Aim: To compare the results of the water used to bath infarcted patients in bed under oxy-hemodynamic variables. Method: This research uses a 2X2 crossover of the interventions: bed bath with constant water temperature at 40°C (BB1) and 42.5°C (BB2) in 20 patients who had acute myocardial infarction (AMI). Dependent variables are: pulse oximetry, cardiac frequency (CF) and axillary temperature (Axt). The inferential statistics will be used in an analysis of the variations found repeated throughout the sample, as well as the Bonferroni test, with a level of significance of 5%. Results: The SpO2 and Axt were higher after BB2 (p<0.05) when compared with BB1. BB2 reduced CF by 1% (p=0.01). Discussion: The individual bath with controlled water temperature is capable of minimizing the oxy-hemodynamic impact. Conclusion: The bath with water temperature at 42.5ºC was shown to be more favorable than at 40°C with regard to SpO2, CF and Axt in infarcted patients.
Objetivo: analisar o impacto dos posicionamentos dorsal, lateral esquerdo e lateral direito de pacientes com síndrome coronariana aguda sobre variáveis oxi-hemodinâmicas. Método: Ensaio clínico randomizado, não controlado, unicego, realizado na Unidade Coronariana de um hospital da rede privada em Niterói, Brasil. Foi analisado o impacto do posicionamento nos decúbitos dorsal, lateral esquerdo e lateral direito sobre as seguintes variáveis: frequência cardíaca, volume sistólico, índice cardíaco, pressão arterial sistólica, pressão arterial diastólica, pressão arterial média, índice de trabalho cardíaco, contração ventricular máxima e consumo de O2, mensuradas por meio de um aparelho de cardiografia por impedância. A randomização do decúbito lateral inicial foi realizada com auxílio de uma planilha eletrônica. Amostra de conveniência composta por 16 voluntários consecutivamente recrutados. Para a comparação das médias, procederam-se as análises de variância (ANOVA) e teste t-Student para amostras pareadas, com nível de significância pré-estabelecido de 5%. Resultados: Amostra majoritariamente masculina, com idade média de 67±11 anos, não etilista e não tabagista. A pressão arterial sistólica apresentou queda significativa em decúbito lateral direito e em dorsal (p < 0,01), não se refletindo em alterações na pressão arterial média. As demais variáveis apresentaram oscilações perceptíveis em decúbito lateral esquerdo, sem significância estatística. Conclusão: A mudança de posicionamento em pacientes com síndrome coronariana, hemodinamicamente estáveis é segura e não gera impacto deletério sobre as variáveis oxi-hemodinâmicas. Há de se considerar as repercussões clínicas em ocorridas em decúbito lateral esquerdo para subsídio na tomada de decisão na prática clínica.
Objetivo: identificar alterações hemodinâmicas e/ou oximétricas decorrentes de três técnicas de banho no leito em pacientes cardiopatas internados em uma Unidade de Terapia Intensiva Cirúrgica. Método: Ensaio clínico tipo crossover 3 X 3 no qual foram aplicadas três tecnologias de banho no leito, no mesmo indivíduo, em três momentos diferentes. As intervenções consideradas foram: banho tradicional a 42,5ºC com temperatura da água monitorada e constante; Banho com frasco-ampola de 1L de água destilada aquecido por 1 minuto no forno de micro-ondas em potência máxima sem controle da temperatura; Banho com lenços umedecidos aquecidos por 20 segundos no forno de micro-ondas em potência máxima. Amostra composta por 18 pacientes em pós-operatório de cirurgias cardíacas. Foram analisadas as variáveis frequência cardíaca, frequência respiratória, Pressão arterial (sistólica, diastólica e média), temperatura axilar e saturação de oxigênio por meio de oximetria de pulso. Para comparação das médias, foi utilizado o teste ANOVA Two-way de medidas repetidas com nível de significância pré-estabelecido de 5%. Resultados: Amostra predominantemente masculina, parda e não fumante, submetida majoritariamente à cirurgia de revascularização do miocárdio. Não houve nenhuma alteração significativa nas variáveis estudadas (p > 0,05), considerando os diversos tipos de banho nos diferentes momentos (antes, durante e após o procedimento). Conclusão: Os banhos no leito com água a 42,5°C, com frasco-ampola de água destilada aquecido e com lenços umedecidos, em pacientes pós-cirurgia cardíaca e internados em UTI, não acarretam nenhuma alteração significativa sobre as variáveis oxi-hemodinâmicas.
OBJETIVO: Construir um algoritmo para sistematização das etapas do banho no leito tradicional no paciente adulto internado com Síndrome Coronariana Aguda. MÉTODOS: Desenho descritivo elaborado em consonância com o Projeto integrado “Consumo de oxigênio pelo miocárdio e aspectos hemodinâmicos durante o banho no leito de pacientes com infarto agudo do miocárdio”. A construção do algoritmo foi através do Bizagi Modeler versão 3.0. RESULTADOS: O algoritmo fragmentou-se em duas partes: avaliação e tomada de decisão sobre a temperatura da água a ser empregada no banho e a execução do banho. Este foi dividido em três subpartes, higienização dorsal, lateral e finalização todos com tempo de execução específicos. Para cada subparte considerou-se tempos específicos para a execução do banho. CONCLUSÃO: O algoritmo foi um projeto inovador, podendo ser utilizado tanto na prática assistencial quanto como ferramenta educacional. A etapa subsequente será a validação por experts.
