Achromobacter xylosoxidans is an aerobic gram-negative bacillus and important cause of bacteremia in immunocompromised patients. We describe a leukemia pediatric patient with severe neutropenia who developed bacteremia with A xylosoxidans resistant to multiple antibiotics, and treated the patient with tigecycline and piperacillin-tazobactam in addition to supportive medications.
Objective: The aim of this study was to evaluate the effects of using ACE inhibitors on insulin resistance, glucose metabolism, body fat composition, and lipid profile in children over 10 years of age with obesity-associated metabolic syndrome (MS).Methods: A total of 53 children with MS, who had been followed for at least one year were included in the study. The sample was divided into two groups: Group 1-30 obese children (13 female, 17 male) who were not using an ACE inhibitor and Group 2-23 obese children (13 female, 10 male) who were using an ACE inhibitor. Anthropometric and laboratory dataobtained at baseline and at the 3rd, 6th, and 12th months of follow-up were compared in the two groups.Results: Comparison of the data in the two groups at 3rd, 6th, and 12th months revealed no statistically significant differences in terms of weight standard deviation score (SDS), body mass index SDS, weight for height percentile, body fat percentage, and very low-density lipoprotein (VLDL)values. However, there were statistically significant differences in mean glucose and insulin levels, homeostasis model assessment for insulin resistance, LDL and high-density lipoprotein values, and highly significant differences in mean triglyceride values.Conclusions: The positive effects of ACE inhibitor drugs, particularly on hypertriglyceridemia and insulin resistance, might bring them forth as first-line drugs in the treatment of obese and hypertensive children. Randomized, controlled, double-blind, and long-term studies are needed for a definitive conclusion. Conflict of interest:None declared.
Öz: Oyunlaştırmanın, öğrencilere ilerleme aşamalarını göstererek öz değerlendirme yapabilecekleri önemli fırsatlar sunan yapısının, öğrenci merkezli öğrenme ortamlarında ihtiyaç duyulan değerlendirme yöntemlerine çözüm sunacağı düşünülmektedir. Bu bağlamda gerçekleştirilen çalışmada probleme dayalı öğrenme ortamında oyunlaştırma kullanılarak öğrencilere alternatif değerlendirme fırsatı sağlanmaya çalışılmış ve bu amaçla gerçekleştirilen uygulamanın incelenmesi amaçlanmıştır. Oyunlaştırmanın alternatif değerlendirme yöntemi olarak geçerliliği noktasında kanıt elde edebilmek için korelasyonel çalışma gerçekleştirilmiştir. Bu çalışma, probleme dayalı öğrenme çerçevesinde sekiz hafta boyunca bir devlet üniversitesinin teknik bilimler meslek yüksekokulunda öğrenim gören 26 öğrenci üzerinde yürütülmüştür. Çalışmanın verileri üst biliş ölçeği, bağlılık ölçeği, akademik başarı testi ve oyunlaştırma puanları kullanılarak toplanmıştır. Öğrencilerin oyunlaştırma puanları ile üst biliş puan ortalamaları ve akademik başarı puanları arasında pozitif olmasına karşın orta düzeyde anlamlı olmayan bir ilişkinin olduğu görülmüştür. Akademik başarı puanları ile bağlılık puan ortalamaları arasında pozitif, düşük düzeyde ve anlamlı olmayan bir ilişki bulunmuştur. Buna karşın oyunlaştırma puanları ile bağlılık puan ortalamaları arasında pozitif, yüksek düzeyde ve anlamlı bir ilişkinin olduğu görülmüştür. Bu ilişki oyunlaştırmanın bağlılığı artırıcı etkisine işaret etmektedir. Sonuç olarak, oyunlaştırmanın öğrencilerin notlarına doğrudan etki eden değerlendirme yöntemi olarak kullanılmasından ziyade süreç içi biçimlendirici değerlendirme olarak kullanılmasının daha doğru olacağı düşünülmektedir.
A sternal wedge osteotomy is more commonly performed in patients with pectus excavatum compared to those with pectus carinatum. All redo and pectus arcuatum cases need a wedge osteotomy for proper correction. Wedge osteotomy is very likely in more aggressive corrections with more rib resections.
Marczewski (2015) has introduced a new classification model for different user types. Tondello et al. (2016) developed and later revised (Tondello et al., 2019) "The Gamification User Types Hexad Scale" based on this model. The aim of this study is to adapt this scale into Turkish. The validity and reliability study of the scale was conducted on 330 university students. Confirmatory factor analysis (CFA) on the model also showed that the fit indices were acceptable. The Cronbach's alpha coefficients of the factors were also within the acceptable range. Additionally, it was found that there was a significant difference between the mean scores of the lower 27% and upper 27% groups for both the items and the sub-factor scores. As a result, it can be concluded that this valid and reliable scale composed of 24 items and 6 factors can be used as an effective tool in the gamification solutions considering the characteristics of the target group.
