Objective:This study aimed to identify possible associations of the presence or absence of posttrauma sequelae with the factors inherent to the traumatized tooth and treatment.Materials and Methods:This retrospective study was performed through the evaluation of records and radiographs of a center of reference for dental trauma between January 2008 and December 2014. The factors were analyzed and associated with posttrauma sequelae, such as pulp necrosis and root resorption.Statistical Analysis Used:A binomial logistic regression model was fit (P < 0.05).Results:In luxations, it was possible to observe 37% pulp necrosis, 16% inflammatory root resorption, and 8% replacement resorption. The binomial logistic regression revealed that male gender (P = 0.0392, odds ratio [OR] = 2.79), avulsion injury (P = 0.0009, OR = 12.27), and elapsed time >16 days between the time of trauma to the beginning of the endodontic treatment (P = 0.0450, OR = 7.53) showed a greater chance of presenting a posttrauma complication.Conclusions:Gender, type of injury, stage of root development, and time after trauma until the beginning of the endodontic intervention were related to the appearance of sequelae.
IntroduçãoO tratamento endodôntico é realizado, na maioria dos dentes, com taxas de sucesso variáveis 1,2 . Grande parte dos dentistas realiza esse tratamento em sessões múltiplas 3 . No entanto, nas últimas dé-cadas, mais de 70% das escolas, em diversas áreas geográficas, e pesquisadores defendem o tratamento em sessão única 4 . É pouco contestada a possibilidade de realização do tratamento do canal radicular em sessão úni-ca nos casos diagnosticados como pulpite irreversí-vel 4,5 . No entanto, em casos de necrose pulpar com ou sem periodontite apical, a literatura é controversa 6 . A periodontite apical é causada por bactérias no interior dos canais, e o seu tratamento deve ser feito com a remoção da causa. O preparo mecânico associado a uma irrigação pode promover culturas bacterianas negativas em 40% a 60% dos dentes tratados 7,8 . Além disso, o uso da medicação intracanal foi sugerido para potencializar a obtenção de culturas negativas 9 . Historicamente, inúmeros tratamentos e medicações entre sessões foram utilizados para a terapia de dentes infeccionados, mas ao longo dos anos o número de sessões foi reduzido. Para os casos de sessões múltiplas, um curativo de demora à base de hidróxido de cálcio tem sido proposto como padrão--ouro 4 . Assim, acreditava-se que a taxa de reparação do tratamento em sessões múltiplas devia ser mais elevado do que o tratamento em sessão única 9 .
ResumoObjetivo: Este trabalho teve como objetivo avaliar a influência da lima patência na manutenção do comprimento de trabalho (CT) durante o preparo coroa-ápice, utilizando o sistema oscilatório em canais artificiais de acrílico. Material e método: Foram utilizados 30 cubos de acrílico: a) grupo 1 (n=10): sem a lima patência e preparo no CT (CT=16 mm); b) grupo 2 (n=10): com a lima patência K#10, comprimento de patência de 17 mm e preparo no CT; c) grupo 3 (n=10): sem a lima patência, uso de uma lima K#10 em 16 mm para recapitulação e preparo no CT. Água destilada foi utilizada como solução irrigadora. O resultado foi analisado estatisticamente pelos testes de Levene e Kruskal-Wallis (p≤0,05), pelos quais se tornou possível observar a perda do comprimento de trabalho após a instrumentação, em diferentes abordagens, em relação à lima patência. Resultado: Os resultados obtidos demonstraram que houve diferença estatisticamente significante entre os grupos 1 e 3 quando comparados ao grupo 2; entretanto, ao comparar-se o grupo 1 com o grupo 3, estes apresentaram semelhanças estatísticas. O grupo 2 foi o único que conservou o CT após a instrumentação em 16 mm, enquanto os grupos 1 e 3 apresentaram uma perda de até 2 mm do CT. Conclusão: Conclui-se que a lima patência em 1 mm além do CT mostrou-se eficaz na manutenção do seu comprimento durante o preparo do canal radicular.Descritores: Endodontia; lima de patência; patência apical. AbstractObjective: This study aimed to evaluate the influence of patency file in maintaining the working lenght (WL) for the crown-down preparation using oscillating system in artificial canals of acrylic. Material and method: Thirty acrylic cubes were divided: a) Group 1 (n=10): without patency file and it were prepared in CT (CT=16 mm); b) Group 2 (n=10): with the patency #10 K-file, patency length of 17 mm and it were prepared in CT; and c) Group 3 (n=10): without patency file, using a #10 K-file in 16 mm for recapitulation and it were prepared in CT. Distilled water was used as irrigating solution. The results were statistically analyzed by the Levene and the Kruskal-Wallis test (p≤0.05), where it observed the loss of WL after instrumentation in different approaches to patency file. Result: The results showed a statistically significant difference between groups 1 and 3 compared to group 2, however the group 1 be compared in relation to these 3 showed statistical similarities. Group 2 was the one who saved the WL after instrumentation in 16 mm, while group 1 and 3 showed a loss of up to 2 mm WL. Conclusion: It was concluded that the patency file at 1 mm beyond the WL was effective in maintaining its length during root canal preparation.Descriptors: Endodontics; patency file; apical patency. A calcificação pulpar pode representar um desafio ao tratamento endodôntico principalmente durante a abertura coronária. A correta avaliação dos aspectos clínicos e radiográficos é indispensável para evitar a perfuração radicular. Este trabalho tem como objetivo mostrar por meio do relato de um caso clínico o selamento de uma perfuração radicular lateral com Agregado Trióxido Mineral (MTA). Paciente, 32 anos, sexo feminino, com queixa de dor leve ao mastigar no dente 25, desde que foi atendida na Unidade Básica de Saúde. Clinicamente, o dente 25 apresentava restauração provisória oclusomesial, não respondeu ao teste de sensibilidade pulpar, e o periodonto estava normal. Radiograficamente, havia sugestão de perda óssea mesial e comunicação com o ligamento periodontal. Na tomografia computadorizada de feixe cônico confirmou-se a presença de perfuração na face mesiopalatina, no terço médio, com perda óssea, comunicação com o ligamento periodontal e calcificação do canal radicular. Devido à presença de tecido de granulação, optou-se pela descontaminação e indução de reparo da área por meio da inserção de medicação a base de hidróxido de cálcio associado ao propilenoglicol. Após 12 meses de trocas deste curativo, foi realizado selamento com MTA branco em íntimo contato com o periodonto. Realizou-se a blindagem coronária com cimento de ionômero de vidro e resina composta. Apesar da localização e do tamanho desfavorável para o selamento desta perfuração, após 1 ano verificou-se no controle clínico-radiográfico-tomográfico que diante do tratamento executado o dente mostra-se em função, reparação óssea e assintomático.Descritores: Endodontia; Calcificações da Polpa Dentária; Tomografia Computadorizada de Feixe Cônico.
