Objectives: To assess the functional capacity evolution and the physiologic score in patients with proximal femoral end fractures, as well as to compare the final results of the treatment provided to the several pre-fracture variables. Materials and Methods: A prospective study with patients over 40 years old diagnosed with proximal femoral fracture. The patients were submitted to a pre-established protocol and followed up on an outpatient basis for a period of one year. Results: 68 patients were assessed (27 men and 41 women), with a mean age of 75,84 years. 83,82% were submitted to surgical treatment. A mandíbula é o único osso móvel da face e participa de funções básicas como mastigação, fonação e deglutição, além de participar da manutenção da oclusão dentária ocupando juntamente com a maxila a maior porção óssea do esqueleto facial 1 .Devido a sua topografia, anatomia e projeção no terço inferior da face, é freqüentemente atingida por traumas podendo resultar em fraturas, principalmente em acidentes de trânsito, agressões, quedas ou acidentes esportivos 10 .As fraturas mandibulares podem levar à deformidades, sejam por deslocamentos ou perdas ósse-as não-restauradas, com alterações de oclusão dentária ou da articulação temporomandibular (ATM). Quando não identificadas ou tratadas adequadamente, estas lesões podem levar à seqüelas graves, tanto estéticas como funcionais 1,6,11 .Este estudo tem como objetivo avaliar 166 pacientes consecutivos com fraturas de mandíbula atendidos e tratados no setor de Trauma de Face da disciplina de Cirurgia Plástica da UNIFESP-EPM, distribuídos conforme o sexo, idade, etiologia, localização das fraturas, técnicas de tratamento e complicações pós-operatórias. PACIENTES E MÉTODOForam estudados 166 pacientes consecutivos, vítimas de trauma mandibular (em um total de 267 fraturas), atendidos e tratados na disciplina de Cirurgia Plástica da UNIFESP/EPM no período de janeiro de 1991 a março de 1996.Os pacientes foram analisados através do levantamento de prontuários, avaliação das radiografias do pré e pós-operatório e através das Fichas de Trauma, específicas do setor de Trauma de Face da Cirurgia Plástica da UNIFESP / EPM. Foram então agrupados segundo o sexo, faixa etária, lesões associadas, tratamento e complicações pós-operatórias. RESULTADOSDo total de pacientes, encontramos 135 homens (81,3%) e 31 mulheres (18,7%), em um índice masculino/feminino de 4,3:1 (gráfico 1).A faixa etária variou de 1 ano e 4 meses a 59 anos (média de 27,11). As idades foram agrupadas em décadas, e a faixa etária mais acometida foi dos 20 aos 29 anos (42,8%), e a menos dos 50 aos 59 (4,8%) (gráfico 2).A etiologia mais freqüente de fraturas de mandíbula neste estudo foi relacionada aos acidentes de trânsito (81 casos -48,8%), em que 41,6% envolviam veículos de transporte e 7,2% atropelamento; seguido das quedas (44 casos -26,5) e agressões (39 casos -23,5%). Somente dois pacientes sofreram acidentes esportivos (gráfico 3). As diferentes etimologias dos acidentes de trânsito e agressões encontram-se na tabela 1.Foram identificadas 267 fraturas nos 166 pacientes tratados (1,6 fraturas/paciente), sendo que 86 pacientes (51,8%) apresentavam fraturas úni-cas e 80 pacientes (48,2%) fraturas múltiplas, com média de 2,2 fraturas/paciente no último grupo.A região do corpo da mandíbula foi a mais atingida (76 fraturas -28,5%), seguida do côndilo (71 fraturas -26,6%), região sinfisária (53 fraturas -19,9%), ângulo (38 fraturas -14,2%), e alveolar isoladamente (5 fraturas -1,9%). In this work an epidemiological analysis on tibial shaft fractures was performed. During four years, our service treated 179 fractures, 132 in male, 47 in female, aged 14 to 83 years. The 21 to 30-year-old patiens were the more injured. Of these, 120 were open and 59 close fractures of which prevailing cause was road traffic accident. The study based on patients promptuaries analyses and radiographs. The fractures occurred 97 times in the middle third (54.18%); 102 times (56.98%) presented simple fragments, and 57 (31.38%) oblique lines. We treated close and open fractures, respectively, 48 and 38 cases with plaster cast immobilization; 3 and 67 with external fixation after plaster cast immobilization; 5 and 12 with osteosynthesis by means of plate and screws, and 2 and 3 with external fixation only. In both close and open fractures, respectively, 7 and 20 cases of pseudarthrosis and 1 and 11 of infections have occurred. With the data obtained we verified an actual validity of the epidemiological studies as a contribution for better identifying lesions features and their treatment and complications. This allows proceedings and apprenticeship refinement.

ObjectiveTo evaluate the reproducibility of the radiographic classifications of Gartland and the Association for Osteosynthesis/Association for the Study of Internal Fixation (AO/ASIF) for supracondylar fractures of the humerus in children.MethodsOn two occasions, 50 radiographs in anteroposterior and lateral views were evaluated by three pediatric orthopedists in accordance with the Gartland and AO/ASIF pediatric classifications. Their responses were subjected to statistical analysis consisting of calculation of the κ coefficient to assess the intra- and interobserver concordance, in both classifications.ResultsThe strength of the intraobserver concordance was high or near perfect for the three examiners in the two classification systems. The strength of the interobserver concordance was high in the two systems, with κ coefficients of 0.756 for the Gartland classification and 0.766 for the AO/ASIF classification.ConclusionThe Gartland and AO/ASIF classification systems showed similar reproducibility and performance. High strength of concordance was seen in the intra- and interobserver analyses.

Resumo Objetivos Demonstrar a prevalência de sintomas de depressão e ansiedade em pessoas com a síndrome do túnel do carpo atendidas em um ambulatório de cirurgia da mão e descrever as características clínicas e epidemiológicas dessa população. Métodos Foram avaliadas pessoas com o diagnóstico de síndrome do túnel do carpo na consulta inicial em um período de coleta de dados de 6 meses. Características clínicas e epidemiológicas foram anotadas, e as pessoas que possuíam diagnóstico de ansiedade e/ou depressão foram avaliadas. Resultados Um total de 101 pessoas apresentavam síndrome do túnel do carpo e destas, 38 apresentavam diagnóstico de depressão e 29 de ansiedade. Houve prevalência de mulheres de baixa renda, com predominância do nível de escolaridade fundamental. Mais da metade dos pacientes apresentava pelo menos uma comorbidade sistêmica associada. Conclusão Tanto para ansiedade como para depressão, as características que influenciaram estatisticamente nos sintomas em pacientes com síndrome do túnel do carpo independente das demais características avaliadas foram gênero, tabagismo, e renda familiar (p < 0,05).

Objective: To report clinical and epidemiological characteristics and risk factors of patients with Carpal Tunnel Syndrome (CTS) on sick leave admitted in a University Hospital. Methods: This is a transversal study conducted with patients admitted sequentially over 18 years of age, without distinction of gender and race in the period between September and November 2014. Patients answered a questionnaire and underwent physical examination carried out by the authors. Results: Twenty-five patients were admitted within three months, all females, the mean age was 50.24 years old (standard deviation 10,39) . Among the professions they performed, general and cleaning services were the most prevalent. Most patients featured obesity, followed by depression and systemic arterial hypertension. Approximately half of them were on sick leave. Sleep disorders were also a frequent complaint. Conclusion: CTS is a frequent cause of sick leave and it is related to obesity, dyslipidemia and depression. Level of Evidence IV, Series of Cases.
