A comparative study was conducted on the psychological features of chronic pancreatitis, peptic ulcer, ulcerative colitis and the irritable colon syndrome by means of psychological tests and interviews. The patients with irritable colon syndrome were found to be the most neurotic of all, whereas those with the definite type of chronic pancreatitis and ulcerative colitis presented prominent alexithymic characteristics. About one half of the patients having peptic ulcer showed alexithymic features. Despite their neurotic tendencies shown on the psychological tests, the patients with the suspicious type of chronic pancreatitis were socially better adjusted than those with irritable colon syndrome. It was also noted that the concept of alexithymia needs to be elucidated by including such factors as over-adaptation to the environment, certain distorted life habits and lack of bodily sensations.
61 patients suffering from chronic pancreatitis were studied in order to clarify the psychosomatic aspects of this disease. They were classified into 5 types, out of which 3 types were examined in detail according to their personality traits and life habits. It was found that the mind-body relationship took the primary form in the psychosomatic and neurotic types, while in the organic type it took the secondary form such as distorted life habits. This finding indicated that the distortion in the personality structure and that of life habits were closely interrelated. The authors also discussed the clinical features of chronic pancreatitis from the standpoint of alexithymia. The patients with the organic type were most alexithymic, which were followed by the psychosomatic type. It was learned that alexithymic problems were reflected in the patients’ life habits.
After the termination of the Second World War, Japan was forced to reorganize its social and value systems. It has been a very difficult task to establish a constructive identity because the Japanese suddenly overthrew the old social establishment and traditional value system. Not only the direct ill-effects of modern industry such as the destruction of nature and also air pollution, but also drastic changes in the socio-cultural background have exerted a serious influence upon the health of the Japanese. I have presented some psychosomatic problems in which such an influence has been reasonably confirmed.
