Due to their unique relationship with patients, nurses should be involved in EOL care decision making; however, patients, families or nurses are not often involved in the decision-making process in Turkey.
Policymakers and nurse managers should be aware of any negative issues regarding nursing practice environments and job security to improve nursing outcomes.
Using research results in nursing practices and basing nursing practices to evidence have been viewed as an important factor in increasing care quality. The aim of this research was to identify the barriers arising from utilizing research findings perceived by nurses. The research was conducted with participation of 748 nurses. The study is descriptive in nature, and the 'Barriers Scale' and a question form developed by the researchers were used as data collection instruments. The data were gathered through face-to-face interviews after the necessary permissions were taken. A total of 54.3% of the nurses are ≤ 30 years old, and 49.6% of them are graduates of high school-foundation degree. The nurses (63.8%) indicated the most important barrier as 'there is not enough time to read about research at work'. According to the results of this study, providing the nurses with both financial and information support after graduation is an important factor in facilitating research and using research results. Depending on the results of our study, we recommend that the number of publications in nursing and the opportunities of access to these publications be increased, and there should be more management support for research, and nurses should be provided with time.
Amaç: Bu çalışma, üst düzey yönetici hemşirelerin hastanelerde hemşirelik çalışma ortamı hakkındaki görüş ve deneyimlerini ortaya koymak amacıyla yapılmıştır. Yöntem: Bu tanımlayıcı ve nitel çalışma, İstanbul'da gerçekleştirilmiş olup, özel ve devlet hastanelerinde çalışan 18 üst düzey yönetici hemşire ile yarı yapılandırılmış bireysel derinlemesine görüşmeler yapılmıştır. Veriler kayıt cihazı ile kaydedilmiş ve tematik olarak içerik analizi yapılarak değerlendirilmiştir. Bulgular: Liderlik ve yönetim uygulamaları, hemşire insan kaynağı ve yönetimi, iletişim ve ilişki yönetimi olmak üzere üç ana tema belirlenmiştir. Yönetici hemşirelerin özerkliğinin düşük olduğu, hastane yönetim kararlarına katılımının yetersiz olduğu ve yönetici hemşirelerin temizlik faaliyetleri gibi hemşirelik dışı destek hizmetlerin koordinasyonunu sağladıkları saptanmıştır. Yönetici hemşireler, hastanelerde eğitim-geliştirme faaliyetleri ve terfi olanaklarının yanı sıra hemşirelerin nitelik ve niceliklerinin çok yetersiz olduğunu, iş yükü ve devir hızlarının çok yüksek olduğunu, çalışma koşullarının iyi olmadığını ve özellikle kuşakları yönetmede çok zorlandıklarını belirtmişlerdir. Ayrıca hemşire-hekim ve hemşire-hemşire arasındaki iş birliği ve iletişimde sorunlar yaşandığına da değinmişlerdir. Sonuç: Bireysel derinlemesine görüşmelerden elde edilen sonuçlar, yönetici hemşirelerin hemşire çalışma ortamıyla ilgili sorunlar yaşadıklarını göstermektedir. Yönetici hemşirelerin sağlıklı çalışma ortamı standartlarını yaşama geçirebilmeleri için kendi ekipleri, birlikte çalıştıkları yöneticiler ve politika yapıcılar ile iletişim ve iş birliklerini güçlendirmeleri gerekmektedir.
ÖZET Gebelik süresince ilaç kullanımı hem annenin, hem de fetusün sağlığını yakından ilgilendiren önemli bir konudur. İlaçların fetus üzerindeki etkilerini belirleyen özellikler; ilacın tipi, dozu, alınma sıklığı, plasenta ve anne sütüne geçişi, ilacın dağılımı, metabolizması, boşaltımı, diğer ilaçlarla birlikte alınıp alınmadığı, her fetusun ilaca olan cevabı, fetusun yaşı, annenin sağlığı ve beslenmesidir. İlaçların fetus üzerine etkileri konusunda çeşitli sınıflandırma sistemleri geliştirilmiş ve sınıflandırma sistemleri arasında bazı ajanların sınıflandırılmasında farklılıklar olduğu belirlenmiştir. Sağlık personelinin bu alandaki bilgilerinin geliştirilerek güncellenmesi gerekmektedir. Bu makalede gebelikte ilaç etkileşimi, ilaç sınıflama sistemleri ve danışmanlık ele alınmıştır. Drug type, dose, receiving frequency, the placenta and breast milk passage, drug distribution, metabolism, which was received with other drugs, each fetus response to drugs, the fetus age, maternal health and nutrition are characteristics that determine their effects on the fetus of drugs. Various classification systems have been developed for the effects of drugs on the fetus and it has been determined that there are differences in the classification of some agents among the classification systems. It is necessary for the health personnel to improve and update the information in this area. This article explains drug interactions in pregnancy, drug classification systems and counseling.
