Обращено внимание на противоречие, связанное с оценкой совместимости лекарственных (ЛВ) и вспомогательных (ВВ) веществ: как несовместимые рассматривают такие ВВ и ЛВ, которые могут химически взаимодействовать между собой. В то же время в составе некоторых оригинальных препаратов имеются ВВ и ЛВ, способные к химическому взаимодействию. Для устранения этого противоречия введено понятие «потенциальная несовместимость» ВВ и ЛВ — способность веществ к прямому или опосредованному химическому или неблагоприятному физическому взаимодействию, которое потенциально может проявиться при производстве или хранении лекарственного препарата и оказать влияние на его качество. Подчеркнуто, что вывод о несовместимости ВВ и ЛВ можно делать только в том случае, если не удается обеспечить их совместимость в лекарственном препарате. Рассмотрены способы оценки совместимости ВВ и ЛВ: стрессовые эксперименты с использованием хроматографических методов анализа; стрессовые эксперименты с использованием спектральных методов анализа; эксперименты с использованием термических методов анализа и калориметрии. Предложен и обоснован системный подход «Step-by-Step Approach» к исследованию совместимости ВВ и ЛВ, основанный на экспрессном тестировании взаимодействия ВВ и ЛВ не только в модельных смесях, но и в образцах разрабатываемых лекарственных форм. Это имеет решающее значение для надежных выводов о совместимости ВВ и ЛВ. Обсуждены известные типы потенциально несовместимых веществ, важные для прогноза потенциальной несовместимости ВВ и ЛВ на основании особенностей структуры и химических свойств молекул. Даны рекомендации, связанные с исследованием совместимости ЛВ и ВВ.
