ObjectivesAn increasing number of International Medical Graduates (IMG), who are defined to be physicians working in a country other than their country of origin and training, immigrate to Western countries. In order to ensure safe and high-quality patient care, they have to take medical and language tests. This systematic review aims to (1) collect all empiric research on intercultural communication of IMGs in medical settings, (2) identify and categorize all text passages mentioning intercultural issues in the included studies, and (3) describe the most commonly reported intercultural areas of communication of IMGs.MethodsThis review was based on the PRISMA-Guidelines for systematic reviews. We conducted a broad and systematic electronic literature search for empiric research in the following databases: MEDLINE, BIOSIS Citation Index, BIOSIS Previews, KCI-Korean Journal Database and SciELO Citation Index. The search results were synthesized and analyzed with the aid of coding systems. These coding systems were based on textual analysis and derived from the themes and topics of the results and discussion sections from the included studies. A quality assessment was performed, comparing the studies with their corresponding checklist (COREQ or STROBE). Textual results of the studies were extracted and categorized.ResultsAmong 10,630 search results, 47 studies were identified for analysis. 31 studies were qualitative, 12 quantitative and 4 studies used mixed methods. The quality assessment revealed a low level of quality of the studies in general. The following intercultural problems were identified: IMGs were not familiar with shared decision-making and lower hierarchies in the health care system in general. They had difficulties with patient-centered care, the subtleties of the foreign language and with the organizational structures of the new health care system. In addition, they described the medical education in their home countries as science-oriented, without focusing on psychosocial aspects.ConclusionThere is a need for a better training of IMGs on culture-related and not culture-related topics in the new workplace country. The topics that emerged in this review constitute a basis for developing these courses. Further empiric research is needed to describe the findings of this review more precisely and should be in accordance with the existing reporting guidelines.
Hintergrund: Ärztinnen und Ärzte in Weiterbildung sind ein wesentlicher Pfeiler der Gesundheitsversorgung im deutschen Gesundheitssystem. Ärztliches Personal in Weiterbildung qualifiziert sich nach dem Studium noch bis zur fachärztlichen Reife. In Deutschland ist die Arbeitsbelastung dieser Berufsgruppe so hoch, dass der Wunsch nach Entlastung geäußert wird. Eine Möglichkeit dazu bietet die Delegation von ärztlichen Tätigkeiten an Physician Assistants (studierte ärztliche Assistenten/-innen, PA). Es herrscht jedoch häufig Unsicherheit darüber, wie Physician Assistants ausgebildet sind und unter welchen Kautelen die Gesundheitsversorgung anstelle von ärztlichem Personal in Weiterbildung durch PA erfolgen kann. Methode: In einer fokussierten Internetrecherche wurde die Entwicklung des Berufsbildes PA in Deutschland nachvollzogen und aktuelle Studiengänge, deren Dauer und Voraussetzungen, sowie die Empfehlungen zu Mindestinhalten und zu erwerbenden Kompetenzen recherchiert und mögliche Tätigkeiten und Einsatzbedingungen erarbeitet. Letztere wurden dann den Tätigkeiten und Einsatzbedingungen des ärztlichen Personal in Weiterbildung gegenübergestellt. Ergebnisse: Obwohl seit 2005 in Deutschland das Studium möglich ist, sind PA in Deutschland noch selten. Die Voraussetzungen für und die Dauer des Studiums sind aktuell abhängig vom Anbieter. Das Studium sollte zu zahlreichen praktischen Kompetenzen qualifizieren, welche die Übernahme ärztlicher Tätigkeiten in Delegation ermöglicht. Die Rahmenbedingungen zur Delegation an ÄrztInnen in Weiterbildung oder an PA sind vergleichsweise ähnlich, so dass diese Berufsgruppen in Teilen sehr analog zueinander eingesetzt werden können. Wesentliche Unterschiede umfassen Tätigkeiten unter Arztvorbehaltes und innerhalb des ärztlichen Kernbereichs. Diskussion: Der Einsatz von PA stellt immer eine Zuarbeit an die fachärztliche Ebene dar mit dem Ziel, die Mitglieder des ärztlichen Teams zu entlasten. Daten und Erkenntnisse aus dem deutschen Gesundheitswesen zu diesem Berufsbild fehlen bislang. Schlussfolgerung: Gesundheitsversorgung durch PA kann im Alltag vergleichsweise ähnlich zur Versorgung durch Ärztinnen und Ärzte in Weiterbildung erfolgen, sofern die Tätigkeiten außerhalb des Arztvorbehaltes und des ärztlichen Kernbereichs angesiedelt sind.
Zusammenfassung Ziel der Studie ÄrztInnen in Deutschland sind überlastet. Entlastung könnte durch Delegation von ärztlichen Aufgaben an Physician Assistants (PA) geschaffen werden. Obwohl seit 2012 PA im deutschen Gesundheitswesen mitarbeiten, gibt es wenig Daten. Wir haben die Vor- und Nachteile der Delegation aus Sicht von ÄrztInnen, die seit Jahren an PA delegieren, sowie die Arbeitsbedingungen, die Zufriedenheit und Prozesszeiten von PA untersucht. Methodik Semi-quantitative Querschnittserhebung an einer Abschlusskohorte des Studienganges PA und den an diese seit drei Jahren delegierenden ÄrztInnen. Retrospektive Analyse von Wartezeiten und Aufenthaltsdauer in einer interdisziplinären Notaufnahme. Ergebnisse Die ÄrztInnen waren mit den PA sehr zufrieden und gaben eine deutliche Entlastung durch diese an. Auch die PA zeigten eine sehr hohe berufliche Zufriedenheit. Prozesszeiten in der Notaufnahme wurden durch die Mitarbeit einer PA nicht verlängert. Schlussfolgerung ÄrztInnen, die seit Jahren an PA delegieren, sind entlastet und mit den PA zufrieden, gleichermaßen sind PA zufrieden mit ihrem Beruf.
The patient communication test evaluates the communication skills of foreign doctors in OSCE-setting using standardized actor-patients. This improves the communication skills and enhances patient safety in the German health care system by smooth inter-professional and patient-centered communication.
In consultation with the State Examination Office Stuttgart, some processes for optimisation have been devised, e. g., to develop a special curriculum which could be used for guidance and to recruit more examiners. The recommendations of the focus group from Baden-Wuerttemberg are an important step towards more transparency and possibly towards a Germany-wide standardisation of this exam.
