Đặt vấn đề: Bắc cầu hoặc chuyển vị các động mạch xuất phát từ quai động mạch chủ (cảnh, dưới đòn) là kỹ thuật thường qui trong phẫu thuật Hybrid cho các tổn thương phức tạp của động mạch chủ ngực. Động mạch đốt sống là nhánh bên đầu tiên của động mạch dưới đòn trái, tuy nhỏ nhưng được khuyến cáo bảo tồn trong các phẫu thuật Hybrid. Trường hợp có bất thường giải phẫu – như động mạch đốt sống trái xuất phát thẳng từ quai động mạch chủ …, thì nên điều trị theo cách nào. Mục tiêu của báo cáo là đề xuất chỉ định và kỹ thuật tái lập tuần hoàn động mạch đốt sống khi có bất thường giải phẫu (hoặc hẹp do xơ vữa) trong phẫu thuật Hybrid điều trị bệnh lý động mạch chủ ngực. Phương pháp nghiên cứu: Nghiên cứu mô tả ba trường hợp lâm sàng bệnh lý – chấn thương vùng quai động mạch chủ có bất thường vị trí xuất phát động mạch đốt sống trái được điều trị bằng can thiệp nội mạch và tái lưu thông động mạch đốt sống vào động mạch cảnh chúng trái. Kết quả: Không quan sát thấy biến chứng liên quan đến phẫu thuật và tái lập tuần hoàn động mạch đốt sống rất tốt trên phim chụp kiểm tra sau mổ. Kết luận: Mặc dù chỉ định còn gây tranh cãi, tái thông động mạch đốt sống trái bất thường vào động mạch cảnh trái là kỹ thuật an toàn, đem lại lợi ích cho bệnh nhân, có thể thực hiện thường quy tại bệnh viện Hữu nghị Việt Đức.
Tổng quan: Thiếu máu đại tràng là một biến chứng ít gặp nhưng rất nguy hiểm trong phẫu thuật thay đoạn động mạch chủ bụng. Các yếu tố nguy cơ biến chứng gồm có: tuổi cao, suy thận trước mổ, phồng động mạch chủ bụng vỡ, thời gian mổ kéo dài, không tái lập tuần hoàn động mạch chậu trong. Chẩn đoán sớm bằng soi đại tràng. Điều trị bằng phẫu thuật cắt đoạn đại tràng nếu còn chỉ định với tiên lượng nặng. Tỷ lệ tử vong cao do viêm phúc mạc và sốc nhiễm trùng. Can thiệp nội động mạch chủ bằng stentgraft có tỉ lệ gặp biến chứng này thấp hơn. Phương pháp nghiên cứu: dựa trên một ca lâm sàng tại bệnh viện Hữu nghị Việt Đức và nhìn lại y văn, báo cáo nhằm mô tả các yếu tố nguy cơ, các phương pháp chẩn đoán sớm và giải pháp điều trị đối với loại biến chứng nặng nề này. Trường hợp lâm sàng: Bệnh nhân nam - 81 tuổi, tiền sử tăng huyết áp và nhiều yếu tố nguy cơ khác, được phẫu thuật thay đoạn động mạch chủ bụng – chậu do phồng động mạch, dấu hiệu biến chứng hoại tử đại tràng trái xuất hiện rõ vào ngày thứ 4 sau phẫu thuật, được điều trị thành công bằng phẫu thuật cắt đoạn đại tràng cấp cứu. Kết luận: Các yếu tố nguy cơ thiếu máu đại tràng sau phẫu thuật thay đoạn động mạch chủ bụng rất đa dạng. Chẩn đoán sớm khó khăn, tốt nhất bằng soi đại tràng. Phẫu thuật cắt đoạn đại tràng nếu còn chỉ định với tiên lượng nặng, tỉ lệ tử vong cao. Can thiệp nội mạch có thể làm giảm tỉ lệ biến chứng.
Nghiên cứu mô tả các bệnh nhân chấn thương động mạch khoeo do trật khớp gối được điều trị phẫu thuật tại bệnh viện Hữu nghị Việt Đức từ tháng 01/2017 đến tháng 07/2019. Có tổng số 21 bệnh nhân, 19/21 là nam giới, tuổi trung bình: 35,6 ± 13. Thời gian thiếu máu chi trung bình 22,4 ± 31,9 giờ. Mất mạch ngoại vi gặp ở tất cả bệnh nhân. 52,4% được phẫu thuật tái lập tuần hoàn bằng ghép đoạn tĩnh mạch hiển. Không có trường hợp nào tử vong, cắt cụt sau mổ. Chấn thương động mạch khoeo do trật khớp gối là tổn thương nặng, đe dọa sự sống còn của chi thể cũng như tính mạng người bệnh. Phẫu thuật cố định xương đồng thời tái lập lưu thông mạch máu cần được thực hiện cấp cứu để đạt được hiệu quả điều trị tối đa
Background: Infected prosthetic arterial grafts at the Scarpa triangle is a serious complication in vascular surgery with high failure results. Extra-anatomic bypass is one of solution in literature, but there is no research in Vietnam yet. This report will share some experiences of Viet Duc hospital.[1] Subjects and methods: 03 patients with infected prosthetic arterial grafts at the Scarpa triangle after lower extremity vascular surgery were treated by femoral artery ligation, perineal femorofemoral bypass or obturator foramen bypass for lower extremity revascularization, on-site wound care. Result: Primary patency rate is 100% after surgery, inguinal incision can be well healed. Re-examination after 6 months without any signs of infection or limb ischemia, the inguinal incision was completely healed. Conclusion: Extra-anatomical bypass is an effective method for revascularization of the lower extremities in the case of infected prosthetic arterial grafts.
Mục tiêu: Vùng Scarpa là một đường vào mạch máu thông dụng, tuy nhiên do vị trí giải phẫu và nhiều hệ thống bạch huyết, nên khá hay gặp biến chứng nhiễm trùng vết mổ và rò bạch huyết, đôi khi rất khó điều trị. Báo cáo nhằm đóng góp chỉ định điều trị, đề xuất kỹ thuật thực hiện và chăm sóc đối với các trường hợp nhiễm trùng vùng Scarpa sau phẫu thuật mạch máu. Đối tượng – phương pháp: Nghiên cứu ca lâm sàng tiến cứu một trường hợp có bệnh lý nền phức tạp, bệnh động mạch chi dưới rất nặng, thể trạng béo bệu, bị nhiễm trùng gây biến chứng mạch máu rất khó điều trị sau phẫu thuật tái thông động mạch chậu–đùi phải. Kết quả: Bệnh nhân nam, 62 tuổi, tiền sử đái đường, tăng huyết áp, phẫu thuật hybrid đặt stent động mạch chậu ngoài, bóc nội mạc động mạch đùi chung và bắc cầu đùi-khoeo phải bằng đoạn mạch nhân tạo PTFE số 8 có vòng xoắn, mổ lại do giả phồng động mạch đùi chung phải, cấy vết mổ có vi khuẩn. Vết mổ vùng bẹn không liền được, nguy cơ tái bục miệng nối mạch máu dẫn đến phải thắt mạch, được chăm sóc bằng hệ thống hút liên tục áp lực âm (VAC) trong 149 ngày, cuối cùng vết mổ liền hoàn toàn, bảo tồn được các cầu nối mạch máu nuôi chi. Kết luận: Hút áp lực âm liên tục (VAC) là một giải pháp điều trị có hiệu quả, an toàn cho các nhiễm trùng kéo dài của vết thương sau phẫu thuật mạch máu.
