The sensitivity and specificity of the four scoring systems were not sufficient enough in diagnosing acute appendicitis in our patient group. We concluded that the most important factor affecting the decision for surgery in suspected acute appendicitis is the surgeon's experience combined with physical findings of repeated clinical examinations.
INTRoDuCTIoN: In paediatric cases, trauma remains the most cause of morbidity and disability. Although abdominal trauma is observed less frequently in paediatric cases than isolated head trauma, it is still the leading cause of morbidity and mortality among children. Previous studies at the national level have either focused on blunt abdominal trauma in all age groups or other traumas at paediatric level. The studies targeting solely paediatric abdominal blunt trauma cases have not yet been evaluated. The aim of this study was to analyse and report the demographic characteristics, causes of trauma, developed pathologies, treatment approaches, and mortality rates in patients presenting to our emergency department with blunt abdominal trauma. MATeRIAl AND MeTHoDS: The present study was designed as a retrospective study of 36 paediatric patients admitted to the Emergency Department of Kars Harakani State Hospital with blunt abdominal trauma between 2018 and 2019. ReSulTS: In the abdominal region, the most commonly injured organ was the liver (22 cases, 52.4%), while 13 (31%) cases had splenic trauma. Thirty-one (73.8%) patients had other body injuries in addition to the abdominal trauma, the most common of which were fractures (15 patients, 35.7%) and lung traumas (12 patients, 28.6%). CoNCluSIoNS: The organs that are damaged during the injury and the parameters that can be used to detect them provide important data for the rapid interference and treatment of life-threatening situations.
Aim: We aimed to investigate whether or not artificial ascites media formed using glycerin are effective in the prevention of intraperitoneal adhesions. Methods: Thirty-six Wistar albino male rats were used in the study. The rats were divided into 3 groups as follows. Group I: control group; group II (isotonic group): 3 ml of 0.9% NaCl was injected into the peritoneal cavity, and group III (glycerin group): 0.5 ml of liquid glycerin and 3 ml of 0.9% NaCl was injected into the peritoneal cavity. Results: There were serious adhesions in the control group. Adhesion rates were lower in the isotonic group compared with the control group, but the difference was not statistically significant (p > 0.05). When adhesion rates of the glycerin group were compared with the control and isotonic groups, significant differences were found, especially between the glycerin and control groups (p < 0.05). Conclusions: According to the results of our study, the use of isotonic solution and liquid glycerin decreases postoperative adhesions. We suggest that glycerin was more effective as it has the chemical ability to draw water to its media. As such, the formation of adhesions may be decreased by increasing the amount of physiological liquid inside the abdomen.
Introduction
Caudal epidural anesthesia, when used as a sole method for surgical anesthesia, has favorable effects on the recovery duration and the time spent in the recovery unit. In this study we made a retrospective analysis of pediatric surgery operations under local, regional and general anesthesia. We aimed to find shorter postoperative recovery times with local and regional anesthesia.
Materials and methods
Data of the pediatric patients undergone subumbilical surgery during the two-year period in Pediatric Surgery clinic were collected. The patients’ age, sex, surgery type, anesthesia and airway control routes, as well as duration of anesthesia, operation and recovery were obtained.
Results
Data of 937 patients were analyzed, of whom 811 (86.6%) were males. Caudal anesthesia was performed in 240 patients (25.6%) and the mean age of these patients was 3.83 ± 3.00 years. The patients with caudal and local anesthesia spent significantly less time in the postoperative recovery unit, compared with general anesthesia groups (P < 0.001).
Conclusion
Caudal anesthesia as a sole method for pediatric subumbilical surgery is a relatively safe method. Patients having operation under caudal anesthesia have faster discharge times from postoperative recovery units, compared with general anesthesia. This probably reduces recovery unit expenditures.
