Проанализирован аутопсийный материал легких умерших от коронавирусной инфекции COVID-19 больных (n = 123: 54 женщины, 69 мужчин; средний возраст-71 (30-94) год; продолжительность заболевания-14 (3-65) суток), подтвержденной методом полимеразной цепной реакции. Атлас явился итогом аутопсийной работы патологоанатомической службы Москвы по посмертной диагностике проявлений и осложнений COVID-19 в 2000 наблюдениях – самого большого в мире объема вскрытий умерших от COVID-19. Это стало возможным благодаря тому, что в Москве в 100 % случаев летальных исходов с подозрением на COVID-19 производятся патологоанатомические вскрытия с детальным клинико-морфологическим анализом. Результаты исследований легли в основу новой методики, которая детализирует рекомендации ВОЗ по выбору причин смерти у умерших при COVID-19. Применение на практике этой методики позволило усовершенствовать не только учет случаев смерти от COVID-19, но и оказало существенное влияние на подходы к лечению таких пациентов. Вкладом в теорию патологии является введение понятия клинических и морфологических «масок» COVID-19. В атласе представлен широкий спектр проявлений и осложнений COVID-19, что позволит наметить пути изучения их патогенеза и морфогенеза, совершенствовать методы диагностики и лечения больных, детальную классификацию причин смерти, прямо или косвенно связанных с COVID-19.
За период с 6 марта по 30 апреля 2020 г. в разных стационарах Москвы умерли 543 (219 женщин и 324 мужчины), средний возраст которых составил 67,47±14,67 года. Изучены легкие от 23 умерших с прижизненно подтвержденным диагнозом COVID-19 (ПЦР-исследование на материале из носоглотки, у части умерших положительный анализ ПЦР в материале, полученном из трахеи и ткани легких при аутопсии). Средний возраст этих умерших составил 72,27±10,74 года, длительность заболевания колебалась от 5 до 32 сут. При тяжелом течении COVID-19 у больных возникает вирусная интерстициальная пневмония с развитием диффузного альвеолярного повреждения, которое обусловливает тяжелое течение заболевания, гипоксию и дыхательную недостаточность. Обнаружено неполное соответствие между морфологическими изменениями в легких (фазы диффузного альвеолярного повреждения) и длительностью заболевания, что, вероятно, связано с неустановленной продолжительностью бессимптомного течения у многих пациентов. Течение новой коронавирусной инфекции характеризуется преимущественным повреждением легких, повышением концентрации гемоглобина в крови, снижением SpO 2 , лимфопенией с одновременным нарастанием лейкоцитоза, признаков гиперкоагуляции у крайне тяжелых больных. Возникновение вирусно-бактериальной пневмонии возможно прежде всего у лиц при интубации трахеи с трахеостомой и длительной ИВЛ на любом этапе течения заболевания.
Adrenergic plexuses in the myocardium and adrenal medulla were studied histochemically under conditions of increased left or right ventricular afterload. Under conditions of high afterload not accompanied by heart failure the density of sympathetic myocardial innervation remained unchanged in the loaded ventricle, but increased in the intact ventricle. Comparison of the state of the sympathoadrenal system under conditions of increased afterload complicated or uncomplicated by heart failure revealed common prognostically unfavorable changes: sharp decrease in the density of adrenergic nerve plexuses in the ventricular myocardium and activation of adrenal chromaffin cells.
The production of massive pulmonary embolism in dogs initially results in elevated norepinephrine levels in the stellate ganglion and ventricular myocardia. Six hours after its onset, destructive changes in the stellate ganglion nerve cells are more pronounced than at 1 h and their functional activity is decreased, as are norepinephrine levels in the adrenergic neurons and their terminals. A characteristic feature of compensated massive pulmonary embolism is the presence of many neurons showing pronounced hyperchromatosis and pyknomorphic shrinkage. Key Words: experimental massive pulmonary embolism; sympathetic nervous system; stellate ganglionAlterations in the state of the sympathetic nervous system (SNS) and in cardiac work are closely interrelated and interdependent. We have shown previously that acute massive pulmonary embolism (MPE) is accompanied by considerable hemodynamic shifts in the pulmonary and systemic circulations [1] and by structural and metabolic changes in the ventricular myocardia, and that these changes are most strongly marked when cardiac insufficiency has developed [4,5]. The purpose of the present work was to study the state of stellate ganglion (SG) neurons and of adrenergic nerve plexuses of the ventricular myocardia in experimental MPE complicated or uncomplicated by cardiac insufficiency. MATERIALS AND METHODSThe experiments were conducted on a total of 59 mon~el dogs (body weight 15-20 kg) under natuRussian State Medical University, Moscow. (Presented by V. S. Saverev, Member of the Russian Academy of Medical Sciences} ral ventilation (their chest was not opened) and general anesthesia achieved by fractional intravenous administration of thiopental sodium (20 mg/kg) following premedication with Promedol (10 mg/kg) injected intramuscularly. The procedures used to catheterize the heart and vessels, record hemodynamic parameters, and produce acute MPE were described in our earlier article [1].The dogs with MPE were divided into three groups. The In'st two groups consisted of dogs without detectable signs of circulatory insufficiency (animals with compensated MPE). In these groups, material for neurohistological study was taken after euthanasia with a lethal dose of thiopental sodium 1 h or 6 h after MPE production. The third group comprised dogs in which the onset of MPE was followed by the rapid development of fatal cardiac failure (animals with decompensated MPE); after their death tissue samples were taken for morphological study. The occurrence of the pyocyanic bacillus in the blood and spleen is compared after intramuscular administration to intact and immunized animals. It is demonstrated that blood clearance is performed by transferring the causative agent to splenic phagocytes. It is suggested that antibodies agglutinize the microorganisms in the primary focus of infection and prevent their entry into the blood.
Background: Pathological anatomy, patogenesis and the morphogenesis of manifestations and complications of COVID-19 remain insufficiently studied. The fullest information on structural bases of organs and tissues alterations by new coronavirus disease can be obtained as a result of autopsies. Aims: The aim of the study was to study the morphological changes of lungs and other organs of the autopsies of COVID-19 deceased persons. Results of 2000 autopsies of people who died of a severe form of COVID-19 in Moscow, consisting of 1212 men and 788 women, from March 20 to May 22, 2020 (a ratio 1.54:1) aged from 20 to 99 years (on average 68.515.63 years) were presented. This experience was previously generalized in the Atlas COVID-19 pathology. Autopsies were made in the converted interstationary pathoanatomical offices at strict observance of rules of biosafety according to standard and legal documents of WHO, Russian Ministry of Health and Rospotrebnadzor. Results: Morphological changes of lungs with varying severity and extent were detected in all examined cases; however, damage to other organs was also common, which in some cases prevailed over pulmonary changes and was the cause of death. The main morphological changes in lungs were diffuse alveolar damage and microangiopathy, alveolar hemorrhage syndrome, thrombosis, and thromboembolism. Conclusion: The involvement of the lungs, other organs, and vascular system in the pathological process is a result of multiple factors. It is advisable to implement clinical and morphological masks of COVID-19.
