Mục tiêu: Đánh giá đặc điểm lâm sàng, cận lâm sàng của bệnh nhân mở thông dạ dày bằng phương pháp đẩy và xác định một số yếu tố liên quan đến biến chứng nặng và tử vong trong 90 ngày sau mở thông. Đối tượng và phương pháp: Nghiên cứu mô tả, theo dõi dọc. 39 bệnh nhân có chỉ định mở thông dạ dày bằng phương pháp đẩy. Sau mở thông bệnh nhân được theo dõi và đánh giá các yếu tố ảnh hưởng trong 90 ngày hoặc cho tới khi tử vong. Kết quả: Tỷ lệ nam giới nhiều hơn chiếm 61,54%, tuổi trung bình trong nghiên cứu là 79,1 ± 8,2. Tổn thương đường tiêu hóa gặp nhiều nhất là loét thực quản chiếm 35/39 bệnh nhân. Viêm trợt thực quản và GERD gặp ở 27/39 bệnh nhân. Nhóm biến chứng nặng có chỉ số BMI là 14,24 ± 2,85, thấp hơn nhóm không bị là 20,0 ± 2,61. Chỉ số Hemoglobin là 92,0 ± 25,62 so với nhóm không bị là 105,8 ± 2,91. Mức albumin là 24,9 ± 5,88 so với 31,11 ± 4,72 ở nhóm không bị. Kết luận: Bệnh nhân mở thông dạ dày ở bệnh viện Lão khoa có tỷ lệ nam nhiều hơn nữ. Tổn thương gặp nhiều trên bệnh nhân có chỉ định mở thông là loét thực quản, viêm trợt thực quản và GERD. Các yếu tố được xác định có liên quan tới các tai biến nặng của người bệnh là chỉ số BMI, hemoglobin, albumin và số lượng tiểu cầu thời điểm thực hiện thủ thuật.

Đặt vấn đề: Thở oxy dòng cao qua mũi (HFNC) làm giảm nhu cầu đặt nội khí quản (NKQ) ở bệnh nhân suy hô hấp do COVID-19. Ngược lại trì hoãn đặt NKQ có thể dẫn tới kết cục xấu. Ngừng tuần hoàn nội viện xuất hiện trong đại dịch COVID-19 thường xuyên, chiếm 14%. Ngừng tuần hoàn có khả năng đe dọa tính mạng và liên quan đến rối loạn chức năng tim và dẫn tới tử vong. Chỉ 12% bệnh nhân được hồi sinh tim phổi sống sót ra viện và chỉ 7% bệnh nhân sống sót sau khi xuất viện với tình trạng thần kinh bình thường hoặc giảm nhẹ.1 Phương pháp: Mô tả hồi cứu trường hợp lâm sàng về đặc điểm lâm sàng, cận lâm sàng, xử trí, kết quả và nhìn lại y văn. Trường hợp: Ca bệnh đầu tiên chúng tôi mô tả ở một bệnh nhân nữ 62 tuổi bị COVID-19 mức độ nguy kịch. Bệnh nhân được thở HFNC thất bại vào ngày thứ 9 sau đó được đặt NKQ thở máy, bệnh nhân xuất hiện ngừng tuần hoàn nội viện sau đặt NKQ 45 phút. Nguyên nhân gây ngừng tuần hoàn có thể là do toan hô hấp cấp tính, tổn thương cơ tim cấp (nhồi máu cơ tim hoặc viêm cơ tim cấp) và hậu quả của cơn bão Cytokine. Bệnh nhân được cấp cứu ngừng tuần hoàn bởi các bác sĩ hồi sức cấp cứu. Tuần hoàn tự nhiên được tái lập sau 5 phút. Bệnh nhân được an thần, thở máy xâm nhập. Bệnh nhân được rút nội khí quản sau 6 ngày và xuất viện vào ngày thứ 35 với tình trạng thần kinh bình thường. Kết luận: Việc trì hoãn đặt NKQ ở bệnh nhân thở HFNC có thể dẫn tới kết cục xấu. Khi xảy ra ngừng tuần hoàn, cấp cứu đúng quy trình bởi các bác sĩ có kinh nghiệm giúp tăng khả năng tái lập tuần hoàn tự nhiên và giảm tỷ lệ biến chứng thần kinh.
