ResumoIntrodução: Diante dos baixos índices de cura e sobrevida do câncer bucal, é crucial que se diagnostique essa doença precocemente. Objetivo: Avaliar o grau de conhecimento de cirurgiões dentistas a respeito do câncer bucal. Casuística e Métodos: A pesquisa foi realizada por meio da aplicação de um questionário a um grupo de cirurgiões dentistas, exercendo atividades clínicas no município de Fernandópolis, SP, Brasil. Resultados: De 124 questionários distribuídos, o índice de resposta à pesquisa foi de 31,5% (n=39), sendo que a maioria dos participantes foi do sexo feminino 51,3% (n=20). Do total da amostra, 48,7% (n=19) afirmaram encaminhar o paciente com lesão suspeita de malignidade, para um dentista especializado. Dos entrevistados, 82,1% (n=32) afirmaram que carcinoma espinocelular é a lesão maligna mais comum na boca, e 48,7% (n=19) consideraram, corretamente, a língua como a estrutura anatômica mais comumente afetada pelo câncer de boca. A maioria dos participantes 76,9% (n=30) apontou a úlcera indolor como aspecto clínico inicial do câncer de boca, e 74,4% (n=29) consideraram a leucoplasia como a lesão bucal mais propensa a evoluir com malignidade. Todos os participantes atribuíram corretamente a ocorrência do câncer de boca ao tabagismo e 97,4% (n=38) atribuíram a ocorrência da doença ao etilismo. Conclusão: O conhecimento dos cirurgiões dentistas participantes nesta pesquisa foi considerado satisfatório, especialmente no que diz respeito ao diagnóstico clínico da doença. Objective: To evaluate the extent of the dental surgeons' knowledge regarding oral cancer. Patients and Methods: A group of dental surgeons performing clinical practice activity in the city of Fernandópolis, São Paulo State, Brazil answered a structured questionnaire. Results: Of the 124 questionnaires distributed, the return rate was 31.5% (n=39). Most of the respondents 51.3% (n=20) were women. Of the total sample, 48.7% (n=19) of the dental surgeons stated to refer the patient with injury suspected of malignancies to a specialized dentist. Of the respondents, 82.1% (n=32) reported that squamous cell carcinoma was the most common malignancy in the oral cavity, and 49.7% (n=19) implicitly acknowledged the tongue as the most commonly affected anatomical structure by oral cancer. Most of the dental surgeons 76.9% (n=30) pointed the painless ulcer as the initial clinical feature of the oral cancer, while 74.4% (n=29) considered the leukoplakia as the most likely lesion to evolve with malignancy. The term molar-incisor hypomineralization describes an uncommon pathological entity characterized by the hypomineralization of permanent first molars and commonly affecting permanent incisors. In this paper we describe a case of a 9 years old female patient attended with severe dental sensibility, masticatory and phonetic difficulties. Clinical examination revealed severe enamel structural defects affecting all four permanent first molars and opaque stains affecting permanent incisors, besides caries lesions on posterior teeth and severe osseous basis discrepancy. Radiographic examination evidenced permanent first molars imbrication and shallow lesions on the enamel of these teeth. Therefore, molar-incisor hypomineralization was defined. After fluoride varnish applications, glass ionomer cement restorations and home performed fluoride mouthwash, patient continued to report dental sensibility. Molar-incisor hypomineralization is capable of led the affected patient through morbidity which results on quality of life decrease. Due to severe dentin exposure, therapeutic interventions aiming to reduce dental sensibility are not always effective. Neste trabalho é descrito um caso de uma paciente de 9 anos de idade que se apresentou com sensibilidade dental severa, dificuldades mastigatórias e fonéticas. O exame clínico revelou graves defeitos de esmalte estruturais que afetavam todos os quatro primeiros molares permanentes e manchas opacas que afetavam os incisivos permanentes, além de lesões de cárie em dentes posteriores e grave discrepância entre as bases ósseas. O exame radiográfico evidenciou imbricação dos primeiros molares permanentes e lesões superficiais no esmalte desses dentes. Diante desse quadro clínico, foi definido o diagnóstico de hipomineralização molar-incisivo. Após aplicações de verniz fluoretado, restaurações de cimento de ionômero de vidro e bochechos fluoretados realizados em casa, a paciente continuou a apresentar sensibilidade dental. A hipomineralização molar-incisivo proporciono à paciente alta morbidade e redução na qualidade de vida. Diante da grave exposição da dentina, intervenções terapêuticas com o objetivo de reduzir a sensibilidade dental nem sempre são eficazes. Introduction: Specific words have been adopted to designate the several structures in Human Anatomy. However, the same structure has different names, usually derived from proper names (or Eponyms), which is still reflected in conceptual errors that complicate the Teaching and Learning of Anatomy in Multidisciplinary, Transdisciplinarity and Metadisciplinarity approaches. Within the current importance of the generalist, critical and humanistic background of the professional, the multiplicity of names of scientists and researchers attributed to anatomical structures has created real difficulties during the preparation, writing and reading of scientific papers, including the communication between Researchers and Professors. Objective: The aim of this study was to critically analyze the use of eponyms in Health Sciences, as well as compiling the eleven mains terms in which the same eponym has been used to identify at least two different structures. Results: Eponyms are not representatives of the irst anatomists to describe structures or indicate some topographic, morphological or functional aspects associated with parts of the Human Body, and so must be replaced with a location, descriptive or etiological terminology that facilitates the structural understanding. Conclusion: Because of its clinical importance, eponyms will not completely disappear, but due to discrepancies and secondary confusions to its use, there have been many suggestions and impositions of anatomical societies to minimize their use.
BackgroundPhotodynamic therapy is a therapeutic modality that has consolidated its activity in the photooxidation of organic matter, which arises from the activity of reactive oxygen species.ObjectiveTo evaluate the effect of red laser 660nm with the photosensitizer methylene blue on Propionibacterium acnes in vitro.MethodThe experimental design was distributed into four groups (1 - control group without the application of light and without photosensitizer, 2 - application of light, 3 - methylene blue without light, and 4 - methylene blue with light). Tests were subjected to red laser irradiation 660nm by four cycles of 5 minutes at 3-minute intervals.ResultsIt was evidenced the prominence of the fourth cycle (20 minutes) groups 2, 3 and 4.Study limitationsDespite the favorable results, the laser irradiation time photosensitizer associated with methylene blue were not sufficient to to completely inhibit the proliferation of bacteria.ConclusionFurther studies in vitro are recommended to enable the clinical application of this photosensitizer in photodynamic therapy.
O cirurgião-dentista, em seu cotidiano laborativo está intimamente ligado com o risco de ocorrer em seu consultório uma emergência médica. Mesmo não sendo comuns, essas emergências podem acontecer em ambiente odontológico. Podem acometer qualquer indivíduo, antes, durante ou após a intervenção odontológica. Desta maneira além dos acometimentos relativos à saúde de seus pacientes o cirurgião-dentista também está às vistas com as implicações legais que estas emergências geram para o profissional, tendo de se ater a toda responsabilidade que tem para com o paciente, no sentido da promoção de saúde, e no sentido legal. O cirurgião-dentista deve estar preparado para todos os eventos que possam a vir a acontecer em seu ambiente de trabalho. Uma avaliação do estado geral de saúde de seus pacientes, adoção de medidas preventivas e jurídicas, dá maior segurança no atendimento, bem como, as implicações éticas e legais as quais estão submetidos os cirurgiões-dentistas. Através da pesquisa bibliográfica procuramos alertar e orientar o cirurgião-dentista sobre as principais situações de emergência médica que poderão ocorrer no consultório odontológico. Apresentaremos os sinais e sintomas das principais emergências médicas e quais as formas de tratamento ou os primeiros cuidados que deverão ser tomados para a manutenção da vida do paciente. A maioria dos estudos aponta uma elevação no número de emergências médicas, associada a uma grande insegurança dos cirurgiões-dentistas no enfrentamento destas situações emergenciais.
Introduction: Oral cancer is a worrying disease which claims for actions aiming its early diagnosis and prevention. Objective: To describe lesions diagnosed during oral cancer prevention campaign performed in the city of Fernandópolis, Brazil, in 2014. Material and methods: Patients who attended for the basic health units of the city of Fernandópolis on the day of the campaign were examined by a previously trained dentist who searched for oral lesions suggestive for oral squamous cell carcinoma or potentially malignant lesions. Patients with suspicious lesions were scheduled for re-evaluation by an expert in oral diseases for obtaining the right diagnosis of the lesions. Results: 1,003 patients were examined during the campaign. Among them, 94 presented oral lesions, although, only 54 attended for re-evaluation and adequate diagnosis conduct by the dentist expert in oral diseases. Of the re-evaluated patients, 42 (77.77%) were diagnosed with oral benign lesions, whereas 13 (24.07%) were diagnosed with normal oral variation and only 1 (0.09%) was diagnosed with oral squamous cell carcinoma. Conclusion: Low oral malignant lesions diagnosis rates found on this campaign might be due to lack of methodology, which needs to be improved aiming to reach patients in real risk groups for oral cancer development. Furthermore, based on the rated of benign lesions and normal oral variations, dentists need more continued education regarding oral cancer for clinically detect oral malignant lesions and instruct patients regarding this malignancy.
ResumoIntrodução: Medicações para controle da dor, do estresse e da ansiedade podem ser utilizadas em várias especialidades da Odontologia. Os benzodiazepínicos são atualmente os medicamentos de escolha, principalmente o midazolam, que apresenta ampla margem de segurança clínica, rápido início de ação, pequena incidência de reações adversas, sendo indicado até mesmo para crianças como pré-medicação em procedimentos de curta duração. Objetivo: Este estudo teve como objetivo apresentar um caso clínico de retratamento endodôntico em odontopediatria sob sedação mínima com midazolam administrado pela via oral. Relato do caso: Paciente de 7 anos, obesa, não adaptada à assistência odontológica, apresentou como queixa principal "bolinha de pus" na região do elemento 36, já tratado endodonticamente. Ao exame clínico foi constatada fístula intra-oral e ao exame radiográfico foi observado cone de guta-percha trepanando o ápice dentário. O retratamento endodôntico foi realizado sob sedação mínima com midazolam, inicialmente na dose de 15 mg, e em sessões posteriores na dosagem de 30mg. A paciente teve seus sinais vitais monitorados durante todas as intervenções. Após 7 meses do término do retratamento, a radiografia de controle aponta recuperação tecidual no ápice e região de furca do elemento dentário, clinicamente os sinais e sintomas foram totalmente controlados, e segundo os familiares, o sucesso do tratamento não seria possível sem o uso da sedação mínima. Conclusão: Este e outros relatos positivos poderiam inferir que, num futuro próximo, a sedação mínima com fármacos benzodiazepínicos venha a se tornar uma ferramenta indispensável no ambiente odontológico. Benzodiazepines are currently the drugs of choice, especially midazolam, which has a wide margin of clinical safety, rapid onset of action, low incidence of adverse reactions, and even suitable for children as premedication in short procedures. Objective: This study aimed to present a clinical case of endodontics pediatric dental care under minimal sedation with midazolam administered orally. Case report: 7 years old pacient, obese, not adapted to dental care, presented as main complaint "ball of pus" in the region of the element 36, already treated endodontically. On clinical examination revealed an inter oral fistula and radiographic examination was observed a cone of gutta-percha transcend dental apex. Endodontic retreatment was performed under minimal sedation with midazolam, initially at a dose of 15 mg, and in subsequent sessions in the dosage of 30mg.
