Медикаментозная профилактика и хирургическое лечение тромбозов представляют собой устоявшуюся и важную часть микрохирургических операций. До настоящего момента не решены вопросы профилактики микроциркуляторных нарушений при выполнении аутотрансплантации комплексов тканей у детей, отсутствуют показания и противопоказания к тому или иному виду антикоагулянтной терапии, а также отсутствуют исследования предоперационных рисков развития сосудистых нарушений при микрохирургических аутотрансплантациях кровоснабжаемых комплексов тканей. Таким образом, целями исследования являлись оценка протромботических факторов и оптимизация выбора антикоагулянтной терапии при микрохирургических операциях у детей. С 2011 по 2020 гг. в ФГБУ «НМИЦ ДТО Г.И. Турнера» выполнено 382 микрохирургические аутотрансплантации комплексов тканей на сосудистой ножке у детей с врожденной и приобретенной патологией ОДА. В 317 случаях выполнили аутотрансплантации пальцев стоп в позицию пальцев кисти, из них 243 (76,7%) при врожденной патологии кисти и 74 (23,3%) при приобретенных деформациях верхних конечностей. Использование балльной системы оценки до-и интраоперационных рисков позволяет на ранних этапах оценить возможность развития микроциркуляторных осложнений и выбрать оптимальный вид антикоагулянтной терапии для каждого пациента.
