Background: The digital Auto-Trak™ system is a technology capable of automatically adjusting the triggering and cycling mechanisms during pressure support ventilation (PSV). Objective: To compare Auto-Trak with conventional settings in terms of patient-ventilator synchrony and discomfort. Methods: Twelve healthy volunteers underwent PSV via the mouth by breathing through an endotracheal tube. In the conventional setting, a pressure support of 8 cm H2O with flow cycling (25% peak inspiratory flow) and a sensitivity of 1 cm H2O was adjusted. In Auto-Trak the triggering and cycling were automatically set. Discomfort, effort of breathing, and the asynchrony index (AI) were assessed. In a complementary bench study, the inspiratory and expiratory time delays were quantified for both settings in three mechanical models: ‘normal', obstructive (COPD), and restrictive (ARDS), using the ASL 5000 simulator. Results: In the volunteer study the AI and the discomfort scores did not differ statistically between the two settings. In the bench investigation the use of Auto-Trak was associated with a greater triggering delay in the COPD model and earlier expiratory cycling in the ARDS model but with no asynchronic events. Conclusions: Use of the Auto-Trak system during PSV showed similar results in comparison to the conventional adjustments with respect to patient-ventilator synchrony and discomfort in simulated conditions of invasive mechanical ventilation.
Introdução: O Acidente Vascular Cerebral (AVC) está crescente entre os jovens pela exposição dessa população aos fatores de risco clássicos. Objetivo: detectar as características epidemiológicas, clínicas e tratamento ofertado a jovens com acidente vascular cerebral em um hospital público de referência em Fortaleza. Metodologia: Pesquisa do tipo documental, transversal e de campo, com abordagem quantitativa, desenvolvida no período de junho a outubro de 2018, no Hospital Geral Dr. Waldemar Alcântara (HGWA). As informações foram coletadas em prontuários de pacientes com idades entre 15 a 29 anos, que tiveram AVC nos últimos sete anos. Resultados: Foram analisados 18 prontuários de jovens que tiveram AVC entre dezembro de 2011 a maio de 2018 internados no setor de neurologia do hospital. O perfil que se destaca é de mulheres com média de 25,6 ± 2,8 anos de idade. O AVC isquêmico foi mais frequente. Os fatores de riscos predominantes foram: hipertensão arterial sistêmica, doenças cardiovasculares, sedentarismo e o histórico familiar. As principais características clínicas descritas foram: disartria, hemiparesia e hemiplegia. O tempo médio de hospitalização foi entre 15 a 30 dias. A conduta fisioterapêutica registrada visavam procedimentos afim de recuperar a funcão respiratória e motora. Conclusão: Pôde-se concluir que o AVC isquêmico é o mais comum, e a maioria atingida entre os jovens, são as mulheres hipertensas, cardiopatas, sedentárias e com histórico familiar. Esse tema ainda precisa ser pesquisado, pois este estudo apresenta uma amostra limitada sendo necessário ampliar em mais unidades hospitalares para compreendermos a real situação do AVC precoce em Fortaleza.
A aprendizagem baseada em problemas (ABP) é um tema que necessita de reflexão, notadamente, no que se refere ao ensino de Fisiologia Humana. Como tal, é fundamental abordar novos métodos de ensino que contribuam para que os discentes desenvolvam protagonismo na construção do conhecimento, sendo essa uma necessidade real da disciplina de Fisiologia Humana para que ela passe a ser vista e compreendida dentro de uma abordagem significativa. Assim, o presente estudo objetiva apresentar uma proposta didática desenvolvida com base na ABP, interdisciplinaridade e método ativo de ensino-aprendizagem. Foram elaborados seis modelos de aulas com os seguintes temas: Sistema Muscular, Sistema Endócrino, Sistema Cardiovascular, Sistema Respiratório, Sistema Urinário e Sistema Nervoso. Cada aula contempla: introdução, que apresenta uma breve revisão teórica sobre o tema de estudo; objetivos de aprendizagem, habilidades e competências a serem desenvolvidas; apresentação da questão-problema; elaboração de mapas cognitivos; metodologia de desenvolvimento da aula; e referências (livros, artigos e ferramentas digitais). O método ABP coloca o discente no centro do processo de ensino aprendizagem, por meio de um currículo interdisciplinar e significativo para a sua carreira acadêmica e profissional futura. Contudo, deve ser implementado aos poucos respeitando suas dificuldades e o nível de aceitação do novo método. Espera-se que novas pesquisas possam ser desenvolvidas para ampliar a discussão sobre o método ABP e que tais metodologias possam ser implantadas e estudadas em relação à construção do conhecimento cognitivo e desenvolvimento de habilidades e competências.
