ResumoEntendendo as visitas domiciliárias enquanto tecnologia de interação no cuidado à saúde da família, este artigo teve por objetivo compreender os significados atribuídos por essas famílias à visita domiciliária realizada pela Estratégia Saúde da Família (ESF), com a intenção de reconhecer as dificuldades e potencialidades dessa prática. Nessa perspectiva, utilizou-se abordagem qualitativa, em que os dados foram coletados através de entrevista aberta e analisados segundo a Teoria das Representações Sociais. A pesquisa foi realizada no território de abrangência de uma Unidade Básica de Saúde da Família (UBSF) localizada na zona leste do município de São Paulo, sendo selecionadas cinco famílias aleatoriamente (sorteio). Os resultados mostraram que a visita domiciliária, apesar de apresentar limitações devido à concentração em torno de práticas curativistas direcionadas aos indivíduos, que tornam secundárias a produção de autonomia e a corresponsabilização das famílias no cuidado à saúde, foi concebida como importante meio de aproximação entre as famílias e a ESF, favorecendo o acesso às ações e aos serviços de saúde, sendo apontada como instrumento de humanização da atenção à saúde ao propiciar a construção de novas relações entre usuários e profissionais e a formação de vínculo entre esses.
Objetivo: Comparar a presença e extensão dos atributos considerados essenciais e derivados da Atenção Primária em Saúde (APS) entre Unidades Básicas de Saúde (UBS) localizadas na zona leste do município de São Paulo que desenvolvem a Estratégia Saúde da Família (ESF) e as UBS tradicionais. Métodos: Todos os médicos (a)s, enfermeiro (a)s e cirurgiões (ãs)-dentistas contratados pela APS Santa Marcelina, que atuam em UBS, foram convidados a participar da presente pesquisa. A coleta de dados foi realizada por meio do PCATool. Resultados: Foi encontrado um melhor desempenho das UBS com ESF em relação aos atributos longitudinalidade (p=0,0330), integralidade (serviços prestados) (p=0,0011), orientação comunitária (p=0,0017) e consequentemente no escore essencial (0,0327) e geral (0,0098), apontando na direção que o processo de trabalho na ESF favorece que tais atributos apresentem melhor desempenho. Conclusão: A Estratégia Saúde da Família tem favorecido agregar qualidade à Atenção Primária à Saúde. Resumo Estudio comparativo de las unidades de salud con y sin la Estrategia Salud de la Familia a través PCAToolObjective: To compare the presence and extension of attributes that are considered essentials and derived from Primary Health Care (PHC) in Health Care Units (HCU) localized in the eastern region of the city of São Paulo, that work under the optic of Family Health Strategy (FHS) and the ones that do not, called traditional units. Methods: All the physicians, nurses and dentists, hired by the PHC Santa Marcelina institution, who work in these HCU were invited to participate in the study. The data collection was realized by the use of PCATool. Results: The results indicate a better performance of the HCU with FHCS with respect to the PHC attributes such as longitudinality (p=0.0330), integrality (of services offered) (p=0.0011), community orientation (p=0.0017) and consequently in the essential score (0.0327) and in general (0.0098). The scores lead us to infer that the work process in the FHCS offers a better setting for the development of these attributes. Conclusion: It can thus be concluded that HCU with FHCS aggregate more quality to Primary Health Care.

A família vem ocupando um lugar central nas políti-cas públicas, principalmente no setor saúde, através da Estratégia Saúde da Família. Essa centralidade foi fruto de um processo histórico e cultural de significações, por vezes normativas, sobre a famí-lia. Nesse processo, situam-se os trabalhadores da Estratégia Saúde da Família (ESF) enquanto atores sociais que constroem suas práticas e concepções sobre famílias dentro de uma realidade social permeada por contradições. Considerando as Representações Sociais como instrumento para identificação de uma realidade social, esse estudo se vale dessa abordagem para descrever as representações sociais de família construídas por trabalhadores da ESF. A pesquisa foi realizada em uma Unidade Básica de Saúde da Família da periferia de São Paulo, com um representante de cada categoria profissional que compõe as equipes de saúde da família. O material analisado demonstrou limitações no processo de formação dos trabalhadores no que se refere aos temas família e comunidade. As representações apontam para uma visão ampliada das configurações familiares, porém percebe-se uma desvalorização das camadas populares. São permeadas por sentimentos de angústia, principalmente quando se referem às famílias atendidas na ESF. Porém, podem-se vislumbrar possibilidades de construção de projetos de saúde eficazes, caso haja investimento na formação desses trabalhadores, visto que eles não se mostram indiferentes às necessidades de saúde das famílias atendidas na ESF.
Background: In ST-segment elevation myocardial infarction, the need for early recanalization of the culprit artery, associated to the difficult access of the general population to medical services that provide such treatment, contribute to increased mortality in such patients. This is the rationale for implementing the LATIN project, acronym for Latin America Telemedicine Infarct Network, in areas characterized by high demographic density and difficult access to healthcare services. The objective of the present study was to analyze the first year results of this project. Methods: The sample included six centers with telemedicine systems participating in the LATIN registry. In cases of acute myocardial infarction, the electrocardiogram and clinical summary were sent to the mobile phones of the interventional cardiologists at the reference hospital. Clinical and angiographic characteristics, intervals between care delivered and hospital outcomes were analyzed. Results: From June 2014 to June 2015, a total of 125 patients were enrolled, mean age of 58.3±11.1 years, 66.4% males, 30.4% suffering from diabetes mellitus and 28.8% smokers. A total of 85 primary percutaneous coronary interventions were performed with a mean door-to-balloon time of 46.4±25.0 minutes. The pharmaco-invasive strategy was adopted in 20.0% of patients. In-hospital mortality was 5.6%. Conclusions: The telemedicine system was a valuable partner in early diagnosis and rapid initiation of the gold standard treatment for acute myocardial infarction in the region analyzed, with possible impact in survival, cost reduction and morbimortality. Diante do exposto, encontra-se o racional de implementação do projeto denominado LATIN, acrônimo para Latin America Telemedicine Infarct Network, em áreas caracterizadas por concentração demográfica densa e dificuldades de acesso à saúde. O objetivo do presente estudo foi analisar os resultados do primeiro ano deste projeto. Métodos: A amostra compreendeu seis centros dotados de telemedicina participantes do registro LATIN. Nos casos de infarto agudo do miocárdio, eletrocardiograma e resumo clínico eram encaminhados por mensagem para os celulares dos cardiologistas intervencionistas do hospital de referência. Foram analisadas as características clínicas, angiográficas, os intervalos de atendimento e os desfechos hospitalares. Background and Objectives: There is a limited evidentiary base on the development of family medicine in different contexts and countries. The lack of evidence impedes our ability to compare and characterize family medicine models and identify areas of success that have led to the effective provision of care. This paper offers a comparative compilation and analysis of the development of family medicine training programs in seven countries: Brazil, Canada, Ethiopia, Haiti, Indonesia, Kenya, and Mali. Methods: Using qualitative case studies, this paper examines the process of developing family medicine programs, including enabling strategies and barriers, and shared lessons. An appreciative inquiry framework and complex adaptive systems thinking inform our qualitative study. Results: Committed partnerships, the contribution of champions, health policy, and adaptability were identified as key enablers in all seven case studies. The case studies further reveal that some enablers were more salient in certain contexts as compared to others, and that it is the interaction of enablers that is crucial for understanding how and why initiatives succeeded. The barriers that emerged across the seven case studies include: (1) resistance from other medical specialties, (2) lack of resources and capabilities, (3) difficulty in sustaining support of champions, and (4) challenges in brokering effective partnerships. Conclusions: A key insight from this study is that the implementation of family medicine is nonlinear, dynamic, and complex. The findings of this comparative analysis offer insights and strategies that can inform the design and development of family medicine programs elsewhere.
Background The shortage and high turnover of physicians is a recurrent problem in health care systems; this is especially harmful to the expansion and full operation of primary health care (PHC). The aim of this paper is to analyze incidence and associated factors with physician turnover in primary health care services in the East Zone of São Paulo City. Methods This is a retrospective cohort study of 1378 physicians over a 15 years’ time period based on physicians’ administrative records from two distinct secondary databases. Physicians’ individual characteristics were analyzed including graduation and specialization. Survival analysis techniques such Kaplan-Meier and Cox Regression were used to analyze the termination of contract. Results One thousand three hundred seventy-eight physicians were included in the study of which 130 [9.4%(CI95 8.0–11.1%)] remained in the PHC services. The mean and median time until the occurrence of the physician leaving the service was 2.14 years (CI95% 1.98–2.29 years) and 1.17 years [(CI95% 1.05–1.28 years)]. The probability of contract interruption was 45% in the first year and 68% in the second year. Independent factors associated with TEC were identified: workload of 40 h/week HR = 1.71 [(CI95% 1.4–2.09), p < 0.001]; initial salary ≤1052 BGI HR = 1.87 [(CI95 1.64–2.15), p < 0.001]; time since graduation ≤2 years HR =1.36 [(CI95 1.18–1.56), p < 0.001]; and the conclusion of residency in up to 3 years after leaving the service HR = 1.69 [(CI95 1.40–2.04), p < 0.001]. Conclusions The time of employment of the physician in PHC was relatively short, with a high probability of TEC in the first year. Modifiable factors such as working hours, starting salary, time since graduation from medical school and need to enter in a residency program were associated with TEC. In pointing out that modifiable factors are responsible for long term employment or the end of contract of physicians in PHC services of the Unified Health System in the periphery of a metropolitan area, the study provides support for the planning, implementation and management of policies and strategies aimed at attracting and retaining physicians in suburban, priority or underserved regions.
