Introduction: The most recent study to report Helicobacter pylori antibiotic resistance rates in Bangladesh was published 15 years ago and did not include levofloxacin. We therefore aimed to determine the current antibiotic susceptibility of H. pylori to amoxicillin, clarithromycin, metronidazole, tetracycline and levofloxacin in Bangladesh. Methodology: This study included 133 consecutive patients who underwent endoscopy examination at Dhaka Medical College in November 2014. The serial two-fold agar dilution method was used to determine the minimum inhibitory concentrations of the five antibiotics. Results: Among 56 cultured strains, H. pylori showed high rates of resistance to clarithromycin and metronidazole (39.3% and 94.6%, respectively). Moreover, levofloxacin showed an emerging antimicrobial resistance pattern (66.1%), which was higher in patients with gastritis than that in those with peptic ulcers (p = 0.02). The resistance rate of levofloxacin was significantly higher in patients living in Dhaka city compared to those living in the village (p = 0.049). However, amoxicillin and tetracycline resistance rates were very low. Resistance to both metronidazole and levofloxacin was most commonly observed. Conclusions: The rates of resistance to clarithromycin, metronidazole, and levofloxacin were high in Bangladesh, which suggests that triple therapy based on these drugs may not be useful as first-line therapies in Bangladesh. Alternative strategies such as furazolidone-based triple therapy, bismuth-based quadruple therapies, or sequential therapy may be more effective for patients in in Bangladesh.

Objective: To identify and compare the bacterial community profile of mucosal tissues from a gastric antrum ulcer and a duodenal ulcer in Helicobacter pylori (Hp) positive dyspeptic patients.Methods: Genomic DNA was extracted from the mucosal tissues obtained from 18 patients diagnosed with gastric antrum or duodenal ulcers. A library was constructed using 16S rRNA gene amplification, and Miseq high-throughput sequencing was used to analyse the amplified products. Bioinformatics methods, including operational taxonomic units (OTUs), hierarchical clustering, and a diversity analysis, were performed to investigate and characterize the community composition.Results: The proportion of Helicobacter in the mucosa of patients with a gastric antrum ulcer was significantly higher than that of patients with a duodenal ulcer. However, the diversity of the bacterial community in the gastric antrum ulcer mucosa was significantly lower compared with the mucosa of the duodenal ulcer. There were significant differences in microbial community structure between the gastric antrum ulcer and the duodenal ulcer. Notably, Helicobacter, Prevotella, Neisseria, and Streptococcus were also predominant genera in the bacterial community of the duodenal ulcer mucosa, and they outnumbered those species in gastric antrum ulcer mucosa.Conclusion: The bacterial community composition and the corresponding abundance differ between the mucosal tissues of Hp positive gastric antrum ulcer and duodenal ulcer patients. Additionally, the bacterial community diversity in the mucosal tissues from gastric duodenal ulcer patients is higher than that from gastric antrum ulcer patients, and Helicobacter is not the absolutely predominant genus.

Aims: Helicobacter pylori (H. pylori) is the primary causative agent of peptic ulcer in multiple developing countries, including Bangladesh. This study was designed to investigate the diagnostic value of a rapid immunochromatography-based H. Pylori stool antigen (HpSAg) test to screen H. pylori infection in Bangladeshi population. Methodology and results: A total of 140 suspected peptic ulcer patients who underwent upper gastrointestinal endoscopy at Chittagong Medical College and Hospital, Chattogram, Bangladesh, were included in the study. Histopathology, Rapid Urease Test (RUT), and Microscopic examination of the stained smears were conducted to define H. pylori positive cases. Later, stool antigen detection test was done in H. pylori positive status group, H. pylori negative status group, indeterminate status group, and healthy controls. Out of 140 peptic ulcer suspected patients, 75 (53.6%) patients were confirmed to have peptic ulcer or erosions. Although the proportion of antral erosion was 57.4% in patients who were below 40 years, the proportion decreased to 23.4% in patients over 40 years. Patients over 40 years were mostly suffering from Prepyloric erosion (42.9%). All peptic ulcer patients were also positive on histopathological analysis. However, micorscopic grading of curved bacilli and RUT found 93.3% (70/75) and 89.3% (67/75) patients positive, respectively. High sensitivity (95%), specificity (80%), and diagnostic accuracy (91%) scores for HpSAg assay was obtained in our study. Conclusions, significance and impact of studies: The HpSAg test, for a comparatively less sophisticated assay, can be efficient in detecting the presence of H. pylori pre-and post-therapy and provide more valid test results than other invasive test methods.
