hữu nghị Việt Đức đã sáng tạo một kỹ thuật vòi voi cải tiến dựa trên các vật tư sẵn có, chi phí thấp và khả thi hơn cho người bệnh Việt Nam. Báo cáo nhằm thông báo ca lâm sàng bệnh động mạch chủ ngực phức tạp được điều trị thành công bằng kỹ thuật vòi voi cải tiến vào tháng 12 năm 2019.
Đặt vấn đề: Bắc cầu hoặc chuyển vị các động mạch xuất phát từ quai động mạch chủ (cảnh, dưới đòn) là kỹ thuật thường qui trong phẫu thuật Hybrid cho các tổn thương phức tạp của động mạch chủ ngực. Động mạch đốt sống là nhánh bên đầu tiên của động mạch dưới đòn trái, tuy nhỏ nhưng được khuyến cáo bảo tồn trong các phẫu thuật Hybrid. Trường hợp có bất thường giải phẫu – như động mạch đốt sống trái xuất phát thẳng từ quai động mạch chủ …, thì nên điều trị theo cách nào. Mục tiêu của báo cáo là đề xuất chỉ định và kỹ thuật tái lập tuần hoàn động mạch đốt sống khi có bất thường giải phẫu (hoặc hẹp do xơ vữa) trong phẫu thuật Hybrid điều trị bệnh lý động mạch chủ ngực. Phương pháp nghiên cứu: Nghiên cứu mô tả ba trường hợp lâm sàng bệnh lý – chấn thương vùng quai động mạch chủ có bất thường vị trí xuất phát động mạch đốt sống trái được điều trị bằng can thiệp nội mạch và tái lưu thông động mạch đốt sống vào động mạch cảnh chúng trái. Kết quả: Không quan sát thấy biến chứng liên quan đến phẫu thuật và tái lập tuần hoàn động mạch đốt sống rất tốt trên phim chụp kiểm tra sau mổ. Kết luận: Mặc dù chỉ định còn gây tranh cãi, tái thông động mạch đốt sống trái bất thường vào động mạch cảnh trái là kỹ thuật an toàn, đem lại lợi ích cho bệnh nhân, có thể thực hiện thường quy tại bệnh viện Hữu nghị Việt Đức.
Background: The hybrid procedure proved to be very suitable for Vietnamese conditions in the treatment of complex aortic diseases in the arch and on supra-renal lesion. There is a difficult situation where the ascending aorta is more dilated than can be accommodated by stent-grafts. Wrapping techique is a solution to this situation. Methods: Report on a clinical case of successful application of aortic wrap technique to treat complex aortic cross injuries at Cardiovascular and Thoracic Center, Viet Duc University Hospital. Results: A 73-year-old female patient was admitted to the hospital because of sudden chest pain. History of high blood pressure, heart failure, dyslipidemia and hypothyroidism. MSCT revealed type B aortic dissection, left hemothorax complication, the entire ascending aorta and a part of cross dilated to 50mm (maximum stent-graft size is 46mm). Poor general condition, severe and complicated acute aortic injury, if the classic surgery requires total thoracic aorta replacement – the risk of death is very high, so the hybrid solution is a reasonable choice if having the solution is to solve the problem to create a landing zone for stent-graft. Hybrid surgery was started with open sternotomy, bilateral aortic - carotid bypass with Dacron prothesis 9mm, using a large artificial vessel to wrap around the ascending aorta to reduce the medium size to 38mm. Drain and close the sternum. Next is endovascular intervention – installing a stent-grafts of 42x42x200 and 42x38x150 then 38x34x150 to the diaphragm, plug for left subclavian artery. Left pleural drainage. The patient was discharged 10 days after surgery. Follow-up after 2 years for good results. Conclusion: The hybrid procedure can still be applied in the situation of ascending aorta dilatation with wrapping technique – decreasing the size of the aorta to fit the size of the stent-graft in some high-risk situations. with classic surgery.

Objectives: We report two clinical cases that wherein stent graft-induced new entry (dSINE) after frozen elephant trunk (FET) procedure for type A aortic dissection were resolved successfully by additional stent graft placement. Methods: Describe rare clinical cases. Results: Two patients were hospitalized after FET procedure for type A aortic dissection treatment. The former had refractory abdominal pain and the latter had sudden chest pain; both their abdominal thoracic CT scanner showed dSINE. We decided to place additional stent grafts to cover the new entry. Both patients recovered well and were discharged after 5 and 12 days, respectively, without any significant complications. Conclusions: Supplemental stent graft placement is an effective and safe treatment choice for dSINE post-FET procedure.
