Цель. У полиморбидных больных с артериальной гипертензией (АГ) изучить клиническую эффективность пациентоориентированной терапии в сравнении с традиционной терапией путем оценки рациональности (вторичная комбинированная конечная точка) и общей смертности (первичная конечная точка). Материал и методы. У 1000 полиморбидных больных с АГ в циркулярном проспективном исследовании оценили динамику рациональности и общую смертность при пациентоориентированной и традиционной терапии. Результаты. Начиная с 24 нед, стратегия пациентоориентированной терапии продемонстрировала статистически значимое преимущество перед традиционной терапией по критерию вторичной комбинированной конечной точки (Student's t-test: р=0,049 на 50-й и 100-й нед; р=0,002 на 150-й нед; р=0,004 на 200-й нед). К завершению исследования в пациентоориентированной подвыборке положительная тенденция значений рациональности была достигнута у абсолютного большинства респондентов, тогда как в подвыборке традиционной терапии преобладающая часть исходных значений изменилась несущественно. Начиная с 50 недели исследования показатель смертности в подвыборке традиционной терапии, по сравнению с подвыборкой пациентоориентированной терапии продемонстрировал тенденцию опережающего роста, сохраняющегося до завершения исследования, при этом тренданализ продемонстрировал экспоненциальный рост тенденции. Различия общей смертности в подвыборках оказались статистически значимыми. Была выявлена сильная связь между повышением значений показателя «рациональность» и снижением общей смертности. Заключение. Подход, основанный на управлении лечением по критерию рациональности, продемонстрировал возможность повышения эффективности лечения, в том числе снижению общей смертности без привлечения дополнительных ресурсов и медицинских вмешательств.
Для профилактики тромбоэмболических осложнений у больных фибрилляцией предсердий (ФП) неклапанного генеза рекомендовано применение варфарина или прямых оральных антикоагулянтов [1]. Варфарин в клинической практике используют в течение многих десятилетий, однако непредсказуемость его фармакокинетики, фармакодинамики и широкий спектр опасных взаимодействий требует тщательного мониторинга международного нормализованного отношения (МНО). При этом на фоне лечения варфарином у 12% пациентов развиваются нежелательные эффекты [2].
Aim. To evaluate treatment adherence and prevalence of CYP2C9 and VKORC1 gene mutations in patients with atrial fibrillation (AF) and provide rationale of choice for oral anticoagulation therapy. Material and methods. Treatment adherence was evaluated in 137 AF patients (aged 35-85 years) with quantitative estimation of drug therapy adherence along with compliance to medical support and lifestyle modifications. Among them 82 patients underwent polymerase chain reaction (PCR) analysis of CYP2C9 and VKORC1 gene polymorphisms. Results. Patients receiving anticoagulation therapy are characterized by lower level of adherence compared to patients without anticoagulants (65.2±19.3% vs 68.5±19.1%; Wald-Wolfowitz; p<0.05). Considering all studied parameters men are less adherent than women (54.7±18.6% vs 60.6±16.7%; Kolmogorov-Smirnov; p<0.05). Patients receiving new oral anticoagulants (NOAC) have better compliance compared with patients of warfarin group. Mutations in CYP2C9 gene were detected in 32.9%, VKORC1 -in 68.3%, and their combination -in 21.9% of study participants. Warfarin therapy may be potentially dangerous in such patients due to low adherence. Conclusion. Considering high prevalence of CYP2C9 and VKORC1 gene mutations treatment adherence should be estimated to optimize choice of anticoagulation therapy. Оценить приверженность к лечению и распространенность мутаций генов CYP2C9 и VKORC1 у пациентов с фибрилляцией предсер-дий (ФП), а также обосновать принципы выбора пероральных антикоагулянтов для этой категории пациентов. Материал и методы. У 137 больных ФП 35-85 лет изучили приверженность к лечению с количественным расчетом показателей привер-женности к лекарственной терапии, медицинскому сопровождению и модификации образа жизни. У 82 больных из этой же выборки ме-тодом полимеразной цепной реакции исследовали полиморфизм генов CYP2C9 и VKORC1. Результаты. Для пациентов, получающих антикоагулянтную терапию, характерен более низкий уровень приверженности к лечению в сравне-нии с пациентами, ее не получающими (65,2±19,3% против 68,5±19,1%; Wald-Wolfowitz; р<0,05). При этом по всем исследуемым пара-метрам мужчины менее привержены к лечению, чем женщины (54,7±18,6% против 60,6±16,7%; Kolmogorov-Smirnov; р<0,05). Пациен-ты, получающие варфарин, менее привержены к лекарственной терапии в сравнении с пациентами, получающими новые пероральные ан-тикоагулянты (НОАК). Мутации гена CYP2C9 выявлены у 32,9%, VKORC1 -у 68,3%, а их одновременное носительство -у 21,9% участни-ков исследования. У таких больных низкий уровень приверженности к лечению делает терапию варфарином потенциально опасной. Заключение. Цель. Изучить клинико-экономические преимущества пациентоориентированного подхода в сравнении с традиционным лечением полиморбидных больных с синдромом артериальной гипертензии (АГ) по критериям вторичной комбинированной (выгодность) и первичной конечной (общая смертность) точек. Материал и методы. В регистровом когортном исследовании у 1000 полиморбидных больных с синдромом АГ оценили динамику показателя «выгодность» и общую смертность при пациентоориентированной (n=500) и традиционной (n=500) терапии. Результаты. Изменение выгодности связано не с видом или стоимостью применяемых лекарственных средств, оригинальным либо генерическим характером препаратов, или кратностью их суточного приема, а с выраженностью ответа стартовой терапии и эффективностью ее последующей модификации на основе оценки исходной приверженности респондентов к лечению. Основанный на активной обратной связи пациентоориентированный подход продемонстрировал преимущества перед традиционным. Принцип оценки «стоимость жизни в эффективных рублях» показал, что при пациентоориентированном лечении, даже в случаях увеличения прямых расходов на лекарственную терапию, достигаемое клиническое улучшение при снижении общей смертности обеспечивает большую выгодность лечения, чем традиционная терапия. При исходных значениях выгодности, составлявших 4,22 и 4,28 эффективных рублей, к 100-й нед исследования они достигли 3,08 и 4,09, а к 200-й-2,75 и 3,75 рублей, соответственно. За 200 нед смертность составила 3,8% в первой, и 5,4% во второй подвыборке (р=0,017). Заключение. Анализ выгодности может рассматриваться как важный шаг к разрешению «конфликта интересов» субъектов оказания медицинской помощи. Введение в клиническую практику оценки выгодности позволяет практическому врачу обосновывать клинико-экономическую целесообразность конкретной терапевтической схемы, в том числе-выбор коммерческой номенклатуры лекарственных средств.
On the basis of the retrospective stratified research of 307 patients (143 men and 164 women) with the verified chronic gastritis, association with helicobacter pylori, lead on the basis of 6 municipal establishments of Omsk treatment of the given category of patients is established, that at equal efficiency of out-patient-polyclinic and stationary forms of rendering of medical aid by clinical criterion, from a position of budgeting, it is the most expedient in conditions of out-patient-polyclinic supervision.
