Introduction: Infantile colic is defined as excessive crying for more than three hours a day at least three days a week for one week or more in an otherwise healthy baby. It is most frequently observed in neonates and infants aged two weeks to four months. Objective: To assess the incidence of infantile colic and effectiveness of conventional therapy. Materials and Methods: This was a prospective cross sectional study of 50 cases carried out in Paediatric out patient department in Combined Military Hospital (CMH) Ghatail during the period of September 2010 to August 2012. Data recorded including demographics, clinical presentations, management and the outcome of therapy. Results: Out of the total 50 patients, 24(48%) were male and 26(52%) were female. Male:Female=1:1.08. Babies delivered vaginally suffered more 32(64%), full term delevered babies 33 (66%) suffered more than that of gestational age group <37 wk. Non-exclusive Breastfed (Formula fed, combination of breastfed and formula fed) babies having more incidence of colic 31(62%). Maximum patients 26(52%) had history of cry 4-5 hours per day. Motion (Rocking, bouncing, walking etc) 18(36%), change of diet 14(28%) were considerable satisfactory treatments. Conclusion: Since it is a self limiting condition, conventional therapy should be tried for instant relief. Journal of Armed Forces Medical College Bangladesh Vol.12(1) 2016: 83-87
Introduction: Non-alcoholic fatty liver disease (NAFLD) is a distinct clinicopathologic entity characterized histologically by a spectrum ranging from simple steatosis to steatohepatitis (NASH), cirrhosis and even hepatocellular carcinoma (HCC).Objective: To determine the incidenceand identify the risk factors of NAFLD in non-obese patients.Methods: It was a cross sectional study done in Department of gastroenterology, Nuclear Medicine and ultrasound, Radiology and Imaging, Virology, Biochemistry, BSMMU, Dhaka from March 2012 to June 2014. The patients more or equal to 18 years, non-obese were included. In this study total 190 patients were enrolled.Results: Among them 38 patients were with fatty liver and 152 patients without fatty liver on the basis of ultrasonogram. Where 141 were male and 49 were female with mean age was 49.24 + 9.05 years. Among total, 18.9% had diabetes mellitus, 28.4% had dislipidaemia 24.4% had history of hypertension. Total 18.9% patients having history of diabetes mellitus, normal vs fatty liver disease (20% vs 42.1%, p <0.001) and history of dyslipidemia (10.5% vs 100% , p <0.001), Haemoglobin percentage was 12.16 + 1.32 gm/dL, urine routine microscopic examination glucose present in 9.5% patients. SGPT 56.34 + 16.09 IU/L, SGOT 41.62 + 5.94 IU/L, alkaline phosphatase 99.31+ 19.76 IU/L. We found 6.3% patients were HBsAg positive and no patient was Anti HCV positive.Conclusion: Sedentary life style, Metabolic syndrome, DM, dyslipidameia, are risk factors of NAFLD in non obese person. Elevated liver enzymes are consequences of NAFLD. Though this study has some limitations, it will give some information about emerging liver disease without viral hepatitis.J MEDICINE July 2015; 16 (2) : 89-92
Introduction:Christmas disease (Hemophilia B, factor IX hemophilia) is a rare bleeding disorder due to deficiency of coagulation factor IX 1 . Most commonly factor IX is quantitatively reduced, but in one-third of cases an abnormally functioning molecule is immunologically present. Factor IX deficiency is one-seventh as common as factor VIII deficiency hemophilia but is otherwise clinically and genetically identical.Factor IX deficiency or dysfunction occurs in 1 in 100,000 male births 2 . Accurate laboratory diagnosis is critical, since it is indistinguishable clinically from factor VIII deficiency (hemophilia A) but requires different treatment. Case Report:A 25 years old college student hailing from Mymenshing got himself admitted in BSMMU hospital on 2 nd February, 2009 with complaints of pain and restricted movement of left hip joint for 10 days, recurrent swelling and pain in multiple joint for last 20 years and prolonged bleeding following minor trauma since childhood.On examination patient is mildly anaemic, non icteric, pulse-92/min, BP-110/70 mm of Hg, no edema or lymphadenopathy. On examination of musculoskeletal system, there was wasting of thigh and calf muscles, tone was normal, power was 4/5, movement was reduced in both knee & elbow joints, as well as left hip joint. Nervous system and other system examination reveled nothing abnormal. Christmas Disease (Hemophilia -B) - Abstract:We report a 25 years old man developed Haemarthrosis of left hip joint with a history of recurrent swelling and pain in multiple joints and prolonged bleeding following minor trauma since childhood. Subsequent investigations revealed Christmas disease (Haemophilia B). Hemophilia B is an X-linked bleeding disorder. This case emphasises the importance of considering a diagnosis of haemophilia in a man with unexplained bleeding, even in the absence of a positive family history.[ BSMMU J 2009; 2(2): 90-91]
