Objectives: To compare the effects of diode and erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) lasers in second-stage implant surgery applications. Methods: This is a cross-sectional study that was carried out on patients who received implant treatment at the Departments of Oral and Maxillofacial Surgery and Periodontology, Van Yuzuncu Yil University, Van, Turkey between January 2017 and January 2018. Implants of the patients in the first group (n=20) were exposed with 940 nm Ga-Al-As diode laser while the implants of the second group (n=20) were uncovered with 2780 nm Er,Cr:YSGG laser in the second-stage surgery. Visual analogue scale (VAS) values during day 0 and following the operation on days 1, 2, 3 intraoperative bleeding grades, number of analgesics used in the postoperative period, operation time per implant, and postoperative complications were recorded. Results: When gender, smoking, the presence of systemic disease, toothbrushing frequency, analgesic use, bleeding and complications observed at the control session were analyzed, no statistically significant relationship was found between the 2 groups. It was observed that males had statistically significant higher VAS values than females ( p <0.05). Conclusion: Since diode lasers are more economical, smaller, and can meet the clinical needs of clinicians, it is likely that these lasers may be the preferred choice of the clinicians in surgical procedures.

Bu çalışmanın amacı yetişkin bir alt popülasyonda diş eti çekilmesi insidansı ile periodontal ve epidemiyolojik değişkenleri içeren olası risk faktörleri arasındaki ilişkiyi araştırmaktır. Materyal ve Metot:Bu çalışma 18-53 yaş arasında bulunan 140 gönüllü hasta üzerinde yürütülmüştür. Tüm katılımcıların demografik bilgileri, oral hijyen alışkanlıkları, diş hekimi ziyaret sıklığı, anormal alışkanlık varlığı, sigara kullanımı ve sistemik durumu kayıt altına alınmıştır. Klinik muayenede diş eti çekilmesi tespit edilen 211 alanda, ark dışı diş varlığı, dişlerde malpozisyon, aşınma, fırça izi, mobilite, dentin hipersensitivitesi, oklüzal travma, primer kontakt varlığı, anormal restorasyon kenarları, anormal frenulum varlığı, yapışık dişeti genişliği ve periodontal sağlık durumunu içeren çeşitli değişkenler incelenmiştir. Bulgular: Katılımcıların %89,3' ü sistemik olarak sağlıklı olup, %77,9' u sigara kullanmamaktadır. %31,4 oranında katılımcı yılda 2 kez diş hekimi ziyareti yapmaktadır. Bireylerin %47,9' u günde iki kez diş fırçaladığını belirtmiş, ancak tüm katılımcıların yalnızca %16,4' ünün doğru teknikle fırçaladığı gözlenmiştir. Dişeti çekilmesi riski bulunan hastalar toplam bireylerin %52,1' ini oluşturmaktadır. Bu bireylerin ise %58,9' unda dentin hipersensitivitesi, %35,6'sında frenulum faktörü ve %32,9' unda yetersiz yapışık dişeti tespit edilmiştir. Dişeti çekilmesi yaşla birlikte anlamlı şekilde artmıştır (p<0,05). Periodontitis teşhis edilen bireylerdeki dişeti çekilmesi oranı ise gingivitis teşhis edilenlerden anlamlı şekilde daha fazladır (p<0,05). Sonuç: Bu çalışmanın sınırları dahilinde, periodontal sağlık durumu, fırçalama tekniği, yetersiz yapışık dişeti ve dentin hipersensitivitesi dişeti çekilmesi ile ilişkili faktörler olarak öne çıkmaktadır.

Aim: The aim of this study was to determine the levels of biochemical markers detected in the peri-implant crevicular fluid (PICF) before and 6 months after loading in completely edentulous patients with two implant-supported mandibular overdentures and to correlate them with clinical parameters. Methodology: Two implants were applied to the mandibular canine region of the 20 completely edentulous and systemically healthy patients (11 females, 9 males) with conventional 2-stage surgery. PICF samples of the participants were collected before and 6 months after prosthetic loading, and clinical parameters—including modified plaque index (MPI), gingival index (GI), bleeding on probing (BoP), probing pocket depth (PPD), and keratinized gingival width (KGW)—were recorded. Interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), matrix metalloproteinase-8 (MMP-8), and aspartate aminotransferase (AST) levels were examined in PICF samples via enzyme-linked immunosorbent assay (ELISA). The SPSS package program was used to statistically analyze the data. Results: The average age of the participants was 57.05 ± 9.82 years. Although a decrease was observed in all clinical parameters after prosthetic loading compared to baseline levels, this decrease was not considered statistically significant (p>0.05). PICF volume showed a statistically significant decrease from baseline to 6 months (p<0.05). The decreases observed at 6th month values in all of the biochemical markers examined were not found to be statistically significant, similar to the clinical parameters (p>0.05). Conclusion: Within the limits of this study, the PICF marker levels and clinical parameters determined following osseointegration and 6 months after prosthetic loading are similar. This suggests that the studied sample has prosthetic restorations that contribute positively to the tissue maturation process and apply ideal forces to the tissue.
